ScienceDaily (Jan. 7, 2009) — A new University of Colorado at Boulder study shows the resistance of the avian flu virus to a major class of antiviral drugs is increasing through positive evolutionary selection, with researchers documenting the trend in more than 30 percent of the samples tested.

The avian flu, an Influenza A subtype dubbed H5N1, is evolving a resistance to a group of antiviral drugs known as adamantanes, one of two classes of antiviral drugs used to prevent and treat flu symptoms, said CU-Boulder doctoral student Andrew Hill, lead study author. The rise of resistance to adamantanes -- which include the nonprescription drugs amantadine and rimantadane -- appears to be linked to Chinese farmers adding the drugs to chicken feed as a flu preventative, according to a 2008 paper by researchers from China Agricultural University, said Hill.

In contrast, resistance of the avian flu virus to the second, newer class of antiviral drugs that includes oseltamivir -- a prescription drug marketed under the brand name Tamiflu -- is present, but is not yet prevalent or under positive genetic selection, said Hill of CU-Boulder's ecology and evolutionary biology department. The CU findings should help health administrators around the world plan for the possibility of an avian flu pandemic.

The CU-Boulder study is the first to show H5N1 drug resistance to adamantanes arose through novel genetic mutations rather than an exchange of RNA segments within cells, a process known as re-assortment, said Hill. The research on the mutations, combined with molecular evolution tests and a geographic visualization technique using Google Earth, "provides a framework for analysis of globally distributed data to monitor the evolution of drug resistance," said Hill.

The CU-Boulder-led study appears online in the journal Infection, Genetics and Evolution. Co-authors included CU-Boulder Associate Professor Robert Guralnick, recent CU-Boulder graduate Meredith Wilson, Farhat Habib of Kansas State University and Daniel Janies of Ohio State University.

"As these adamantanes have gotten into nonhuman vectors like birds, the positive selection for resistance to avian flu is rising," said Hill. "If Tamiflu is ever used in the manner of adamantanes, we could conceivably see a similar resistance developing through positive selection."

The research team used an interactive "supermap" using Google Earth technology that portrays the individual gene mutations and spread of the avian flu around the globe, said Guralnick of CU-Boulder's ecology and evolutionary biology department. By projecting genetic and geographic information onto the interactive globe, users can "fly" around the planet to see where resistant H5N1 strains are occurring, said Guralnick, also Hill's doctoral adviser.

For the study, the researchers analyzed 676 whole genomes of Influenza A/H5N1 from National Institutes of Health databases of viruses isolated between 1996 and 2007. The team is comparing how often amino acid sequence changes in genes lead to mutations that affect drug resistance in the H5N1 virus and how often such changes evolve into random mutations that don't affect resistance, Hill said.

The next step is to analyze 2008 data, he said.

First detected in China in 1996, the avian flu has spread throughout Asia and to India, Russia, Pakistan, the Middle East, Africa and Europe by various carriers, including poultry and migratory waterfowl, Hill said. While H5N1 is not highly communicable to humans from birds or between humans, experts are concerned future evolution of this subtype or other subtypes, or genetic re-assortment between subtypes, could make an avian influenza strain more contagious with the potential to cause a pandemic.

"Even if H5N1 is not the flu subtype that develops into the next pandemic, this technique can help us understand the properties of flu viruses and we can use these methods to track mutations in other viruses," said Guralnick. "We can harvest genetic influenza data and monitor it in near real-time, which should give this project some traction to help governments make decisions on managing potential pandemics."

Like the legend of a road map, colors and symbols on the supermap indicate which types of hosts carry the virus or the distribution of genotypes of interest, said Hill. A click by users on viral "isolates" generates computer windows revealing H5N1 mutations linked to positive genetic selection resulting from the spread and use of adamantanes.

The information is linked by computer to the National Institutes of Health's GenBank, a database with more than 75 million sequence records.

According to the Centers for Disease Control in Atlanta, an avian flu pandemic could kill millions of people in America, infect 15 percent to 35 percent of the population and cost well over $100 billion.

Mopping operation begins in Siliguri

SILIGURI, Jan. 7: Mopping up operation began today in the bird flu affected areas of Siliguri sub-division as a follow up to the culling process which started last Saturday. The operation has been smooth so far, according to the district and sub divisional officials. Mopping operation also started at the Rajganj block of Jalpaiguri district today.
About 30,000 birds were killed in course of the four-day culling operation, which ended yesterday. The bird killing campaign is expected to be over by tomorrow evening with the completion of the mopping exercise.
The Darjeeling district magistrate, Mr Surendra Gupta, said today that around 700 birds were slaughtered today, the first day of the mopping operation in the Siliguri sub-division. “The exact figure would be higher as reports from all the teams are yet to reach here,” he said.
Mr Gupta further said that the bird elimination campaign would be over with the mopping up tomorrow. “The sanitisation and the monitoring process would start day after tomorrow and these would continue for some days till the monitoring officials to be deputed by the Centre and the state government give the final certificate signaling the end of the entire process,” the Darjeeling DM said.
Briefing for the Jalpaiguri district the ADM (general), Mr NG Hira, said that around 1,500 birds had been killed in the course of culling at the Rajganj block. “Mopping has started today and it would be completed tomorrow,” he said.

PARIS: The world's paramount authority on trade in farm animals on Wednesday urged countries not to let up in the fight against deadly bird flu.

Outbreaks of H5N1 avian influence among poultry fell last year, as did the toll among humans who came into contact with infected birds, but the story is far from over, said Bernard Vallat, director general of the World Organisation for Animal Health, or OIE.

"There is still a risk from the poultry trade, because the virus has become endemic in some countries that have been unable to get rid of it," Vallat told reporters.

Vallat singled out Egypt and Indonesia as countries where, he said, veterinary surveillance was insufficient.

In China and Vietnam, said Vallat, outbreaks of H5N1 had been managed thanks to systematic vaccination of poultry flocks.

"However, this is expensive and will have to stop one day," he said.

H5N1 bird flu has now killed 248 people since it re-appeared in Asia in 2003, according to the the latest World Health Organisation (WHO) tally.

Of these, 113 died in Indonesia, 52 in Vietnam, 21 in China and 17 in Vietnam.

Deaths last year numbered 31, after 59 in 2007 and 79 in 2006. Eighteen occurred in Indonesia, five in Vietnam and four each in Egypt and in China.

Scientists fear the virus could eventually mutate into a form more easily transmissible between humans, triggering a global pandemic.

Despite the declining human toll, "the theoretical risk of the virus mutating and becoming a killer is still there," said Vallat, who made the comments in a New Year's meeting with reporters in Paris.

Vallat said it was in the interest of rich countries to beef up veterinary surveillance in poor countries.

As a result of globalisation, the spread of an animal-borne disease to other economies can have disastrous consequences, he noted.

On Monday, a 19-year-old Chinese woman died of H5N1 in Beijing after coming into contact with ducks at a poultry market. Authorities in Vietnam also announced on Tuesday an eight-year-old girl had tested positive for H5N1 in the north of the country. - AFP/de


It is E Coli; Not the flu !

A large number of domestic fowls (hens) have reportedly died over the past few days in Misao Lhanvum village located near Khuga Dam in Churachandpur following outbreak of Colibacillosis, E-coli even as more than 40 villagers have been hospitalised with symptom of high fever.
Taking serious note of the matter, the Director of Veterinary and Animal Husbandry and the Director of Health Services have studied the situation.
In connection with hospitalisation of many villagers, the officers, have, however, ruled out any relation with Bird flu.
On receiving the report yesterday on the dead of many hens, specialist (disease) of the Veterinary and Animal Husbandry Department Dr Konsam Gopal was despatched to the village for collection of the dead fowls’ samples.
Talking to mediaper-sons in this connection today, Director of Veteri-nary and Animal Husban- dry Th Dorendro informed that the collected samples have been examined at the Disease Investigation Laboratory of the Department and confirmed that the hens have died from Colobacillosis, E-coli.
Explaining that Colibacillosis, E-coli is a disease of the hen common in winter and rainy season, the Director said that some of the affected hens have been cured after administering tetracycline.
The Director further informed that in all 16 hens have reportedly died in the village in between January 4 and 6.
Dissuading the apprehension over possible outbreak of Bird flu that led to the dead of the domestic fowls, the officer said that the dead of the hens in the village have nothing do with Bird flu. Nonetheless, as part of the Bird flu surveillance, the Department would be sending the samples of the dead hens thus collected to the Eastern Regional Disease Diagnostic Laboratory for further examination.
In view of the outbreak of Bird flu in Assam, import of poultry products including hens, eggs, chicks and chicken feeds from Assam is currently under restriction.
According to the Director, there are in all 100 hens and 47 ducks in the village located at a distance of 7 kms from Churachandpur district headquarters. There are in all 70 households with a population of 486 people in the village.
On the other hand, Health Director W Motilal have clarified that hospitalisation of over 40 villagers in the District Hospital of Churachandpur is due to high fever and it has nothing to due with Bird flu.
The patients are being treated with paracetamol and antibiotics and four of them have been discharged from the hospital today
. The Additional Director (Public Health) Dr Th Bhubonchandra who had gone to the village today also reported back that no symptom of Bird flu have been detected from any of the patients, the Director added.

38 hospitalised in Ccpur village due to unknown disease

An unknown disease has plagued the residents of Misao Lhahvom since Sunday night. The village is located around 8 kms from Churachandpur police station in the south near the Khuga dam canal.
The epidemic has affected the villagers since Sunday night resulting in the hospitalisation of 16 persons within 24 minutes. However, as more and more people are being affected by the seemingly contagious disease, the number of patients till now has risen to a whopping 34 till 8.30 pm yesterday and 38 by today, as 4 more have been admitted to the district hospital.
According, to one of the villagers attending on the patients, on Sunday night one Nemneilam, 18, was the first one to be attacked by the disease. She had a high fever and soon after lost consciousness. Panicking at this development many people from the village arrived at her home to share the anxiety of the family members.
But the visitors were also not spared and suffered the same fate on returning home, he said adding that the number of people suffering was on the rise.
Within 3 to 4 hours during the night the number of patients crossed 10, he said.
The villager Lunthang told the IFP that there was one auto rickshaw in the village and using this the villagers brought many patients to the district hospital.
During all this while, some local NGOs like the ZYA and the KKL assisted them in bringing the patients to the hospital.
Yesterday , a medical team headed by the CMO Dr. Shingkhojam Haujel and Paulginthang from the malaria department went to the village and made an assessment of the situation. The team took the blood samples of 130 people and later the veterinary department officials also visited the village.
The DC Churachandpur, Sumant Singh, meanwhile, paid a visit personally to the patients languishing at the hospital and also sent a message to the additional secretary health urging him to dispatch a medical team urgently to the affected area.
The team which reportedly arrived at the village in compliance with the official call reached here around 4 pm and went straight to the village and thereafter returned to Imphal after collecting some blood samples. The team was led by Dr. K Gopal and it assuaged apprehensions among the local people that the disease could be bird flue, while stating that it was in fact a bacterial disease.
Meanwhile, Dr. T Hatzaw, the medical superintendent, also urged the additional director of the health department to conduct an investigation to find the cause of the disease.
A local body ZEPADA, meanwhile, has urged the officer in-charge of NICD, Plague Surveillance Unit, New Delhi to take immediate steps in the matter.
Today, Th Bubhan Chandra Singh, the additional director of public health and state nodal officer, ISDP visited Churachandpur and had a meeting with the DC and some other health officials in the district.
All the patients’ blood samples were collected today once again and are now under surveillance. However, in spite of all these efforts there is still no confirmation on what the disease was and how it had been caused.
Some blood samples that were tested yesterday were negative and there are no cases of malaria, according to the medical superintendant of the district hospital.

Beijing on alert after bird flu death

AGRICULTURE officials have issued an alert against bird flu after a 19-year-old woman died of the disease in Beijing.

Workers disinfected the Yanjiaoqingong Market in Sanhe, Hebei Province neighboring Beijing, yesterday morning. The woman bought nine ducks at the market on December 19 and died of bird flu on Monday in a Beijing hospital.

The market's five shops selling live poultry have been closed.

The government of Sanhe City, under the jurisdiction of Langfang, has set up an emergency group headed by the city's Communist Party Chief Li Gang to deal with bird flu prevention, quarantine and market-regulation issues.

Local health authorities examined 15 people engaged in the live poultry trade in the market, and all of them were free of the disease. The authorities also surveyed city residents who had been diagnosed with fever and all poultry farms, and found no problems.

Beijing has banned live poultry from other parts of the country from entering the city. Experts have begun to inspect the city's slaughterhouses and poultry farms.

No domestic fowl were kept within 10 kilometers of the Sanjianfangdong Village of Chaoyang District in Beijing where the dead woman lived, said city officials.

In Tianjin, the suspected origin of the ducks, authorities also started bird flu prevention measures. Experts from the Ministry of Agriculture and the city have gone to Jixian County to inspect the poultry market. They found no signs of bird flu.

Bird flu scare in Manipur scuttled

IMPHAL, Jan 7 – Setting to rest the speculations of the outbreak of bird flu in Manipur, Dr Rajen, officer of Disease Investigation Laboratory has clarified that the death of the chicken was due to a water- borne disease.

Cases of death of about 16 chicken in Churachandpur district over the last four days were due to water-borne disease known as ‘Colibacillosis’ which is a common disease, asserted Dr Rajen. “We came to know of the result after the joint director of the Veterinary department of Churachandpur sent a sample to the Directorate,” affirmed Dr Rajen.

However, not ruling out a possible outbreak of the flu in the near future, the officer of the DI Lab informed that surveillance work was under process to check any possible outbreak in the future. He also observed the need for steps and measures to check the flu and warned that Manipur was a very sensitive place for the bird flu.

China raises bird flu alert after woman's death

The woman, who lived in a densely-populated area of Beijing known as Chaoyang District, fell ill on Dec. 24, was sent to the hospital three days later and died on Jan 5. The death was the first in China attributed to bird flu since February last year.

Municipal officials have also informed civilian agencies of the mater and raised the alert level at local hospitals. The five stalls selling live poultry in the market, located in the province bordering Beijing to the south, have been closed.

Commercial air travelers landing in Hong Kong from Beijing will hear a special announcement on bird flu. Passengers traveling to Beijing will be warned to avoid attending festivals marking the Lunar New Year where live birds are slaughtered, the South China Morning Post reported.

Beijing bird flu patient dies

A 19-year-old woman has died of bird flu in the Chinese capital
Beijing. It is the 1st time in almost a year that someone in China
has died after being infected with the H5N1 strain of avian influenza
[virus], which is dangerous to humans.

The woman was admitted to hospital on 24 Dec 2008, and Chinese media
report she had bought a duck at a market outside Beijing. She is
thought to have become unwell after slaughtering the bird.

The bird flu virus is passed on via direct contact with infected
birds or their droppings. Experts fear the virus could mutate into a
strain transmitted from human to human. Since the H5N1 strain
surfaced in Asia in 2003, about 250 people have died as a result of bird flu.

all were negative.

case and the 248th death worldwide. The 19-year-old woman is the 31st
case of H5N1 avian influenza to be confirmed in China, and the 21st
death. The most recent previous case of H5N1 avian influenza in China
occurred in February 2008.

Although resident in Beijing, the victim appears to have contracted
infection from a fowl purchased in a market distant from her district

China issues bird flu alert after woman dies

YANJIAO, China (Reuters) - China issued a bird flu alert Wednesday after a woman died of the virus, the first such death in the country in almost a year, and closed poultry markets for disinfecting in a province surrounding Beijing.

The 19-year-old woman died of the H5N1 virus after coming into contact with poultry in Hebei province, bringing the total death toll in China to date to 21.

In Hebei's Yanjiao, where the dead woman had bought ducks, poultry markets were closed and the sale of live birds stopped as workers in masks and white coats sprayed disinfectant.

The World Health Organization said it appeared to be an isolated case.

"We are concerned by any case of human H5N1 infection, however, this single case, which appears to have occurred during the slaughtering and preparation of poultry, does not change our risk assessment," the WHO said in a statement.

"WHO expects the ministry will continue to keep it updated on this case, and is prepared to offer technical assistance if requested," it added, referring to the Health Ministry.

The virus is generally more active during the cooler months between October and March, although the new Chinese case points to holes in surveillance of the virus in poultry.

Chinese Health Ministry spokesman Mao Qunan was quoted in state media as saying the government would step up monitoring.

"This year we must, on the basis of what we have done in the past, increase monitoring for the transmission of the highly pathogenic bird flu virus in humans," Mao said.

In Beijing, workers fanned out to inspect poultry markets and slaughterhouses in the capital city after the government issued a bird flu alert, the official Xinhua news agency said.

NO POULTRY OUTBREAK

Paul Chan, microbiologist at the Chinese University in Hong Kong, said it was worrying that this case was not accompanied by the detection of the virus in poultry nearby.

"The source of this infection seems to be poultry or the market (where the girl bought the ducks). If that is true, we need to know why we missed the outbreak of the virus in poultry or in the market," Chan said.

"If there was an outbreak in the market, there should have been large numbers of poultry deaths. If people in the markets and the government can't recognize this, then we have a serious problem on our hands," he added.

The H5N1 strain remains largely a disease among birds but experts fear it could change into a form that is easily transmitted from person to person and spark a pandemic that could kill millions of people worldwide.

CIDRAP: China, Vietnam report new H5N1 cases

Jan 6, 2009 (CIDRAP News) – Public health officials in two countries today announced new human cases of H5N1 avian influenza, involving a 19-year-old Chinese woman who died of her infection and an 8-year-old Vietnamese girl who is recovering.

In Beijing, local health authorities said the woman died yesterday after getting sick on Dec 24 and being hospitalized 3 days later, Xinhua, China's state news agency, reported today. Tests at two labs revealed she was infected with the H5N1 virus.

If the World Health Organization (WHO) confirms the case, she will be listed as China's 31st H5N1 case-patient and 21st fatality. Her illness marks China's first human case since February 2008.

The Beijing Municipal Health Bureau told Xinhua that the woman bought nine ducks at a market in Langfang city in neighboring Hebei province on Dec 19. She removed the ducks' internal organs and then gave three of the birds away to family and a friend.

Zhao Qingchao, a Langfang City official, said investigators found that 13 people ate the ducks but only the woman got sick, Xinhua reported. He said the ducks were from Jixian county in northern China

Beijing's health bureau said 116 people had close contact with the woman and that 102 of them were medical workers, according to the Xinhua report. One nurse who had contact with the patient had a fever but has since recovered.

The WHO's office in China released a statement saying the woman's death from the H5N1 virus should not prompt alarm, Agence France-Presse (AFP) reported today. "We are concerned by any case of human H5N1 infection. However, this single case, which appears to have occurred during the slaughtering and preparation of poultry, does not change our risk assessment," the WHO said.

Meanwhile, an official from Vietnam's Preventative Medicine Department told AFP that the 8-year-old girl, from Thanh Hoa province in northern Vietnam, got sick with pneumonia on Dec 27 after eating poultry and was hospitalized on Jan 2. If her case is confirmed by the WHO, she will be listed at Vietnam's 107th case-patient.

Nguyen Huy Nga, who directs the department, said tests revealed the girl's H5N1 infection on Jan 3. Her case is Vietnam's first since March 2008.

An official with the provincial health department said he expected that the girl would be discharged from the hospital soon, according to the AFP report.

Nguyen Huu Dinh, an animal health official in Thanh Hoa, said infected poultry had been detected and culled in the province, AFP reported.

Jeff Gilbert, an avian influenza expert with the United Nations Food and Agriculture Organization (FAO), told AFP that Vietnam is entering a high-risk season because of cold weather that favors virus survival and the Tet (lunar new year) celebration, which often includes eating poultry dishes.

"China has the (lunar) new year coming up, so the situation would be the same," Gilbert said in the AFP report.

If the WHO confirms the new cases reported today, they will raise the global H5N1 total to 393 cases with 248 deaths.

The number of H5N1 cases and deaths in 2008 was the lowest since 2003, when the lethal H5N1 virus began causing outbreaks internationally, according to WHO figures. In 2008 there were 40 cases and 30 deaths, down from 88 and 59 in 2007, 115 and 79 in 2006, 98 and 43 in 2005, and 46 and 32 in 2004.

Although there were fewer cases in 2008, the 75% case-fatality rate for the year was the highest since 2003, a moderate increase from 2007's case-fatality rate of 67%.

Transmission to a health care worker is rare. Most H5N1 H2H transmission involves an index case and family members who are not trained in universal precautions, and have minimal protection. Therefore, independent confirmation of this transmission would be useful.
