Vietnam: Quang Ninh Prov.: Disclosure of avian influenza in Tien Phong commune (Hung Yen)

'We are not seeing any significant evidence of a resurgent ‘third wave’ As the deadly H1N1 flu is making a comeback in places around the world, including in the southwest United States – Georgia in particular, which has been hit particularly hard – authorities are wondering whether these new outbreaks of the influenza virus represent a worrisome “third wave,” a new mutation, or whether something else is in play that’s responsible. Alabama and North Carolina also have reported outbreaks, Centers for Disease Control and Prevention (CDC) officials reported in their latest update on the spread of the virus. CDC warned Georgians to get vaccinated for the H1N1 flu. Georgia has one of the lowest rates of vaccination among its population, which some experts believe is likely contributing to the increase in new infections there. Local activity has also been reported in Arkansas, Louisiana, Mississippi, Tennessee, Virginia, Hawaii, New Mexico and Puerto Rico. Across the world in Malaysia, new A H1N1 clusters have been detected throughout the country, putting the country’s Health Ministry on high alert. Between March 21 and 27, 83 people in Selangor, Malacca and Terengganu were confirmed infected with the virus. Health Director General Tan Sri Dr Ismail Merican said the sudden clusters indicate the H1N1 virus is still active, but stressed that the government is on top of the new outbreaks. The World Health Organization (WHO) reported that the H1N1 pandemic influenza remains the most prominent influenza virus circulating around the world and that the virus remains active in parts of the tropical zones of Asia, the Americas, and Africa. Overall, WHO’s case count now stands at 492 documented cases, with 291 associated deaths.” Authorities continue to be concerned that as pandemic H1N1 comingles with the highly fatal H5N1 bird flu there could be a genetic “reassortment” of the two viruses, potentially resulting in a deadly new flu virus. Writing in his latest bulletin, Michael Osterholm, director of the Center for Infectious Disease Research & Policy (CIDRAP) and director of the National Institutes of Health-supported Center of Excellence for Influenza Research and Surveillance within CIDRAP, said “recent novel H1N1 influenza developments in the southeastern region of the United States merit … attention. They're prime examples of why we must vigilantly expect the unexpected. Osterholm pointed out “an unusual level of hospitalizations linked to lab-confirmed novel H1N1 influenza in the state of Georgia prompted CDC to call a short-notice press briefing and describe the uptick as a ‘worrisome trend.’” “Most of the hospitalized patients are adults who have chronic medical conditions and did not get the H1N1 vaccine,” Osterholm wrote, adding “Georgia had one of the lowest rates of H1N1 vaccination last fall, and people with chronic medical conditions are among the groups considered at high risk of developing complications if they're infected by the novel H1N1 pandemic virus. “I don't know the last time in April we've seen an increasing number of hospitalizations for influenza,” Osterholm stated, adding, “of note, the number in Georgia is similar to the number of hospitalizations seen at the height of the second pandemic wave last October. In short, the trend is unexpected. Does it portend a third wave of the pandemic? We just don't know … But I sure wouldn't rule out the possibility.” “For now, what we do know is that what's happening in Georgia is unusual for this time of the year and that people who are getting sick enough to need hospital care did not get vaccinated and are among the groups considered to be at high risk of developing complications,” Osterholm wrote. "Yes, yes," said Dr Samlee Pilanbangchang, regional director, South-East Asia, WHO, when asked whether the swine flu scare was unnecessarily exaggerated.
He was addressing an function here on the eve of World Health Day.
When the WHO official made the candid admission his colleagues in the organisation intervened and he did not speak further on the matter.
WHO was criticised last year for its handling of the swine flu pandemic and was accused of exaggerating the danger of the virus under pressure from different countries and drug manufacturers.

Laboratory tests confirmed he was positive for H1N1. However, he also experienced problems, uncontrolled diabetes, the complications caused "severe metabolic acidosis" due to total kidney failure, other bacterial infections (sepsis) and diabetes was not controlled. X-ray of the lungs did not show any signs of pneumonia to be associated with H1N1. So his death was directly caused by complications of diabetes.

KOTA BHARU: A total of 40 trainees of the National Service Training Program (NS) at Camp Ethnobotany, Gua Musang, quarantined since yesterday afternoon after experiencing symptoms of disease detection, such as influenza (Inclusion).
All students involved are now placed under strict surveillance Gua Musang Hospital and other health workers involved in the camp is not allowed to interact and mingled with 310 other trainers at the camp said.

ALOR SETAR: A fourth school in Langkawi has closed after its pupils were found to be suffering from influenza-like illness.
The Kedah Education Department closed SJKC Chung Hwa after 40 pupils were found with the illness.
Department director Shahidan Abdul Rahman said the school would be closed for five days beginning today.
“This is the fourth cluster of influenza-like illness in Langkawi and the Health Department is closely monitoring all schools on the island.
“Last week, SMK Langkawi Pulau Tuba, SK Bagan Nyiur Selat and SMK Teknik Langkawi were ordered closed between five and six days,” he added.
Shahidan said SMK Langkawi Pulau Tuba was re-opened on Sunday, SK Bagan Nyiur Selat was re-opened yesterday while SMK Teknik Langkawi would be opened today.
It is learnt that the state Health Department is also closely monitoring SMK Mahsuri and Maktab Rendah Sains Mara after more than 20 students from both schools were treated for flu yesterday.
Meanwhile, Kedah Health Committee chairman Dr Ismail Salleh said 200 students from SMK Langkawi Pulau Tuba, SMK Teknik Langkawi and SK Bagan Nyiur Selat were confirmed positive for the A(H1N1) virus.
“All of them were given anti-viral drugs and then quarantined at home.
The state had earlier recorded a total of 14 A(H1N1) cases with no deaths recorded.
“The state Health Department will closely monitor all schools, higher learning institutions and colleges,” he added yesterday.

JASIN: Family of two who died after birth confirmed infected with the epidemic of Influenza A (H1N1) on Tuesday, while quarantined in their homes in Batu 14 Jalan Jasin, Merlimau near here, to avoid spreading the disease.
Chairman of the Committee of Women, Family and Health states, Datuk Norpipah Abdol, Jasin said the Health Department is directed to monitor the cleanliness of the home involved while ensuring that residents are not independent of external activities. He said his house was also ordered residents to wear face masks at all times when outside the home than they need to keep the house clean of any contamination. "They should not leave home independently until certified safe by the State Health Department.
"I hope that the family comply with all instructions from the relevant parties," he said when contacted here yesterday.
The newspaper reported yesterday, Maimunah Dimah, 54, died on Wednesday last before, followed her second child, Sumarni Md Nor, 30, last Friday due to H1N1 infection.
More sad family involved when Sumarni not find time to see a small child she brought a new, four days ago that had to be placed in the Intensive Care Unit (ICU) Melaka Hospital unconscious shortly after undergoing surgery to remove her baby.

Wife’s last wish to hubby — look after family and mum
MALACCA: Be strong and don’t worry, these were the last words Mustaza Sarmat, 29, said to his wife Surmani Mah Nor as he held her hands before she underwent a Caesarean operation to save their child at the Malacca Hospital last Wednesday.
“I was by her side. I told her to be strong. I even promised her that both of us would raise our family together,” a tearful Mustaza said as he related his last moments with his wife who slipped into a coma during the operation and died two days later on April 2.
“She told me to look after our family and her mother,” he added. Surmani, 29, and her mother, Maimunah Dimah, 54, who died on March 31, had both contracted the A(HINI) virus.
He said that Surmani, who had a mild fever and slight cough, had sought treatment at a private clinic near their home in Jalan Jasin, Merlimau.
He added a routine pre-natal check up at the Malacca Hospital on March 29 revealed the baby’s heart was beating rapidly, prompting doctors to perform a Caesarean to save the girl.
The 2.95kg baby is free from the A(HINI) virus and is in an incubator at the hospital.
“We have yet to name her,” said Mustaza, a contract worker with Indah Water Consortium in Muar.
Meanwhile, Maimunah’s husband and Surmani’s father, Mah Nor Mah Nasir, 65, was still in shock over the family’s double loss but accepted it as fate.
He said the family had not told Surmani’s three-year-old daughter Nuraira Batrisyia of her mother’s death.
He added that Mustaza’s family and his youngest daughter Fatimah, 17, were put under house quarantine for a week since Friday and that the authorities were monitoring their condition daily.

More bird flu cases arising in Vietnam
Another person suffering from a severe case of avian influenza is being treated at the National Tropical Diseases Hospital in Hanoi, the hospital announced on April 5.
A 22-year-old male from the northern province of Bac Kan, the sixth bird flu case since the beginning of the year, is in serious condition and requires the use of a breathing machine, said the hospital’s director Nong Quoc Chi. The patient was transferred to the hospital from the province’s General Hospital on April 3 with symptoms including a cough and high fever. Tests conducted by the National Epidemiology Institute on April 4 showed he was positive for the bird flu virus (A/H5N1). An investigation has revealed there were dead waterfowl in the man’s neighborhood. Four other residents from the man’s commune also showing bird flu symptoms have been quarantined and are also under treatment at the National Tropical Diseases Hospital. Bird flu has claimed two lives in Vietnam so far this year. |

A bird flu case detected in Bac Kan province

The patient, Trung Van Hoa, 22, from Na Tao hamlet, Nhu Co commune, Cho Moi district, is currently in critical condition. He is being treated at the Central Hospital for Tropical Diseases.
The patient got sick on March 30 with symptoms of high fever and cough. He was admitted to Bac Kan provincial hospital, and then sent to the Central Hospital for Tropical Diseases on April 3 where he tested positive for the lethal strain of A/H5N1 virus.
At present, four other people in Nhu Co commune show similar symptoms of high fever and cough. They are under quarantine and being treated at Bac Kan provincial hospital.

Malaysia: Johor Bahru: 40 H1N1 Confirmed Cases

JOHOR BARU: A total of 40 patients in Johor have been confirmed positive for A (H1N1) virus this year.
State Health department director Dr Mohd Khairi Yaakub said that all the cases, which were recorded in the first three months this year, were transmitted locally and no deaths were reported.
“All the infected patients have been quarantined.
“These patients are being treated at the respective hospitals in the state,” he said adding that, the state recorded a total of 704 positive cases were recorded last year.
Dr Mohd Khairi, who was speaking to newsmen after attending the launch of the Malaysia Clinic in Taman Megah Ria here, said the department was continually conducting checks at various checkpoints in the state to prevent the virus from spreading.
He also reminded the people to take precautionary measures such as going for medical check-up when experiencing coughing or flu and wearing masks.
“The people must not be complacent by thinking that the virus is not around anymore.
“They must always be on their guard to prevent infection or spread of the virus,” he said.
Meanwhile, he also announced that the state had already recorded 691 dengue cases this year, resulting in three deaths.
“Two of the patients died in Segamat while another in Kota Tinggi.
“It is a serious matter and thus, the people must play their part in curbing the spread of dengue,” he said.
He added that the people must ensure the cleanliness of their homes or premises to prevent the breeding of mosquitoes.

The available data, though sparse, requires the simple evaluation that this pandemic reservoir is not approaching stasis. The background continues to widen within the existing clades adding polymorphisms from H5N1 and 1918 strains demonstrating that ΣPF11 is far from reaching Omega status. Movement onto this cross-linked background has accelerated recently, now encompassing 2% of all available pandemic sequences.
Areas covered include Texas (3), the Northeastern United States (11), Wisconsin (7), Oregon, Washington D.C., Kentucky, Indiana, North Carolina (5), China (2), Singapore, India, Australia (2), the Ukraine (23), Moldova (5), Russia (18), Iraq (3), Denmark(3), Norway (2), Sweden (3), Poland, Greece and extensive penetration in Spain (17 sequences).

But you must realize that the iota, the jot and even the title, will be considered of value in this forthcoming exercise. Some in the science community appear to find the process of discovery by observation just too tedious. If you are of that mindset, please direct your attention to any of the many “Science for Hire” venues where illogical, but comforting, summations are drawn from cleverly manipulated data streams. Science sells today. But we sometimes speculate that science is becoming just another sell-out, an industry pursuing profit over knowledge?
On the other hand, if careful examination, even hard work, is part of your daily lab book, you'll find evidence in this discussion to introspect.
Leading public health officials continue even today to indicate that the pandemic virus is unchanged. Keep that statement in mind as you read this analysis of heavy change activity in the pandemic reservoir, watching closely for the recent branching into genetic areas matching H9N2.
Also keep in mind that even single changes have demonstrated substantial importance.
Ongoing worldwide studies duplicated by independent, geographically dispersed laboratories, including the labs of those noted public health officials, indicate that a single polymorphism may produce a significant outcome within a Pandemic Influenza reservoir. Numerous accounts of important revisions are on record across a vast geography. 225G has produced a Vaccine Escape event in LvivN6, an elevated Case Fatality Rate and a substantial number of severe outcomes. Individual changes between 157 and 159 have also demonstrated “low reactor” status and registered as Vaccine Escape events. 230I entered the H5N1 Gharbiyah cluster in Egypt and the resultant Case Fatality Rate was 80% for that strain, a substantial increase from the accepted H5N1 CFR.
Three separate single genetic changes are well characterised in leading to various forms of Anti-Viral resistance. TamiFlu Resistance has now dispersed widely in a pattern following the rapid spread of resistance in seasonal influenza via the same Single Nucleotide Polymorphism coding for 275Y on the Neuraminidase. Emergent and attractant H1N1 strains (Triple Reassortments again) co-circulating alongside the pandemic reservoir also carry 275Y. H5N1 inclusions increase in the pandemic reservoir with a continuous flow. The PB2 627K is confirmed as leading to increased replication speed in humans and has recently been documented in a second pandemic sequence. As we have maintained from the beginning of this pandemic, TamiFlu Resistance and 627K are foregone conclusions according to our calculations.
Our team noted an increase in Avian inclusions and recently predicted HA 230I for ΣPF11. The polymorphism was documented last week in Wisconsin (US) on a background that does not deter transmission. The geographic area has a notable increase in cross-linked activity. The individual sample, A/Wisconsin/629-D00337, is primed for spread with 2 additional HA markers, 275A (TX, NM, CA and Sweden) and 377K (aggressively emerging). The NA carries 220K, a marker found on one cross-linked sequence from the Ukraine and across a wide US geographic pattern from December 2009 to the most recent 2010 sequences (North Carolina, New Hampshire, Wisconsin and Nebraska). We consider the Wisconsin0337 sample to be an excellent candidate as a universal donor of sorts due to limited polymorphisms and the fact that each revision is currently an emergent change.
H9N2, a "bird flu" serotype, has been recently evaluated for human pandemic potential. By all appearances, the present H9N2 reservoir is not becoming a pandemic virus, but is very much influencing the genetic acquisition cycle of the currently circulating PF11 pandemic virus. Which is worse, another species-jumping serotype entering the fray or a combinational virus with genetics from multiple pandemic potential reservoirs appending onto the current partially-adapted virus? In either case, the Hydra Effect appears to be operating in full force.
Given this information, obvious questions begin to form. If one genetic change may potentiate variant clinical outcomes, drug resistance or Vaccine Escape, what is the potential for multiple changes producing a variant outcome? And if a single genetic change and/or multiple changes are capable of producing large changes in behaviour, should we, in turn, have a very high and accurate level of surveillance on this reservoir? And what should we do with that important information? Should our public health officials speak from a platform of candor and accuracy when carrying out the responsibilities of their trusted positions?
Let’s investigate now if the viral reservoir is, in fact, unchanged?
More than 80% of the Hemagglutinin positions between 188 and 244, including antigenic areas of the RBD, are on record as polymorphic. Many positions rate multiple changes. Tracking these revisions against various baselines has informed our studies over the past year. One particular range of those variations is documented in the trailing data. For the sake of brevity, only one section of the HA gene segment is discussed. The list is certainly not comprehensive.
The positional summary may perhaps be instructive for those who hold the belief that the pandemic reservoir is stable. The report may provide reversing guidance for those who make public statements to the effect that pandemic H1N1 is not changing. Repetition of their “All Clear” social messaging cannot reverse the weight of the actual data. Repetition of an untruth does not create a fact, but that ongoing strategy of repetition is apparently very effective in managing the perception and manipulating the belief of the unsuspecting public.
The viral reservoir backing this present pandemic is far from stable and is actively acquiring new genetics. Current data does not indicate an immediate direction toward stability.
As is the nature with an IDRREAV, the positions reported here will not necessarily be the most important amino acid positions in the future. The reservoir will evade immunity and escape vaccine by heavily self-revising in the near future at key locations between 131 and 182 (emphasis at 155 to 177). Variation will occur with lower penetration at the head of the HA and from 272 downstream approximately 55 positions. Potential is very high that 22I will achieve density in one or more sub-clades. Expect 100N to spread and penetrate initially on sub-clades without the 22I appearances. South American polymorphisms from their fatal mid-pandemic strains of 2009 will recycle into the United States and other Northern Hemisphere nations with substantial thrust in the coming 90 days. Acceleration of the Avian based cross-linking will continue in the Western world.
Bear in mind also that a solid portion of the changes documented in this current list will become fixed even as additional donations are accumulated. The zoonotic movement from birds into human PF11 sequences is earnestly progressing in the most recent 45 days of available data. Several polymorphisms, HA and NA, from the Georgia hospitalization resurgence may have originated in Avian Influenza samples. Expect continued acquisition from Avian H5N1 and H1N1, and also watch the acceleration from one particularly new Avian donor serotype. H9N2 demonstrated a human jump in late 2008 and has very recently become fully engaged in ΣPF11 genetics with a well-defined etching on the newest cross-linked sequences. The reservoir flux will also be influenced by Swine H1N2 and H1N1 (emphasis on 3 particular emergent strains).
