Abstract

Background

During the 2010–2011 influenza season, a small sub-group of 2009 influenza A(H1N1) viruses (hereafter referred to as 2009 A(H1N1)) emerged that was associated with more severe clinical outcomes in Ecuador and North America. Genetically, the haemagglutinin (HA) of this sub-clade was distinct from HAs found in viruses associated with severe outbreaks in 2010 from the United Kingdom and from other global specimens isolated earlier in the season.

Objective

We report the emergence of a novel 2009 A(H1N1) variant possessing a re-emergent HA D222N mutation obtained from patients with severe respiratory illnesses and phylogenetically characterise these D222N mutants with other severe disease-causing variants clustering within a common emerging sub-clade.

Case reports

In early 2011, three cases of 2009 A(H1N1) infection, two from Quito, Ecuador, and one from Washington, DC, USA, were complicated by severe pneumonia requiring mechanical ventilation, resulting in one fatality. These cases were selected due to the reported nature of the acute respiratory distress (ARD) that were captured in Department of Defence (DoD)-sponsored global influenza surveillance nets.

Results

Genetically, the 2009 A(H1N1) strains isolated from two of the three severe cases carried a prominent amino acid change at position 222 (D222N) within the primary HA receptor binding site. Furthermore, these cases represent an emerging sub-clade of viruses defined by amino acid changes within HA: N31D, S162N, A186T and V272I. Phylogenetically, these viruses share a high degree of homology with strains associated with recent fatal cases in Chihuahua, Mexico.

Discussion

Previously, enhanced virulence associated with the change, D222G, has been clinically linked to severe morbidity and mortality. Initial observations of the prevalence of a novel sub-clade of strains in the Americas suggest that viruses with a re-emergent D222N mutation may too correlate with severe clinical manifestations. These findings warrant heightened vigilance for emerging sub-clades of 2009 A(H1N1) and presumptive clinical implications.

Banan district police chief Buth Sambo said that 1,500 chickens had died or became sick late last month and that about 4,150 other birds at the farm were incinerated on Friday after officials from the Ministry of Agriculture, Forestry and Fisheries found the H5N1 virus in dead and sick birds at the farm.

Cheam Chansophoan, director of the provincial agricultural department, said that the virus had not been detected in any people. “The farm was carefully cleaned, and we are not so worried about infection [of people] because it is located far away from villagers’ homes,” he said, adding that the ministry notified residents in the area to be alert for signs of bird flu.

Neang Kan, a veterinarian at Ta Kream commune, said that the outbreak began on October 25 but was not reported until five days later.

“We are working very hard to prevent infection of people, because when the sick birds are sold, people may get infected. This is a very dangerous situation,” he said.

Sar Ratha also said that he lost US$15,000 due to the H5N1 outbreak, the first one detected in Battambang, which resulted in the incineration of more than 4,000 chickens at his farm in Banan district on Friday.

Ly Sovann, deputy director of the health ministry’s communicable disease control department, said no people in the outbreak area had tested positive for the virus.

Teams of health officials and veterinarians were working in tandem to prevent the outbreak spreading, he said.

No bird flu outbreak in Ohangwena

Rumours of a bird flu outbreak in the Ohangwena region have been nipped in the bud by officials from both the Ministries of Agriculture and Health.
Attempts to clear the air follows mounting panic in the Ohangwena region that bird flu – supposedly from neighbouring Angola – has affected chicken and possibly human beings.
As a result of the allegations on both sides of the border, a number of Angolan nationals have been spotted crossing the Santa Clara border post into Namibia wearing face masks to “prevent a deadly infection”.

“No one wants to take chances. If it’s not swine flu it is bird flu,” said a resident of Oshikango.Deputy Director of Veterinary Services and Animal Disease Control in the Ministry of Agriculture, Dr Johnny Shopala has, however, spelled out in no uncertain terms that although there have been recent unconfirmed reports from neighbouring South Arica of bird flu in ostriches, Namibia has not received any reports on bird flu cases.
There have been no reports of bird flu in animals in Namibia. If it’s in humans, than it is dealt with by the Ministry of Health and Social Services and they should be able to give clarity on that,” said Shopala.
The H5N1 strain of avian influenza, or bird flu, can prove deadly to human beings if the virus combines with human varieties of flu and undergoes a mutation allowing it to jump from human to human.
Although Namibia has been on avian flu alert following an outbreak of the disease in South African in 9 April 2011, no cases have been detected within the Namibian borders.
Acting Ohangwena Health Regional Director, John Hango, said although the region has been dealing with cases of swine flu, also known as H1N1 flu, no bird flu in human cases have yet been reported to his department.
Hango said it was most likely that people mistook the announcement of H1N1 for H5N1, which is bird flu.
“Yes, I can confirm an outbreak of swine flu, but definitely not bird flu,” said Hango.
