A new study released Friday by the Michigan State University finds that viruses may be evolving and adapting more quickly than initially thought.
Scientists leading the study announced they have for the first time evidence of how the virus called “Lambda” evolved to find a new way to attack host cells, an innovation that took four mutations to accomplish.

It is the first time that the researchers have described a way through which viruses evolved a novel way to infect people, which could reveal better means of protecting against some of the world’s most notorious viruses. Researchers said that they studied a virus which is known as a lambda. It infects the gut bacterium Escherichia coli.

Michigan State University scientists described how viruses evolved a new way of infecting cells in little more than two weeks, far sooner than initially thought. In a series of experiments, the bacteria-infecting viruses repeatedly acquired the ability to attack their host bacteria through a different “doorway,” or receptor on the bacteria’s cellular membrane, according to Justin Meyer, the lead researcher and a graduate student...

“We were surprised at first to see Lambda evolve this new function, this ability to attack and enter the cell through a new receptor – and it happened so fast,” the Michigan State University graduate student said in a statement. “But when we re-ran the evolution experiment, we saw the same thing happen over and over.”

On 16 January, a five-year-old girl from northern Jakarta died after being tested positive for the H5N1 bird flu virus. She was a relative of a 24-year-old man who died on 7 January, the World Health Organization (WHO) said, adding that the two had contact with the same pigeons in their neighbourhood.

According to WHO, the latest fatalities brought the death toll from avian influenza in the country to 152, out of 184 confirmed cases since 2006.

The Health Ministry's director-general of disease control and environmental health, Tjandra Yoga Aditama, said there was no evidence that human-to-human transmission was involved in the latest case.

"As long as there are still birds carrying the virus, there will be cases of bird flu in humans from time to time," Aditama told IRIN.

Of 60 bird flu cases detected last year in Indonesia, 11 were fatal, Aditama said.

"We have to continue to maintain our vigilance," Aditama said. "The Indonesian government is taking the threat of bird flu seriously and is taking measures like countries in other parts of the world."

Coordination

But Marius Widjajarta, chairman of the Indonesian Consumer Empowerment Foundation for Health (YPKKI), cited a lack of coordination among different government departments as hampering efforts to contain its spread.

"We can see that the Health Ministry, the Agriculture Ministry and local governments are doing their own things," Widjajarta told IRIN. "What if there was a pandemic?"

"All parties concerned must cooperate to promote changes in people's behaviour and attitude towards the threat of bird flu," he added.

Public awareness campaigns were done sporadically, he said, and official reporting of bird flu cases in poultry has been spotty.

The Health Ministry said bird flu was endemic in poultry in all but three of Indonesia's 31 provinces. The ministry has designated 100 hospitals across the archipelago nation as being well-equipped to treat bird flu patients.

According to WHO, 344 people in 12 countries have died of bird flu since 2003, with Indonesia reporting the most fatalities.

WHO said so far, transmission of H5N1 viruses from animals to humans had only resulted in sporadic human cases or small clusters among close contacts, with no evidence of community-level spread.

"These sporadic human cases and small clusters of human infection with variant influenza viruses are expected and are not considered unusual, and do not change WHO's current assessment of pandemic risk," WHO said in a statement released in December.

A department spokesman said there were no poultry farms within a three- kilometer radius of where the dead goose was found.

The department also found no evidence of anyone keeping poultry in their backyards.

It will continue to monitor the situation and conduct inspections of the area.

The discovery of the dead goose comes days after fresh chicken sales resumed last Friday, following a 21-day ban, when the bird flu warning was lowered from "serious" to "alert."

The department has notified poultry farmers, pet bird shop owners and licensed holders of pet poultry and racing pigeons that proper precautions must be taken.

The spokesman said the department will frequently inspect poultry farms and wholesale markets to ensure that proper precautions against avian influenza are being implemented.

Abstract
In order to examine the prevalence of Staphylococcus aureus on retail pork, three hundred ninety-five pork samples were collected from a total of 36 stores in Iowa, Minnesota, and New Jersey. S. aureus was isolated from 256 samples (64.8%, 95% confidence interval [CI] 59.9%–69.5%). S. aureus was isolated from 67.3% (202/300) of conventional pork samples and from 56.8% (54/95) of alternative pork samples (labeled “raised without antibiotics” or “raised without antibiotic growth promotants”). Two hundred and thirty samples (58.2%, 95% CI 53.2%–63.1%) were found to carry methicillin-sensitive S. aureus (MSSA). MSSA was isolated from 61.0% (183/300) of conventional samples and from 49.5% (47/95) of alternative samples. Twenty-six pork samples (6.6%, 95% CI 4.3%–9.5%) carried methicillin-resistant S. aureus (MRSA). No statistically significant differences were observed for the prevalence of S. aureus in general, or MSSA or MRSA specifically, when comparing pork products from conventionally raised swine and swine raised without antibiotics, a finding that contrasts with a prior study from the Netherlands examining both conventional and “biologic” meat products. For pediatric patients, approximately 1.85x (95% CI 1.45x–2.36x, adj. p<0.0001) as many CA-MRSA infections per ED visit occurred in the second two quarters as occurred in the first two quarters. For adult patients, 1.14x (95% CI 1.01x–1.29x, adj.p = 0.03) as many infections per ED visit occurred in the second two quarters as in the first two quarters. Approximately 2.94x (95% CI 1.39x–6.21x, adj.p = 0.015) as many HA-MRSA infections per hospital admission occurred in the second two quarters as occurred in the first two quarters for pediatric patients. Importance of the Global Regulators Agr and SaeRS in the Pathogenesis of CA-MRSA USA300 Infection
Abstract
CA-MRSA infection, driven by the emergence of the USA300 genetic background, has become epidemic in the United States. USA300 isolates are hypervirulent, compared with other CA- and HA-MRSA strains, in experimental models of necrotizing pneumonia and skin infection. Interestingly, USA300 isolates also have increased expression of core genomic global regulatory and virulence factor genes, including agr and saeRS. To test the hypothesis that agr and saeRS promote the observed hypervirulent phenotype of USA300, isogenic deletion mutants of each were constructed in USA300. The effects of gene deletion on expression and protein abundance of selected downstream virulence genes were assessed by semiquantitative real-time reverse-transcriptase PCR (qRT-PCR) and western blot, respectively. The effects of gene deletion were also assessed in mouse models of necrotizing pneumonia and skin infection. Deletion of saeRS, and, to a lesser extent, agr, resulted in attenuated expression of the genes encoding α-hemolysin (hla) and the Panton-Valentine leukocidin (lukSF-PV). Despite the differences in hla transcription, the toxin was undetectable in culture supernatants of either of the deletion mutants. Deletion of agr, but not saeRS, markedly increased the expression of the gene encoding protein A (spa), which correlated with increased protein abundance. Each deletion mutant demonstrated significant attenuation of virulence, compared with wild-type USA300, in mouse models of necrotizing pneumonia and skin infection. We conclude that agr and saeRS each independently contribute to the remarkable virulence of USA300, likely by means of their effects on expression of secreted toxins.

An 18-year-old man thought to be infected with avian flu was in critical condition at the Tangerang General Hospital as of Tuesday.

“The patient is still in isolation,” hospital spokesman Achmad Muchlis said. “He is exhibiting the symptoms of avian flu, but the team of doctors is still waiting for confirmation from laboratory results.”
The patient, a resident from Me-karsari village in Tangerang regency, suffered from a high fever for six days after which time he was taken to the private Sari Asih Hospital in Karawaci last Wednesday, when he developed respiratory problems.

As his condition worsened, the hospital transferred him to Tangerang General Hospital on Saturday.

According to the hospital’s president director, Mamahit, the family informed doctors that before the patient fell sick, one of the ducks he had raised had died.

“Referring to the case history and the symptoms he was suffering from, we treated him as a bird flu patient,” he told reporters.

A team from the Tangerang Health Agency conducted a survey around the patient’s neighborhood.

“We haven’t as yet found any infected fowl … We also checked all [the patient’s] family members and neighbors, but none of them were sick,” agency head Naniek Isnaeni said. “We are still waiting for the patient’s laboratory results to know what to do next.”

Within the part two weeks, there have been two confirmed deaths from avian flu in Jakarta alone.

Health Minister Endang Rahayu Sedyaningsih said Tuesday that health care facilities in Indonesia had sufficient capacity to properly diagnose avian influenza; however, more efforts were needed to expand the country’s bird flu laboratory capacity, which would allow more sensitive detection of the disease.

“We are conducting polymerase chain reaction [PCR] swab tests on mucus taken from the nose and throat to detect whether it contains the H5N1 virus. The test can produce results in quite a short period of time, but we may need a further test involving virus sequencing in order to get a more accurate result on whether a patient is positive for H5N1. However, not all labs have these facilities,” she said.

Lab testing is one of the areas of expertise the government aims to develop in tackling the disease.

“We are improving 42 labs assigned as diagnostic centers for avian influenza,” said Endang.

Endang said cross-sector collaboration played a key role in tackling the highly pathogenic avian influenza virus. The Health Ministry is collaborating with other institutions, including the agriculture and trade ministries, to combat outbreaks.

The ministry has also distributed anti-influenza drugs called Oseltamivir to health care facilities, including both hospitals and community health centers, free of charge. “We have sufficient stocks of Oseltamivir to last until next year,” Endang said, adding that the government had reserve stocks of more than 1.4 million Oseltamivir tablets ready to distribute if needed.
Tjandra Yoga Aditama, the director general of Disease Control and Environmental Health (P2PL), said that bird flu patients undergoing Oseltamivir therapy would experience a higher chance of recovery compared with those who did not.

"This study shows that the meat we buy in our grocery stores has a higher prevalence of staph than we originally thought," says lead study author Tara Smith, Ph.D., interim director of the UI Center for Emerging and Infectious Diseases and assistant professor of epidemiology. "With this knowledge, we can start to recommend safer ways to handle raw meat products to make it safer for the consumer."
The study also found no significant difference in MRSA contamination between conventional pork products and those raised without antibiotics or antibiotic growth promotants.
"We were surprised to see no significant difference in antibiotic-free and conventionally produced pork," Ms Smith says. "Though it's possible that this finding has more to do with the handling of the raw meat at the plant than the way the animals were raised, it's certainly worth exploring further."
