Global MERS count tops 100 with latest from Saudi Arabia

In a brief statement, Saudi Arabia's Ministry of Health (MOH) has reported three more cases of Middle East respiratory syndrome coronavirus (MERS-CoV), apparently pushing the global case count over 100.
The agency said two of the cases involve a 31-year-old male citizen who has several chronic diseases and is in stable condition in intensive care, and a 55-year-old male citizen who has no symptoms. The older man had contact with a confirmed case-patient. Both cases are in the Asir province in the country's southwest.

The Italian veterinary authorities have reported another outbreak of highly pathogenic avian influenza (HPAI) at another commercial layer farm located in Emilia-Romagna.

The World Organisation for Animal Health (OIE) received follow-up report no. 1 on Wednesday, 21 Aug 2013.

According to the report, out of 584 900 susceptible birds, one case was identified. No deaths were reported, and no birds were destroyed.

On the basis of the monitoring activities implemented after the detection of the primary outbreak in Ostellato, Ferrara province, notified on 15 Aug 2013, a secondary outbreak has been confirmed in Mordano, Bologna province, affecting a commercial farm of layer hens belonging to the same company.

According to the OIE, in the secondary outbreak HPAI subtype H7 has been detected at RT-PCR and no clinical signs have been identified.

Restriction zone has been established, depopulation will be carried out and epidemiological investigation is ongoing.

The depopulation operation in the primary outbreak, in Ostellato, Ferrara province, was completed yesterday, 21 Aug 2013, followed by cleaning and disinfection.

The National Reference Laboratory in Padova confirmed that the virus isolated is an HPAI H7N7 showing an IVPI [intravenous pathogenicity index] of 3.0 [see comment].

Genome sequencing as well as the epidemiological investigation are ongoing.

Preliminary information would suggest that an LPAI virus entered the farm (possibly via wild birds) where it mutated to an HPAI strain.

The source of the outbreaks remains inconclusive.

In October, Saudi Arabia will host millions of travelers on the hajj, the annual pilgrimage to Islam’s holy sites. The hajj carries deep meaning for those observant Muslims who undertake it, but it also carries risks that make epidemiologists blanch. Pilgrims sleep in shared tents and approach the crowded sites on foot, in debilitating heat. They come from all over the world, and whatever pathogens they encounter on the hajj will travel back with them to their home countries. In past seasons, the hajj has been shown to foster disease, from stomach flus to tuberculosis or meningitis.
The Saudi Arabian government has traditionally taken this threat quite seriously. Each year it builds a vast network of field hospitals to give aid to pilgrims. It refuses visas to travelers who have not had required vaccinations and makes public the outbreaks it learns about. This year, though, the Saudis have been strangely opaque about one particular risk—and it’s a risk that has disease experts and public-health agencies looking to October with a great deal of concern. They wonder if this year’s hajj might actually breed the next pandemic.

Abstract

In response to several influenza A(H1N1)pdm09 infections that developed in passengers after they traveled on the same 2 flights from New York, New York, USA, to Hong Kong, China, to Fuzhou, China, we assessed transmission of influenza A(H1N1)pdm09 virus on these flights. We defined a case of infection as onset of fever and respiratory symptoms and detection of virus by PCR in a passenger or crew member of either flight. Illness developed only in passengers who traveled on the New York to Hong Kong flight. We compared exposures of 9 case-passengers with those of 32 asymptomatic control-passengers. None of the 9 case-passengers, compared with 47% (15/32) of control-passengers, wore a face mask for the entire flight (odds ratio 0, 95% CI 0–0.71). The source case-passenger was not identified. Wearing a face mask was a protective factor against influenza infection. We recommend a more comprehensive intervention study to accurately estimate this effect.

Case–Control Study
To assess risk factors for transmission of influenza A(H1N1)pdm09 on the New York to Hong Kong flight, we conducted a case–control study. We compared exposure history and other risk factors of 9 confirmed case-passengers with those of 32 control-passengers in the economy-class cabin. We attempted to contact 55 noninfected passengers who disembarked in Fuzhou and 18 noninfected passengers who disembarked in Hong Kong, and we interviewed all persons >5 years of age who agreed to be interviewed. Crew members and business-class passengers were excluded. A total of 32 noninfected passengers provided complete information and served as controls. Of these 32 control-passengers, 28 boarded in New York; 27 disembarked in Fuzhou and 1 disembarked in Hong Kong; and 4 boarded in Vancouver and disembarked in Hong Kong.
We conducted face-to-face interviews with case- and control-passengers bound for Fuzhou at hospitals or hotel rooms where they were quarantined. For passengers quarantined at home or who disembarked in Hong Kong (including 1 case-passenger in Hong Kong), interviews were conducted by telephone. Using a standard questionnaire, we interviewed case- and control-passengers on factors potentially affecting the likelihood of influenza A(H1N1)pdm09 virus infection during the 7 days before and during the flight. These factors included contact with ILI patients ≤1 week before the flight, moving around the airplane during the flight, lavatory use, handwashing, face mask use (wearing a face mask, for how long, and when they wore it and did not wear it), and talking with other passengers.

(ANSA) - Brussels, August 22 - The European Union Commission has approved Italy's decision to create zones of protection and surveillance around the chicken farm in Ostellato, the Northern Italian province of Ferrara, where a strain of H7N7 bird flu has been identified.

The local municipality of Ostellato as such will be submitted to strict sanitary controls until September 23, together with the nearby towns of Argenta, Comacchio, Migliarino, Migliaro, Portomaggiore and Tresigallo.

The objective will be to avoid any shipments from there to the EU and non-EU zones of live poultry, poultry ready for the laying of eggs, young chicks or eggs bearing chicks.

The EU Commission explained its decision to approve the stringent measures due to the "very contagious disease".

The researchers collected more than 1,000 samples from seven bat species in regions where cases of MERS were identified in Bisha, Unaizah, and Riyadh between October 2012 and April 2013.

Emergence of H7N9 avian flu hints at broader threat

The H7N9 influenza virus did not emerge alone. Researchers have traced the evolution of the deadly avian flu currently spreading in China, and have found evidence that it developed in parallel with a similar bird flu, H7N7, which can infect mammals1.
Although there is no evidence that this H7N7 strain will infect humans, the authors of a study published today in Nature1 say that their finding reinforces the idea that H7 avian viruses are constantly mixing and exchanging genetic material — a process known as reassortment — in Asian poultry markets. This raises the threat that H7N7 will reassort and become able to spread to humans.

"H7 is out there in China and not just in the form of this H7N9," says Richard Webby, a co-author of the study and an influenza specialist at St. Jude Children’s Research Hospital in Memphis, Tennessee.

Tracking changes
Guan's team sampled wild birds and poultry markets around Shanghai in April, weeks after the H7N9 outbreak began there. The researchers collected throat and intestinal swabs from 1,341 birds, including chickens, ducks, geese, pigeons, partridges and quails, plus 1,006 water and faecal samples from bird markets. About 10% of samples tested positive for an influenza virus; of those, 15% were an H7 virus.
When the team sequenced the two viruses’ genomes and compared them to other bird-flu strains, they found H7N9 and H7N7 to be hybrids of wild Eurasian waterfowl strains, such as H7N3 and H11N9. The scientists think that those viruses swapped genes in domestic ducks before spreading to chickens, where they traded genes with a common chicken virus, H9N2. That improved the viruses' ability to spread in chickens, which live in close contact with humans.
So far, the latest H7N7 strain has not infected a human. But Guan and his team found that ferrets could become infected with the virus, suggesting that a spread to humans is possible.
"It really shows that the emergence of these types of viruses can happen at any time," says Camille Lebarbenchon, a viral ecologist at the University of Reunion Island in St Denis, France, who has also studied the evolution of H7N9 using archived viral sequences2.
David Morens, an influenza researcher and senior adviser at the US National Institutes of Health in Bethesda, Maryland, says that the evolutionary pathway that the viruses followed suggests that more surveillance and better sanitation practices at poultry markets are crucial to monitoring risks to human health.
But Ian Lipkin, an epidemiologist at Columbia University in New York City, says that surveillance is not a foolproof solution. "It’s inevitable that something is going to slip through the cracks."

Scientists have found the mysterious MERS virus in a bat in Saudi Arabia.
An international research team said the bat virus is an exact match to the first known human case of Middle East respiratory syndrome. The sample was collected from within a few miles of that patient's home.
The discovery reported Wednesday is considered an important development in the search for the origin of MERS, a deadly respiratory illness that is worrying health officials around the world.
But it's likely that something else — perhaps another animal — is spreading the virus directly to humans, said Dr. Ziad Memish, Saudi Arabia's Deputy Minister of Health and lead author of the report.

The second French patient who had been hit by the coronavirus MERS but no longer carries the virus is still in hospital in Lille in serious condition, officials said Wednesday at the CHR Lille. "He remains hospitalized in intensive care without isolation procedure with a medical condition that is serious," he told AFP a spokesman for the CHR.

 For over three months the patient was hospitalized Lille,  where he was placed in a first time in quarantine before emerging to mid-July. The patient had been infected in late April when he shared in the Valenciennes hospital room the first patient in France, fell ill after returning from a trip to Dubai. Qatar reports third Mers case

Dubai: A 59-year-old Qatari man has been diagnosed with the Middle East Respiratory Syndrome Coronavirus (Mers-CoV), Qatar’s Supreme Council of Health (SCH) has said.
The case brings to three the number of people affected by the virus in Qatar.
In June, a 49-year-old Qatari man, who was the first to be diagnosed with the virus, died at a London hospital following his admission in September.
SCH said that the second case was responding to treatment and was doing well.
The newly infected man was reported to have felt the symptoms while abroad, but underwent a check-up upon his arrival in Qatar.
He was diagnosed with the disease following tests conducted at the National Influenza Laboratory, the SCH said in a statement reported in the local media on Wednesday.
The council said that the patient was receiving “appropriate” medical treatment and that “he is now in a stable condition”.
Surveillance and monitoring procedures are being strictly followed and prompt measures taken for the infection control of all cases of acute respiratory infections, especially those coming back from abroad, the statement said.

#MERS #Coronavirus: 2 Cases of "Corona" in Saudi Arabia and one in Qatar

Dubai, United Arab Emirates

For its part, the Ministry of Health  Saudi Arabia  Tuesday, registered cases of HIV infection in the Riyadh region, bringing the number of those who were injured in the UK with the virus to  76 cases of whom 39 died .
According to the ministry, the المريضتين aged 50 and 59 years old and suffer from multiple chronic diseases, and shall receive the necessary medical care now.
