Two Suspect Bird Flu Patients Referred to Hospital NTB

Two patients suspect bird flu (H5N1) from Central Lombok regency was referred to the Regional General Hospital (Hospital) West Nusa Tenggara Province, on Saturday (15/10). Cases of bird flu that attacked hundreds of chickens have been spread in a number of districts in Central Lombok and Mataram. Quick and responsive handling is expected to be done by all parties that this case did not spread to other districts in the NTB.

Bird Flu Spreads to Most Bali Districts; Lombok on Edge Over Poultry Deaths
Denpasar/Mataram. Health authorities in Bali said on Friday that the bird flu virus that killed two children there last week had spread to eight of nine districts on the island, while residents in neighboring Lombok are reporting similar chicken deaths.
Putu Sumantra, head of the Bali Livestock Agency, said that only Gianyar district had not reported cases of poultry dying from the disease.
“This is quite surprising, especially given how quickly the disease is spreading,” he said.
He added that the pattern of the spread of the disease was similar, although smaller in scale, to 2007, when a bird flu pandemic swept the island.
“Back then, around 4,000 birds died, but this time the figure is much smaller,” Sumantra said. “Nevertheless, the deaths of the two children is a very shocking thing to come to terms with.”
Almost 2,800 birds are confirmed to have contracted avian influenza. More than 1,300 died from the disease and a further 1,200 were culled.
“Most of the cases occurred in poultry raised on small farms,” Sumantra said. “Those on larger commercial farms tend to be vaccinated on a regular basis so aren’t as vulnerable.”
Ketut Kariyasa Adnyana, a member of the provincial legislature, expressed concern that the spread of the disease would hurt the island’s all-important tourism industry.
He also said it came at a particularly critical time, with US President Barack Obama among the clutch of world leaders scheduled to arrive in Bali for the East Asia Summit.
“The provincial and district authorities should take immediate steps to address this problem. We can’t afford to let it get out of hand,” he said.
On the neighboring tourist island of Lombok, sudden poultry deaths attributed to bird flu are also mounting.
Hundreds of chickens died on Friday at an Army dormitory in Mataram, the provincial capital, residents reported. A veterinary team from Bali has been to the site to take back tissue samples to test for avian influenza.
Patriot, a resident, said he and his neighbors had immediately buried the dead birds for fear that they had died from bird flu and that the virus could spread.
However, Retno Prayudo Yanti, head of the municipal animal hospital, said initial tests showed the chickens had probably died from Newcastle disease.
The virus that causes Newcastle disease in birds poses no serious health threat to humans, although exposure to infected poultry can lead to mild flu-like symptoms.
“But even though the initial results suggest Newcastle disease, we still need further tests before we can rule out bird flu,” Retno said.
The provincial health office also said it was not taking any chances and was urging all residents with flu-like symptoms to see a doctor immediately.

Abstract
Current egg-based influenza vaccine production technology can't promptly meet the global demand during an influenza pandemic as shown in the 2009 H1N1 pandemic. Moreover, its manufacturing capacity would be vulnerable during pandemics caused by highly pathogenic avian influenza viruses. Therefore, vaccine production using mammalian cell technology is becoming attractive. Current influenza H5N1 vaccine strain (NIBRG-14), a reassortant virus between A/Vietnam/1194/2004 (H5N1) virus and egg-adapted high-growth A/PR/8/1934 virus, could grow efficiently in eggs and MDCK cells but not Vero cells which is the most popular cell line for manufacturing human vaccines. After serial passages and plaque purifications of the NIBRG-14 vaccine virus in Vero cells, one high-growth virus strain (Vero-15) was generated and can grow over 108 TCID50/ml.In conclusion, one high-growth H5N1 vaccine virus was generated in Vero cells, which can be used to manufacture influenza H5N1 vaccines and prepare reassortant vaccine viruses for other influenza A subtypes.
JVI: H5N1-SeroDetect EIA and Rapid Test: A Novel Differential Diagnostic Assay for Serodiagnosis of H5N1 Infections and Surveillance
Continuing evolution of highly pathogenic (HP) H5N1 influenza viruses in wild birds with transmissions to domestic poultry and humans poses a pandemic threat. There is an urgent need for a simple and rapid serological diagnostic assay which can differentiate between antibodies to seasonal and H5N1 strains that could provide surveillance tools not dependent on virus isolation and nucleic acid technologies. Here we describe the establishment of H5N1 SeroDetect ELISA and rapid test assays based on three peptides in HA2 (488-516), PB1-F2 (2-75), and M2e (2-24) that are highly conserved within H5N1 strains. These peptides were identified by antibody repertoire analyses of H5N1 survivors in Vietnam using whole genome phage display libraries (GFPDL). To date, both platforms demonstrated high levels of sensitivity and specificity in detecting H5N1 infections (clade 1 and clade 2.3.4) in Vietnamese patients as early as 7 days and up to several years post infection. H5N1 uninfected individuals in Vietnam and the US, including subjects vaccinated with seasonal influenza vaccines or confirmed seasonal infections, did not react in the H5N1-SeroDetect assays. Moreover, sera from individuals vaccinated with H5N1 subunit vaccine with moderate anti-H5N1 neutralizing antibody titers did not react positive in the H5N1-SeroDetect ELISA or rapid test assays.
The simple H5N1-SeroDetect ELISA and rapid tests could provide an important tool for large scale surveillance for potential exposure to HP H5N1 strains in both humans and birds.

Anwar Mamonto, known as War (58 ), who is a chicken trader in Gogagoman Village, West Kotamobagu Subdistrict, had one of his chickens die on Monday, 8/30. He thought it was a normal occurrence. They included the eldest son of War, AM/Af (31), his pregnant wife HK/Rah (29), their son AM/Naf (3), plus the in-laws MK/Mus (58 ) and his wife, NM/Nen (54). Also joining them were additional relatives: OM (58 ), Mrs. OM (48 ), FM (29), as well as ChM, residents of Lorong Kembang, Gogagoman Village. War's wife prepared and cooked the chickens with the help of ChM. (ChM may have been the employee War mentioned had fallen ill, and the unrelated individual).

At around 3:00 p.m. on Sunday, 9/4, the first hospital, Regional Public Hospital Datuk (Datoe) Binangkang in Kotamobagu City, decided to reconcile the entire lot of patients over to RSUP Prof Kandou in Manado where they arrived around 6:00 p.m. on Sunday, 9/4.
