The Centre for Health Protection (CHP) of the Department of Health today (December 8) urged members of the public and management of institutions to take preventive measures against respiratory tract infection by observing strict environmental and personal hygiene.
The appeal was made during CHP's investigation of a report of upper respiratory tract infection outbreak in an elderly home in the Eastern District.
As of today, 21 people were affected. Eighteen residents and three staff members, comprising seven men and 14 women aged from 25 to 96, developed symptoms of upper respiratory tract infection, including fever, cough, runny nose and sore throat between November 15 and December 7.
Among them, 18 consulted visiting doctor or out-patient clinics and three others were admitted to the Pamela Youde Nethersole Eastern Hospital. One has been discharged. All of the affected are in stable condition.
CHP officials have conducted field visits to the institution and provided health advice to the staff.
To prevent respiratory tract infection, the public are advised to adopt the following measures:
* Build up good body immunity by having a proper diet, regular exercise and adequate rest, reducing stress and avoiding smoking;
* Maintain good personal hygiene, and wash hands after sneezing or coughing, and wear a mask when having symptoms of respiratory tract infections;
* Maintain good ventilation; and
* Avoid visiting crowded places with poor ventilation.

The above comment from the most recent WHO situation update describes a second recently H5N1 confirmed case in Indonesia. The above case was in East Jakarta, where there have been many previously confirmed cases. This is also true of the case in Riau. However, recent reports have indicated 50% of the poultry sold in markets in Jakarta, Banten, and West Java is H5N1 positive, raising concerns of widespread human cases in the area. The two most recent cases were reported relatively soon after confirmation. However, earlier cases were reported weeks or months after confirmation, raising serious concerns.
In addition to the news blackout on confirmed cases, other lab confirmed cases and clusters have been denied and in other cases, index cases of H5N1 confirmed clusters have been said to be due to lung inflammation, dengue fever, and typhus.
The concerns surrounding these cases has been compounding by a withholding of samples from third party analysis, and a lack of sequences data. The last human H5N1 sequences are from patients who died almost two years ago (January 2007).
Recently a large PCR confirmed cluster was reported in Makassar, and was reported to be lab confirmed was denied by the Ministry of Health. The denial was followed by the placement of a WHO team into the region to monitor H5N1 in poultry. The H5N1 situation in poultry has also been unclear since Indonesia has not filed an OIE report since 2006 and has halted shipment of poultry samples to the WHO regional lab in Australia.
These denials of lab confirmed cases and the withholding of samples and sequences have raised serious pandemic concerns.
Timely reports of confirmed cases and more interactions with third parties, including release of samples and sequences, would be useful.

GENEVA, Dec 9 (Reuters) - Indonesia has confirmed two new cases of human bird flu, the first officially reported since September in the country which remains the hardest-hit by the deadly virus, the World Health Organisation said on Tuesday.
A nine-year-old girl in Riau province developed symptoms on Nov. 7 after poultry apparently died at her home, the WHO said in a statement. She was hospitalised five days later and discharged on Nov. 22 after recovering.
A two-year-old girl from East Jakarta died on Nov. 29 after developing symptoms on Nov. 18, it said. "Investigations into the source of her infection suggest exposure at a live bird market."
The two latest cases took Indonesia's known number of bird flu infections to 139, including 113 deaths, since 2003, according to the United Nations health agency.
As noted further down in this article, not included is the 15-year-old girl from central Java who's doctor publicly stated that she had died from the H5N1 virus early last month.

The primary focus of this week's WHO gathering is to discuss, and hopefully resolve, issues pertaining to the sharing of virus samples and vaccines.
Indonesia is claiming IP (Intellectual Property) rights on viruses that emerge from within their borders, and believe that they should control their distribution and use.
Indonesia has refused to `share' virus samples until guarantees are made that they will be provided with vaccines, made from these samples, at an affordable price.
Additionally, Indonesia has been slow to report human cases of H5N1 infection - preferring to provide aggregate totals every few months instead of real-time reporting.
This dispute has been ongoing for two years, and during that time the world has had very little access to virus samples from Indonesia, creating a dangerous blind spot for scientists hoping to detect changes in the virus.
While the official death toll stands at 113 in Indonesia, due to inconsistent and oft times inaccurate testing, few observers believe that number fully reflects the situation.
Dr. Margaret Chan, Director-General of the World Health Organization, spoke at the opening of the Intergovernmental meeting (full text of speech here), and had this to say about the pandemic threat.
This meeting is yet another expression of the continuing concern about the serious consequences of an influenza pandemic. This concern is shared among governments, and among their multiple ministries, all around the world.
The concern is rightly placed, as is the emphasis on preparedness. I can think of no other health event that is so rapidly global in its sweep, or so potentially devastating in terms of human illness and deaths, and severe economic and social disruption.
