Thai border guards testing for malaria

Friday, 20 February 2009

LOCAL traders making day-trips into Thailand are coming under increased scrutiny, with a particular increase in medical checks - including blood tests - being applied by Thai officials, according to border guards.

Kim Sothy, an immigration official posted at the Cham Yeam international border crossing in Koh Kong province, told the Post that the new tests showed that Thai officials were "looking down on Cambodians".

He said that there had been no advance warnings of the new tests, which had put off local traders from doing business in Thai markets across the border.

"Three people were forced to do blood tests, and 30 minutes later Thai physicians come to apologise to us," he said, adding that the physicians had been told to give at least three days' advance warning to make it easier to inform local people.

Koh Kong Governor Yuth Puthong said the blood tests were unexpected, but that Thai authorities were screening border commuters for infectious diseases.

"There's no problem because the Thai physicians are testing blood to find malaria. But our people didn't know, so it made them afraid," he said. "If they tell us in advance, we will cooperate and work with them."

Suon Sovannarith of Legal Support for Children and Women, an NGO working with migrant workers in Thailand, said the new measures sounded unusual but had not heard confirmations of a new crackdown on infectious diseases.

Avian influenza Outbreak in Phongsaly: Control and surveillance measures ongoing

(KPL) An avian influenza outbreak was confirmed last week in Muang Koua District, Phongsaly Province. This is the first province this year where the disease is detected and where previously avian influenza was not found.
According to the source from FAO Lao office, two red zones, meaning the 1 km zones around a place where avian influenza was detected, have been declared. One red zone is located directly in the district capital city and the center of the other is located approximately three kilometers away on the road to Oudomxay. A joint Department of Livestock and Fisheries (DLF) and Food and Agriculture Organization (FAO) team has been on field mission since February 14th to directly advise the local authorities on adequate control measures and enabled good coordination.
Dr. Bounlom Douangngeun, Director of the National Animal Health Center (NAHC), who is on site within the FAO mission, explained “Intense active surveillance is still ongoing in the villages of the 5km yellow zone surrounding the two red zones to ensure that the disease is not spreading further. The number of samples per village flock being taken is such that it will detect, with 95% confidence, a positive bird at 5% of prevalence. Movement control check posts have been set up and the market in Muang Koua has been closed for poultry and poultry products. Mainly chicken eggs and poultry meat originating from China was confiscated. Outbreak investigation has determined onset of the disease at the end of January, which was simultaneous with the Chinese New Year and Tet festivals. The outbreaks are located at only 1.5 hours road from Vietnam, though origin of poultry products usually available on the market is estimated to be 80% from China passing through Oudomxay province, and only 20% directly from Vietnam. Exact origin of the disease could however so far not be determined. Mortality of infected birds was found to be extremely rapid.”
The villagers from the red zone were informed about the necessary actions, which had to be put in place to control the disease and this included movement control, culling and disinfection, and ban on poultry selling. The main culling operation was finalized yesterday with more than 2500 poultry culled, but a few small scavenging chicken which could not be caught still remain and will be culled within the next days. Compensation is expected to be provided shortly to the poultry farmers at sixty percent of the market value.

Dr. Ricarda Mondry, Chief Technical Adviser for the FAO Avian Influenza Programme pointed out that “Rapid laboratory testing of suspect samples and immediate declaration of avian influenza outbreaks is essential to avoid further spread to other provinces. Furthermore farmers should be well informed on the exact amount and procedures of compensation paid for culled poultry, so that they collaborate with the authorities during the control operations. This will also encourage reporting of suspect poultry mortalities in other villages. Finally it is crucial that adequate funds for implementation of control measures on district level are available immediately.”
The DLF and the Avian Influenza Programme managed by the FAO have been working intensively with all the provinces since 2006 to strengthen the veterinary services in preventing and controlling avian influenza, financed by the governments of Germany, Japan, and the United States.
Dr. Ricarda Mondry mentioned that “the outbreak investigation training for provincial and district livestock staff conducted by FAO and NAHC in Phongsaly seems to have built sufficient local capacity to determine the extent of the outbreak and adjust the culling zone accordingly. Livestock staff from Phongsaly province had also attended the simulation exercise supported by FAO in Oudomxay in June 2008. FAO had furthermore trained 80 VVW in Muang Koua District beginning of 2008, who were responsible for reporting poultry mortality occurring in their village. Since August 2007, FAO has provided continuous support to 10 provinces to follow up any disease reported, be it through the provincial and district livestock offices or through the 166 hotline. The livestock staff have rapidly conducted adequate disease investigation measures and sent in the samples to the AI laboratory.”“The first report of poultry mortality in the district, later confirmed to be avian influenza, was given by a Village Veterinary Worker (VVW) trained by DLF staff with support through FAO in April 2008. This clearly indicates the impact the joint AI programme has on early detection of the disease and that continuous support to train VVW would be beneficial” adds Dr. Mondry.
During an outbreak, coordination among staff at different levels and among involved sectors is an essential factor to contain rapidly the disease. FAO in collaboration with the NAHC of the DLF took the lead in the animal disease investigation and control measures. Information on the diagnosis was shared with the office of the World Health Organization (WHO) and National Center for Laboratory and Epidemiology (NCLE) to ensure rapid response action from the human health side.
“Efficient coordination and implementation of emergency measures by the staff from different sectors from the central, provincial to district level as well as the international partners involved in the rapid detection and control of avian influenza outbreaks is very important, in order to avoid further
spread of the disease which would take a high toll on the livelihood of poor farmers’’, said Dr. Bounkhouang Khambounheuang, Director General of the Department of Livestock and Fisheries.
At present, the ongoing surveillance is being conducted not only in Phongsaly but also in the other neighboring provinces as well in order to ensure the rapid detection of new HPAI cases in the poultry population.

UN warns Vietnam to be "vigilant" on bird flu

Posted: 24 February 2009 2203 hrs

HANOI : UN agencies called on Tuesday for Vietnam to be vigilant in fighting bird flu after three new human cases were discovered this year, including one woman who died.

The World Health Organisation (WHO) and the UN's Food and Agriculture Organisation (FAO) said Vietnam, which has seen more bird flu deaths than any country except Indonesia, had to monitor the situation closely.

"While avian influenza poses a threat to humans and poultry throughout the year, experience in Vietnam has shown that the threat is highest at this time of year," Jean Marc Olive, a Vietnam-based WHO official, said in a joint statement.

"There is a real possibility that the virus will mutate and become easily transmissible between humans. If this occurs we could see the start of a new influenza pandemic."

"The avian influenza virus is also not currently able to spread easily between humans, but influenza viruses are known for their ability to change quickly and can become more adapted to humans with dire consequences," the statement said.

Recombinomics: WHO Hoarding of H5N1 Sequences Raise Pandemic Concerns

Commentary

WHO Hoarding of H5N1 Sequences Raise Pandemic Concerns
Recombinomics Commentary 01:55
February 23, 2009

Of the most recent isolates from China, A/Guangxi/1/2009 belongs to clade 2.3.2, the first human isolate from this clade. A/Hunan/2/2009, belonging to clade 2.3.4, shows genetic divergence from other viruses of this clade. Both viruses should be considered as potential vaccine strains pending further characterization.

The above comments are from the latest version of the WHO publication on H5N1 vaccine targets, “Antigenic and genetic characteristics of H5N1 viruses and candidate H5N1 vaccine viruses developed for potential use as human vaccines”, which was released earlier this month.

Although the comments, in conjunction with the HA phylogenetic tree at the end of the document, provide insight into the rapid evolution and divergence of H5N1 clades and sub-clades, the report is more notable for the data withheld than the data released.

In recent months, the increasing lack of transparency in reports on H5N1 outbreaks has reached an alarming level. China created concern when 8 confirmed cases of H5N1 were reported in January, this year. The concerns were increased because China has not reported associated outbreaks in poultry. Prior to the spike in human cases, China reported two outbreaks of asymptomatic clade 7 H5N1 in poultry in Jiangsu in December, raising concerns that the human cases were also clade 7.

The WHO report is glaringly silent of six of the eight cases. As noted above, the two disclosed cases involve the Funjian strain (clade 2.3). One is linked to a more divergent clade 2.3.4. Previously, almost all human cases in China were clade 2.3.4. The other case is a different Fujian sub-clade, 2.3.2, which led to the first human isolate. However, this sub-clade has been isolated from wild birds in Hong Kong for the past three seasons, and was also isolated in Japan and southern Russia in the spring of 2008. South Korea has also isolated clade 2.3.2 from poultry, but has not released any sequence data. Moreover, a soldier / culler involved in the spring Korean outbreak developed bird flu symptoms and was PCR H5 confirmed. However, South Korea did not file a WHO report because they could not isolate the clade 2.3.2 virus. They argued that the soldier wasn’t clade 2.3.2 positive because the sub-clade had not previously been reported in human cases. The current WHO report shows that the sub-clade can fatally infect humans, which increases concerns because the virus is widespread in wild birds migrating along the East Asian flyway.

However, of greater concern is the WHO silence on the other 6 confirmed human cases in China. The virus isolated from the first case, A/Beijing/1/2009 was said to not have any human flu gene segments, indicating all eight gene segments had been sequenced in early January. Moreover, the sequences were characterized as having small differences with prior isolates from China. The likelihood that the sequence was clade 7 was increased when officials in Hong Kong indicated that the dead birds there were infected Fujian strains. Calde 2.3.2 was in the wild birds, while clade 2.3.4 was in the poultry washing ashore, signaling additional unreported outbreaks in China. However, the officials also said that the human cases in China were due to a strain from the north, strongly suggesting that the human infections were clade 7.

In the WHO report above, clade 7 was discussed because more outbreaks were reported in Vietnam, and the newer clade 7 isolates in Vietnam were distinct from earlier clade 7 isolates, although all isolates were from 2008. Moreover, the isolates in Vietnam were distinct from the previosuly reported clade 7 isolates in China, raising concerns that this clade was rapidly evolving and causing widespread human infections in China.

However, WHO failed to comment on the other isolates from China, and these isolates, including A/Beijing/1/2009 were also not in the phylogenetic tree. The tree had an unprecedented number of recent isolates of interest that were lacking public disclosure of the underlying sequence. This lack of transparency has created concerns because it is well known that H5N1 transmission can be dramatically affected by a very small number of changes in the receptor binding domain, such as those seen in public clade 7 sequences.

In addition to withholding the sequences of the six isolates from China that are not on the tree, WHO regional labs are withholding an unprecedented number of sequences.

Prior to being designated a WHO regional lab, NAMRU-3 would promptly release human H5N1 sequences. However, this season there have been no sequences released, even though two 2009 isolates, A/Egypt/0001-NAMRU-3/2009 and A/Egypt/0585-NAMRU-3/2009 are listed on the WHO tree. These isolates are related to the vaccine resistant sequences reported last season, and raise concerns that there are additional receptor binding domain changes in the human isolates.

Similarly, NAMRU-3 has not released the sequence, A/Pakistan.540-NAMRU-3/2008 from the outbreak in Pakistan at the end of 2007. This outbreak involved one of the most extended human to human transmission chains reported to date for H5N1, yet the sequence of the only recovered isolate has been withheld. It is in the phylogenetic tree and is the first reported human case linked to the Uvs Lake strain the Qinghai sub-clade, 2.2.3, and was said to not have receptor binding domain changes, which would also be a first for a clade 2.2 extended human to human transmission chain. Moreover, the second reported human Uvs Lake isolate, A/Bangladesh/ 2070095/2008, has also been withheld.

In addition to the withholding of human isolates from China, Egypt, Pakistan, and Bangladesh, the novel clade 2.2.3 isolate from western Africa, A/chicken/Togo/CHEK2/2008 is also in the tree, but withheld.

In addition to the withholding of the human and bird clade 2.2 isolates, WHO has also withheld the various clade 7 isolates from Vietnam, A/chicken/Vietnam/NCVD-016/2008, A/chicken/Vietnam/NCVD-093/2008, and clade 7 vaccine target, A/chicken/Vietnam/NCVD-03/2008.

This withholding of important sequences, including those selected or recommended as vaccine targets is without precedent. Even Indonesia, which has a glaring lack of transparency in reporting H5N1 cases, has placed four recent human isolates, A/Indonesia/7379/2008, A/Indonesia/7272/2008, A/Indonesia/7261/2008, and A/Indonesia/8228/2008 on deposit at GISAID.

Thus, the increased hoarding of H5N1 sequences by WHO raises pandemic concerns and these scandalous actions are increasingly harder to justify.

H5N1 Cluster in Tangerang Indonesian Neighborhood Grows

Two people Sepatan, Kabupaten Tangerang, Banten, West Java, is now treated at the Hospital General Regional Tangerang because presumed bird flu. The two presumed victims of avian influenza that is MS, 15 years old, and a five MH, 2 years old.

"The pain that they experience similar to bird flu, we conclude they are presumed," said the Head of Public Health Tangerang District Hani Haryanto told Tempo, Monday (23 / 2).

MS and MH began treatment in the isolation room bird flu Regional General Hospital Tangerang since three days ago. According to Hani, suspected bird flu because they suffer a high fever with hot Geruduk after the Village, Village Mekar Jaya, Sepatan, where they have dozens of live poultry that died earlier.

However, Hani said, not yet known whether the two people that previous contact with poultry or not. "But in the neighborhood where they live poultry that have died," he said.

The above translations describes a second suspect H5N1 in the same neighborhood as the earlier suspect case in Sepatan (see updated map). The two patients are about 10 miles from the confirmed fatal case, and follow reports of additional confirmed or suspect cases in or around Jakarta.

Although many of the patients have not been declared official cases, the clustering of these cases in Jakarta or neighboring locals, including Bogor raises concerns.

Indonesia has imposed a media blackout, and reports confirmed cases on a delayed basis. The delay is reflected in WHO updates, and continues to violate International Health Regulations. The latest confirmed cases from Indonesia were also from the Jakarta area. The January 22 WHO update described a Tangerang housekeeper (29F) who developed symptoms December 11 and died December 16. The second confirmed case (5F) was in the Jakarta suburb of Bekaski who developed symptoms December 23 and died January 22.. These reporting delays, as well as these more recent cases in Bogor and Tangerang continue to raise concerns about Indonesian reporting and WHO enforcement of these reporting delays.

The most recent cases are linked to confirmed or suspect clusters, which also increase concerns.

Timely reporting of these lab confirmed cases would be useful.

Surabaya Avian Influenza Human Case

[Hat-tip Dutchy: From Surabaya Lab Blog, Dr Nidoms team.
More on the confirmed case in Surabaya.]

February 4, 2009

On January 24, 2009 there was a new human case of Avian Influenza infection. The victim, S, 21 years, was a housemaid. She was living in Darmo Permai.

On January 31, 2009 we went to the house of S and investigated the factor of infection. After we arrived there, many questions occured in our mind. How can she get infected? in what way? why there were many questions in our mind because we saw that the living are were clean and hygiene. No pets, no bird and no chicken in her house.

According to her friend who lived there too, she had not contact with chicken before she got sick and she have had fever 3 days before finally the family took her to Soetomo Hospital.

After we investigated deeply, we found the fact that everyday she went to wet market, which was about 1 km from her house. And in the wet market, we found 2 chicken seller who pooled chicken from many area in East Java. Until now we are still tracking the origin of the chicken and do the surveillance in that wet market.

And on February 2, 2009 we went to Muji Rahayu hospital. It was the first hospital which S came to the medical doctor and hospitalized before she finally was brought to Soetomo Hospital. In this hospital there were 32 health worker who involved directly to treat the victim.

Until now, the investigation is still going on.

FLASH NEWS : we got information that 5 staff of Soetomo Hospital are quarantine because suspected bird flu. One of the suspect is medical doctor.

H5N1 Confirmed Case Concerns in Tangerang Indonesia

Recombinomics Commentary 01:55
February 23, 2009

two residents of the Regency and the Tangerang City South are contaminated with the virus H5N1 or Bird Flu. They were Kaila Nabila (2.9) the resident of the Village of Cilenggang Rt 02/04 the District Cilenggang the Subdistrict Serpong the Tangerang City South and Maya Suparni (15) villagers Mekar Jaya the Subdistrict Sepatan the Tangerang Regency.

information from Litbangkes, Kaila Nabila positive affected H5N1.

The above translation describes two cases with bird flu symptoms. One (2.9M) has died and has been lab confirmed, while the other is a hospitalized suspect case. The two cases are about 10 miles apart (see satellite map).

The number of confirmed cases and clusters in Tangerang has been high. The clusters included the first confirmed cases in Indonesia, in 2005. A recently released phylogenetic tree has four isolates from human cases in 2008. These sequences have been placed on deposit at GISAID and continue to evolve away from the clade 2.1 pandemic target sequence, which was from the first confirmed case.
The continued reports of H5N1 confirmed cases or clusters in Tangerang remains a cause for concern.

Clusters in Indonesia last year led to a news blackout and a significant delay on reporting of cases. Moreover, many milder cases in Indonesia are treated with Tamiflu and not tested. This protocol contributes to the high case fatality rate, which has been close to 80% since 2005 and suggests a high level of unreported milder cases.

The recent fixing of Tamiflu resistance, H274Y in H1N1 seasonal flu has increased concerns that co-infections involving season flu with H274Y and H5N1 in hosts being treated with Tamiflu will selected Tamiflu resistant H5N1.

The latest confirmed and suspect cases in Tangerang continue to increase those concerns.

Lab-made Proteins Neutralize Multiple Strains Of Seasonal And Pandemic Flu

ScienceDaily (Feb. 23, 2009) — Scientists have identified a small family of lab-made proteins that neutralize a broad range of influenza A viruses, including the H5N1 avian virus, the 1918 pandemic influenza virus and seasonal H1N1 flu viruses. These human monoclonal antibodies, identical infection-fighting proteins derived from the same cell lineage, also were found to protect mice from illness caused by H5N1 and other influenza A viruses.

Because large quantities of monoclonal antibodies can be made relatively quickly, after more testing, these influenza-specific monoclonal antibodies potentially could be used in combination with antiviral drugs to prevent or treat the flu during an influenza outbreak or pandemic.

A report describing the research, supported by the National Institute of Allergy and Infectious Diseases (NIAID) of the National Institutes of Health as well as the Centers for Disease Control and Prevention, appears online in Nature Structural & Molecular Biology. Wayne Marasco, M.D., Ph.D., associate professor of medicine at the Dana-Farber Cancer Institute and Harvard Medical School in Boston led the research team, which included collaborators from the Burnham Institute for Medical Research in La Jolla, Calif., and the CDC in Atlanta.

"This is an elegant research finding that holds considerable promise for further development into a medical tool to treat and prevent seasonal as well as pandemic influenza," notes NIAID Director Anthony S. Fauci, M.D. "In the event of an influenza pandemic, human monoclonal antibodies could be an important adjunct to antiviral drugs to contain the outbreak until a vaccine becomes available."

Using standard methods of production, initial doses of a new influenza vaccine to fight pandemic influenza would be expected to take four to six months to produce.

Key to their research, Dr. Marasco and his colleagues discovered and described the atomic structure of an obscure but genetically stable region of the influenza virus to which their monoclonal antibodies bind. The hidden part of the influenza virus is in the neck below the peanut-shaped head of the hemagglutinin (HA) protein. HA and neuraminidase are the two main surface proteins on the influenza virus.

The scientists also identified a new mechanism of antibody action against influenza: Once the antibody binds, the virus cannot change its shape, a step required before it can fuse with and enter the cell it is attempting to infect.

Dr. Marasco, Jianhua Sui, M.D., Ph.D., and other Dana-Farber colleagues began their study with avian flu viruses. They scanned tens of billions of monoclonal antibodies produced in bacterial viruses, or bacteriophages, and found 10 antibodies active against the four major strains of H5N1 avian influenza viruses. Encouraged by these findings, they collaborated with Ruben O. Donis, Ph.D., of the CDC Influenza Division, and found that three of these monoclonal antibodies had broader neutralization capabilities when tested in cell cultures and in mice against representative strains of other known influenza A viruses.

Influenza A viruses can include any one of the 16 known subtypes of HA proteins, which fall into two groups, Group 1 and Group 2. Their monoclonal antibodies neutralized all testable viruses containing the 10 Group 1 HAs--which include the seasonal H1 viruses, the H1 virus that caused the 1918 pandemic and the highly pathogenic avian H5 subtypes--but none of the viruses containing the six Group 2 HAs.

Simultaneously, Dr. Marasco's group teamed up with Robert C. Liddington, Ph.D., professor and chair of the Infectious and Inflammatory Disease Center at Burnham, to determine the atomic structure of one of their monoclonal antibodies bound to the H5N1 HA. Their detailed picture shows one arm of the antibody inserted into a genetically stable pocket in the neck of the HA protein, an interaction that blocks the shape change required for membrane fusion and virus entry into the cell.

When they surveyed more than 6,000 available HA genetic sequences of the 16 HA subtypes, they found the pockets to be very similar within each Group but to be significantly different between the two Groups. The genetically stable pockets, they note, may be a result of evolutionary constraints that enable virus-cell fusion. This could also explain why they did not detect so-called escape mutants, viruses that elude the monoclonal antibodies through genetic mutation.

"One of the most remarkable findings of our work is that we identified a highly conserved region in the neck of the influenza hemagglutinin protein to which humans rarely make antibodies," says Dr. Marasco. "We believe this is because the head of the hemagglutinin protein acts as a decoy by constantly undergoing mutation and thereby attracting the immune system to produce antibodies against it rather than against the pocket in the neck of the protein."

Their findings could also assist vaccine developers. Current influenza vaccines target the constantly mutating head of the HA protein and do not readily generate antibodies against the conserved region in the neck.

"An important goal is to redirect the immune response of vaccines to this invariable region of the hemagglutinin to try to obtain durable lifelong immunity," Dr. Marasco states.

The monoclonal antibodies identified in their paper are very well-characterized, Dr. Marasco notes, and he is optimistic about their further clinical development. "These are fully human monoclonal antibodies that are ready for advanced preclinical testing," he says. He currently is arranging to use NIAID research resources to take the next steps: first, testing the antibodies in ferrets, the gold standard animal model for influenza, and then developing a clinical grade version of one antibody that could enter human clinical trials as soon as 18 months from when the development program begins. Should the antibodies prove safe and effective in humans, it could take several years to develop a licensed product.

Despite the availability of influenza drugs and vaccines, seasonal influenza still kills more than 250,000 people worldwide each year. During seasonal flu outbreaks, monoclonal antibodies could be used to treat individuals with impaired immunity due to pre-existing medical conditions or advanced age. In the event of an influenza pandemic, these individuals plus others at risk--for example, first responders and medical personnel and exposed family members and coworkers--could also benefit from this type of therapy.

Two Tangerang residents Terjangkit Bird Flu, one died

22 February, 2009 by redaksiMetro
CILENGGANG- Kaila Nabila (2.9) the resident Kp cilenggang RT 02/04, the Serpong Subdistrict, the Tangerang City South was suspected of dying resulting from terjangkit the bird flu virus. Apart from Kaila, the Tangerang resident other that is Maya Suparni (15) the resident blossomed Jaya, Sepatan was expected also terjangkit the bird flu virus and currently still in the process of the maintenance in bird flu space of RSU Tangerang. Ade Sulaiman, casualties's father claimed in the date to be 14 Febuari then his child experienced the high fever. And at that time also he at once carried the child both of them this to the community health centre and the clinic of the child's specialist close to his house. The "clinic at that time told so that in referred to RS Fatmawati because of having the affected assumption DBD,"ujar him when being met in his house." And in this RS Fatmawati his child received the maintenance for three days. However because the condition for his child continued to descend finally his life was not helped again. Ironically again, Ade admitted to not knowing definitely what became the cause of the death of his child. "Until this I had not received the news from RS Fatmawati what caused my child to die." And I only received the news from the village staff that Litbangkes said my child terjangkit the flu virus burung,"sesal him. Ade also admitted to submitting on his child's trip and currently the child him has been buried in TPU Kramat Tajug. Despite this he hoped that the Kesehatan Service side of the Tangerang Regency to the front more was open to casualties's family. The death of the resident Cilenggang that was expected terjangkit the bird flu virus was justified by the Training of the Team of the Cepat Movement of Bird Flu and Livestock Breeding of the agricultural service of Kabupaten Tangerang, Zaifiwar. According to him, he has received information from Litbangkes that Kaila Nabila positive affected H5N1. And his side has carried out the taking of the sample of the poultry that in this area. "It will not yet have results, later if having me kabarkan,"ucap him." In the meantime the Tangerang resident other that was expected terjangkit the bird flu virus was Maya Suparni (15) the resident blossomed Jaya, Kecamatan Sepatan. And up to now casualties still in treated in RSU Tangerang. Unfortunately the side of RSU Tangerang and casualties's family reluctantly gave information,

Rapid response team off to affected area

By Lila Baral
JHAPA, Feb 22 - In its bid to control the outbreak of bird flu in other parts of Jhapa district from Sharanamati VDC, the government mobilised a rapid response team in the affecteda area on Saturday.

Laboratory tests conducted on dead chickens of a poultry firm belonging to Sajibul Rahaman in Sharanamati have proved that 149 chickens from the firm died of bird flu a week ago. This is the second outbreak of bird flu in Jhapa and the country.

Following the confirmation, the government, on Sunday, declared emergency in the area within a perimeter of three kilometres from Rahman's poultry farm. The 50-member team will start culling chickens in the designated area from Sunday. Coordinator of the team Dr. Suvash Siwakoti said, “We'll start culling chickens and other birds in the designated area from Sunday morning.”

The team said it would cull over 9,000 chickens and other birds in the area in course of a week. The government had culled around 24,000 birds following the confirmation of a bird flu outbreak in Kakadbhitta in the second week of January.

The government has also set up a bird flu control section at the border police post of Taghandubba to curb import of chickens and poultry products from India.

On Saturday, the 50-member team held discussions with local people and apprised them of safety precautions that need to be taken.

Woman dies of bird flu in Vietnam: officials

Woman dies of bird flu in Vietnam: officials AFP/File – A poultry trader inspects a live duck at a market in Ha Vy village, in northern Vietnam. A 23-year-old …

HANOI (AFP) – A 23-year-old woman has died of bird flu in Vietnam, the countrywhich has the world's second-highest death toll from the virus, medical officials told AFP on Sunday.

The woman died on Saturday, said the director of the hospital where she was being treated in Quang Ninh province.

Her death brings Vietnam's death toll from bird flu to 53 since the end of 2003. Only Indonesia has seen more people killed by the virus.

The patient died at 2:30 pm Saturday from the H5N1 strain of the virus, the hospital director Nguyen Hong Hanh told AFP.

"She and her family had been in contact with chickens that had contracted bird flu," he added.

Since the beginning of the year, three people have been struck with bird flu in Vietnam, all in the north of the country, but the victim was the first to have died.

An eight-year-old girl recovered while a 32-year-old man remains in a Hanoi hospital.

"The risks associated with avian flu are significant," the agriculture ministry's veterinary department said Saturday on its website.

"The concern stems above all from the carelessness of the general population, which generally does nothing to protect itself."

Vietnam managed to contain the virus in 2006, but in 2007 recorded new cases of human infection and deaths. Last year there were five deaths from the H5N1 virus, all in the first quarter of the year.

Ten Vietnamese provinces are currently experiencing bird flu outbreaks.

According to the World Health Organisation, H5N1 has killed 254 people across the world since 2003. The WHO has not yet registered the latest death in Vietnam.

HPAI is spread

TTO - Ngày 21-2, Department of Animal Health or the Mekong Delta provinces to date on avian influenza and other types of disease on animal events are complex and tend to spread out a large scale.

In Bac Lieu, HPAI recurrence in Phuoc Long district are at risk of spread of the surrounding area. Only four days after recurrence, and has spread nearly 20 farms in two years under village communes of Phuoc Long district. Vice veterinary stations Phuoc Long district, said local authorities are having difficulties on the staff just to monitor events closely disease,

Situation of HPAI in the Ca Mau place very complex, 6 / 9 districts, the city has been HPAI. Authorities say the risk of spreading disease is high. Provinces are measures necessary to the process object purchase, transportation of poultry without quarantine, restrict spread.

Has 10 provinces including Ca Mau, Soc Trang, Nghe An, Hau Giang, Quang Ninh, Quang Tri, Bac Ninh, Bac Lieu, Khanh Hoa and Ninh Binh have HPAI not last 21 days.

According to tests by about epidemiology of central, patients Ly Tai Mui to be positive for influenza A H5N1 virus.

Although the doctor's Provincial Hospital in collaboration with the Institute of Medicine Clinical tropical countries, Bach Mai hospital in the treatment exposed it special for patients with positive influenza A H5N1, but the situation patients with more severe symptoms: overflowing on the two lungs, respiratory failure level progress, the target still, liver failure, failure has increased, and internal poisoning. After 18 days of treatment was positive, Ly Tai Mui has died.

Initial estimates, the cost of treatment for patients about 20 million / day.

* As of 21-2, HPAI has reoccurred in 4 provinces in the Mekong Delta Ca Mau, Bac Lieu, Soc Trang and Hau Giang. In Ca Mau, the local recurrence HPAI first Mekong Delta, up to 18-2, (more than 1 month after detecting the first outbreak) flu occurred in 4 communes of 3 districts is Tran Van Time, Thoi Binh and Tan Phu. Number of birds have all the 10,000 children, most of which are run of ducks, not two full vaccination nose stipulated.

In the provinces of Soc Trang, Bac Lieu and Hau Giang, prevention of HPAI are being carried out urgently. Nguyen Hien Trung, Department Chief Veterinary Department Hau Giang, said: Its Health Hau Giang has just received results 4 / 5 flocks of ducks with the amount of the 2790 Vi Thuy district (where the outbreaks occurred 8 and 9-2) with positive influenza A H5N1.

There are 16 provinces appear on diseases of cattle, poultry

(22/02/2009-9: 07)

Outside agencies, the prevention on GSGC disease requires the participation of the people.

On 21-2, from Department of Animal Health (MARD), the country has 16 provinces appear on diseases of cattle and poultry are: Hoa Binh, Quang Binh, Kon Tum, Bac Kan, Quang Ninh, Quang Nam , Phu Tho, Ca Mau, Soc Trang, Nghe An, Hau Giang, Quang Tri, Bac Ninh, Bac Lieu, Khanh Hoa and Ninh Binh.

Just from the start to this week (16-2 to 21-2), the list of provinces with HPAI, and lo mồm long móng on animals, blue ear disease on pigs continuously extend from 10 provinces increased to 16 provinces. The individual Quang Ninh appear all 3 types of diseases mentioned above, besides, the province Quang Nam also appear on and lo mồm móng long ears and blue.

So, until this time on all 3 domains: North, Central, South both have local disease appear on the cattle and poultry. Worrying, although local in the country, especially provinces are HPAI, and lo mồm long mong, disasters have implemented green measures for prevention, but more disease is not signs with them, but the Bùng and risk spreading.

In fact that is not only concerned about the number of provinces increased in the list, but at some provinces have, throughout each day to continue to detect more number of cattle and poultry infected ... There are many causes, but over here that need to raise awareness of prevention disease on cattle and poultry in the provinces.
