Nathan Wolfe: The Man Who Tracks Viruses Before They Spread
The Viral Storm
The Dawn of a New Pandemic Age
by Nathan Wolfe
Hardcover, 304 pages | purchase
The New Yorker once called virologist Nathan Wolfe "the world's most prominent virus hunter." Wolfe, the director of the Global Viral Forecasting Initiative, spends his days tracking emerging infectious diseases before they turn into deadly pandemics.
In The Viral Storm, Wolfe describes how most of those emerging infectious diseases originally start out in animals before making the jump to us.
-snip-
Avian Flu
Wolfe also recently traveled to Asia, where the first cases of the H5N1 flu were spotted several years ago. H5N1 has killed hundreds of people around the world since it reemerged in 2003.
The original strain of H5N1, explains Wolfe, jumped directly from birds to humans. Other viruses, meanwhile, make the leap to domestic animals before infecting humans; that mixing can then introduce deadlier strains of unknown viruses into the world.
"So what happens is you have a particular pig out there and it could get infected with two different viruses — maybe one that's been in human and one that's been in the original reservoir of a bird — and they can mix and match their genes and create mosaic daughter viruses that will have completely novel properties," he says.
That's where things can get particularly deadly. Wolfe says he thinks of viruses as having two dials: one for how transmissible they are and one for how deadly they are. The H1N1 virus, which spread to over 200 countries in 2009 and 2010, is considered to be extremely transmissible — but not extremely deadly. Meanwhile a different strain of bird flu — the H5N1 — is incredibly deadly but didn't spread as effectively as the H1N1 strain. Wolfe's team tracked people who had H1N1 but he says they were more interested in people with the deadlier strain.
"And the reason why was because we were really scared that these H5 and H1 viruses would come together in a particular human or pig and create one [virus] that was off the [transmissible and deadly] dials," he says. "And that's of course the devastating pandemic we're trying to avoid."

India Government frees sale of Tamiflu, only drug effective against swine flu and bird flu
Government frees sale of Tamiflu, only drug effective against swine flu and bird flu
India has decided to lift "restricted sale" of the drug and make it available through all chemist shops against a doctor's prescription. Till now, oseltamivir was the only drug available for treatment of H1N1 swine flu virus humans and indiscriminate and unregulated use access to this drug would have made it ineffective.
That's why the drug was kept under Schedule X of the Drugs and Cosmetics Rules, 1945 which meant it could only be sold by 480 select chemists who had permission to sell Schedule X drugs. The drug had to be kept under lock and key and in premises only responsible people had access to. Record of sales of this drug had to be maintained for a period of two years.
However, the Drug Technical Advisory Board (DTAB) on October 10 decided to bring the drug under Schedule H which means it can be stocked by chemists but sold only against prescription of a registered medical practitioner.
"Till now, limited access was permitted to make these formulations available to the needy patient only and avoid its indiscriminate use. Once the notification is made that oseltamivir phosphate and zanamivir formulations have been brought under Schedule H, as decided in the DTAB meeting on October 10, all chemists in India will be allowed to stock and sell it. However, over the counter sale of these formulations won't be allowed. Chemists will have to sell it against prescription," a health ministry official said.
Tamiflu is a class of medications called neuraminidase inhibitors. These drugs work by stopping the spread of the influenza (flu) virus in the body. It helps shorten the time you have flu symptoms such as a stuffy or runny nose, sore throat, cough, muscle or joint aches, tiredness, headache, fever and chills. Companies that make this drug include Cipla, Metco, Hetero, Strides and Roche.
This drug is presently being used as the first line of treatment in the US, Europe and Japan for cure of seasonal influenza.
DTAB said, "India now has an indigenous swine flu vaccine developed in 2010. Three vaccine manufacturers have already been granted permission to manufacture and market H1N1 vaccine - Serum Institute in Pune, Cadila Healthcare in Ahmedabad and Bharat Biotech in Hyderabad. In view of the fact that incidence of swine flu has come down, it would be suitable to include the drugs under schedule H." http://timesofindia.indiatimes.com/india/Government-frees-sale-of-Tamiflu-only-drug-effective-against-swine-flu-and-bird-flu/articleshow/10381749.cms

Two Suspected Cases of Bird Flu in Mataram
Mataram, West Nusa Tenggara.
Two people suspected to have contracted the H5N1 strain of avian influenza are being treated at West Nusa Tenggara General Hospital in Mataram.
The two patients were identified as 11-month-old Muhammad Aditya from Praya in West Lombok district and 53-year-old Inaq Sahnun from Pringgarata in Central Lombok district.
Mentari, Sahnun’s daughter-in-law, said that two days earlier Sahnun began running a fever, had a cough and shortness of breath. She was taken to a hospital in Praya where she was placed under observation for two hours before being sent to Mataram for suspected bird flu.
“The doctor at Praya suspected [Sahnun] had contracted bird flu, but said further tests were needed,” Mentari said.
Sahnun’s house is located in an avian influenza-endemic area, where more than 400 chickens were reported to have died suddenly. Preliminary findings suggest the cause of death may have been bird flu.
Sahwan, the father of Aditya, the other patient, said his son had been running a fever for two days. He said he brought Aditya to a public heath center in Pringgarata before taking him to Mataram.
Sahan, the infant’s grandmother, said he often played with the around 100 chickens that the family raised at their house. Twenty of the chickens died suddenly in the past week, also suspected to be from bird flu.
“Besides, Aditya’s father’s chickens, many of the neighbors’ chickens are also dying, even today,” she said. “Hundreds have already died in that area.”
Sahan added that Sahwan was a temporary worker with the local livestock agency and had in recent weeks been helping spray disinfectant in chicken pens and educating residents on how to respond to a suspected bird flu outbreak.
“He explained to them that they should bury or incinerate the dead poultry, but many residents continue to just dump the dead birds in open drains,” she said.
The two patients were brought to the hospital on Saturday afternoon and have been isolated from other patients. Aditya, however, was accompanied in his room by his mother, who wore a face mask.
Both Aditya and Sahnun have been put on intravenous drips as doctors await the arrival of a special team dealing with human bird flu cases.
There has been a sudden increase in the number of poultry dying unexpectedly in Lombok in the past fortnight, with thousands reported dead in Central Lombok district and Mataram.
The spike comes as the neighboring resort island of Bali also grapples with a surge in poultry deaths attributed to bird flu. Two children were confirmed to have died from the disease there earlier this month. http://www.thejakartaglobe.com/home/two-suspected-cases-of-bird-flu/471993
