Draft Guidance on Allocating and Targeting Pandemic Influenza Vaccine

Introduction

Effective allocation of vaccines will play a critical role in preventing influenza and reducing its effects on health and society when a pandemic arrives. The specific type of influenza that causes a pandemic will not be known until it occurs. Developing a new vaccine in response will take several months and pandemic vaccine may not be available when cases first occur in the United States. Moreover, once vaccine production begins, it will not be possible to make enough new vaccine to protect everyone in the early stages of a pandemic.

The U.S. Government is taking steps to minimize the need to make vaccine allocation decisions by supporting efforts to increase domestic influenza vaccine production capacity. Significant funding is being provided to develop new vaccine technologies that allow production of enough pandemic influenza vaccine for any person in the United States who wants to be vaccinated within six months of a pandemic declaration. Until this goal is met, Federal, State, local and tribal governments, communities, and the private sector will need guidance on who should be vaccinated earlier during the pandemic to best protect our people, communities, and country.

Issues to consider in drafting guidance on pandemic influenza vaccination are different and more complex than in developing recommendations for vaccination against annual influenza. In contrast with annual influenza, during a pandemic nobody in the population is likely to have immunity to the virus, many more people will become ill, and rates of severe illness, complications and death are likely to be much higher and more widely distributed throughout the population. The greater frequency and severity of disease will increase the burden on health care providers and institutions and may disrupt critical products and services in health care and other sectors. National and homeland security could be threatened if illness among military and other critical personnel undermines their capabilities. Because the needs that must be addressed by pandemic vaccination differ from annual influenza vaccination, the guidance on vaccination differs as well.

This draft guidance is intended to provide strong advice to support planning an effective and consistent pandemic response by States and communities. Nevertheless, it is important that plans are flexible as the guidance may be modified based on the status of vaccine technology, the characteristics of pandemic illness, and risk groups for severe disease – factors that will remain unknown until a pandemic actually occurs.

The Federal Government has embarked on a rigorous and collaborative process that seeks input from all interested parties in developing this strategy. Hearing opinions from persons and organizations with a wide variety of interests and concerns is the best way to ensure that allocation of vaccine in the early stages of a pandemic is fair and provides the best chance for our country to emerge from a pandemic with minimal levels of illness, death, and disruption to our society and economy.

Public and stakeholder input have been central to the development of this draft guidance and will continue to be crucial as the guidance is finalized. Comments received on this draft guidance will be considered by a Federal Government working group made up of members of various U.S. Government agencies, and will contribute to a final guidance document. Comments on this draft strategy will be solicited through:

  • Public meetings,
  • Stakeholders meetings,
  • Web-based public dialogue, and
  • A request for comments posted at www.pandemicflu.gov and in the Federal Register.

Vaccination will be only one of several tools that can be used to fight the spread of influenza when a pandemic emerges. Other approaches include public health measures in communities, businesses, and households to reduce and slow the spread of infection; using antiviral medications for treatment and prevention; and washing hands and covering coughs and sneezes. These strategies will be the initial mainstay of a pandemic response before vaccine is available and continue to have important effects as vaccination availability progresses. Guidance around vaccine use is meant to be applied in conjunction with and in the context of these other pandemic response efforts. 2009-H1N1 Influenza Vaccine Development Next Steps: Questions and Answers

1. What do the new orders on these contracts support?

The objective of the new orders on these contracts is to allow the U.S. government to manufacture and test pre-pandemic vaccines against the newly emerging 2009-H1N1 virus for the U.S. pre-pandemic influenza vaccine stockpile. This is an important step in order to build a stockpile of at least 40 million doses of 2009-H1N1 vaccine and add it to the stockpile of pre-pandemic vaccines. Because of the virus’ recent emergence, there were no doses of 2009-H1N1 in the U.S. stockpile.

2. How is the 2009-H1N1 different from seasonal influenza?

Each influenza season H1N1 is one of the viruses circulating in people and causing disease. The 2009-H1N1 has acquired H1 and N1 genes from swine to create a new version of the H1N1 virus (called 2009-H1N1) that is significantly different from the seasonal H1N1 that has been circulating in people.

3. Will the seasonal vaccine protect me against the 2009-H1N1 influenza?

Preliminary analysis of serum from people immunized with the seasonal influenza vaccine done at the CDC suggests that the seasonal vaccine is unlikely to provide protection against 2009-H1N1.

4. Will this vaccine be made differently than the seasonal influenza vaccine?

No. This vaccine will be made using the same processes and facilities that are used to make the currently licensed seasonal influenza vaccines.

5. Will all this vaccine be injectable?

No. Some of the vaccine will be a live attenuated vaccine that is sprayed into the nose to immunize a person.

6. What about other vaccine manufacturing technologies, like recombinant or cell-based vaccines?

For this phase the U.S. government response has been focused on the currently licensed vaccine manufacturers. We are continuing to investigate how the U.S. government may use other technologies as part of the response to the 2009-H1N1.

7. When will the work on the 2009-H1N1 vaccine start?

The work has already started to develop a vaccine, and these new orders on existing contracts are for the first large-scale manufacturing of ingredients that could be used in a potential H1N1 vaccine. Laboratories are already working on the generation of the seed viruses needed for vaccine production. These should be available for the distribution to the vaccine manufacturers around June 1. Once the manufacturers have completed their seasonal influenza vaccine production, they will start production of the 2009-H1N1 vaccine.

8. What testing will be done on these vaccines?

In addition to the usual testing done for seasonal influenza vaccines, there will be clinical testing in people to determine the most effective and safest dose to generate a strong immune response to the 2009-H1N1 virus.

9. How many doses will be needed to be immunized?

This will be determined during the clinical studies. It could be one dose, like the seasonal vaccine, but it could be two doses since the population has little or no immunity to the 2009-H1N1 virus, which has not infected humans previously.

10. What is an adjuvant?

An adjuvant is an additive to a vaccine that helps to generate a stronger immune response to the vaccine. When using an adjuvant it is often possible to reduce the size of the vaccine dose and the number of doses needed. So the use of an adjuvant can reduce the amount of vaccine needed to immunize a population.

11. Does the current seasonal influenza vaccine use an adjuvant?

No, there are currently no U.S. licensed influenza vaccines with adjuvant.

12. How will it be decided if an adjuvant will be used in this vaccine?

Experts will review the safety and immunogenicity data from the clinical testing of this vaccine and make a determination based on what is the safest and most effective dosage for the generation of a strong immune response.

13. What is an influenza pandemic?

An influenza pandemic is a global outbreak of disease that occurs when a new influenza virus appears in the human population, causes serious illness, and then spreads easily from person to person. Unlike seasonal influenza, people will have little immunity to the virus that causes a pandemic.

At the Canadian-Egyptian Business Council, they discussed the seriousness of Swine Flu, but they also went on to discuss the seriousness of the impact of plague and epidemics. That would be plural. “epidemics”.

In Kafr El-Sheikh, the Governor had a meeting with health officials to address the epidemic diseases. That would be a plural. “diseases”. Also, Dr. Hatem el-Gabali, pointed out a quarantine will be opened and all the cities to quarantine the university to receive the pilgrims and Umram performers, with the full cooperation from the Saudi side, to review all points.

At a group meeting with “Doctors Without Rights”, Dr. Hatem el-Gabali said to expect a rise in H1N1 to reach hundreds of thousands, to the point where the count ends.

Dr. Hatem el-Gabali, has also reserved 2.5 million doses of Tamiflu to bring the total to 5 million reserves.

In Menofia, they plan to have convoys in ten villages for treatment outside of hospitals, for bird flu and swine flu. This is to reduce the central crowds rush in the event of an outbreak of avian influenza and swine flu. The convoy would have a clinic for inner chest children, the abdominal, and a pharmacy for disbursement of necessary treatment, free of charge.

In Giza Governorate, the decision has been made to monitor all entrances and exits to avoid transmission of the plague.

The Minister of Health, Dr. Hatem el-Gabali has also decided to urgently establish a center for management for the crisis of swine flu epidemic. This center will be electronically linked to 29 provinces and the World Health Center, and it’s regional office in Cairo, as well as the CDC in Atlanta. He also says, the process of elimination is not realistic. The goal is put in place to respond to “any adjustments that might arise in the field”.

In Alexandria, they have decided to have separate hospitals for Bird Flu, and Swine Flu. Even though, they don’t haven’t had any current cases of Bird Flu, I guess it should have its own designation.

Major General Ahmed Abdeen held the governor of Kafr el-Sheikh, a meeting with officials of health and preventive medicine, Kfar-Sheikh on Sunday to discuss preparations for the maintenance of the Directorate of Health to address the epidemic diseases.

7 relatives catch swine flu in West Bank village

The Palestinian health minister says seven people from one extended family have been diagnosed with swine flu.

Health Minister Fathi Abu Mughli says two siblings returning from the US infected five of their relatives in the northern West Bank village of Barka. In total, 11 Palestinians in the West Bank have contracted swine flu. About 300 Israelis have been diagnosed with swine flu. (AP)

1st domestic case in Cambodia

Cambodia's health minister on Saturday [27 Jun 2009] announced the
country's 1st domestic case of swine flu, days after its 1st
infections were reported among visiting American students.

One Cambodian national and one Filipino traveller had tested positive
for the A (H1N1) virus, Mam Bun Heng told AFP, bringing the country's
toll to 6.

"(They) are now being taken good care of at hospital and are getting
better," he told AFP on Saturday [27 Jun 2009]. "We are working hard
to... prevent more domestic infections within our country," he added.

Four US students on a school trip in Cambodia were confirmed this
week as the country's 1st cases of the virus.

Globally, the World Health Organization says nearly 60 000 cases of
the disease have now been recorded, with 263 deaths.

Laos confirms 1st 3 cases of new flu

Laos and the World Health Organization confirmed on Thursday [25 Jun
2009] the communist country's 1st 3 cases of the H1N1 flu virus, all
of them foreigners.

A 9-year-old American boy and an Australian man were recovering after
being infected with the virus also known as swine flu, government
spokesman Khenthong Nuanthasing said.

Another foreigner, whose nationality was not known, had also
contracted the virus and was being treated in hospital in the capital
Vientiane, he said.

"The Australian is living in Laos but has fully recovered," he said
by telephone. "The other 2 cases were detected by a scanner when they
entered the country at the airport."

The WHO office in Laos confirmed the cases and said all 3 were recovering.

Neighbouring Cambodia confirmed 3 more cases on Thursday [25 Jun
2009], all of which were American students. Cambodia reported its 1st
case on Wednesday [24 Jun 2009], a 16-year-old American from the same group.

"All 4 persons are in a stable condition and recovering well without
any complications," Cambodia and the WHO said in a joint statement.

More than 1000 people have been infected in Thailand, although no
deaths have been reported.
As the H1N1 virus has spread around the globe, the WHO has declared
an influenza pandemic and advised governments to prepare for a
long-term battle against the virus.


There have been more than 56 000 confirmed cases of the H1N1 virus
and at least 249 people have died.

Thailand puts flu death toll at 2

The influenza A (H1N1) virus has killed 2 people in Thailand,
Thailand's Public Health Ministry confirmed on Saturday [27 Jun 2009].

The 1st victim, a 40-year-old woman from Bangkok, died from pneumonia
and haemoptysis, Witthaya Kaewparadai, public health minister, told a
press conference at 11 a.m. local time to brief details on the flu death.
The woman, who was sent to a private hospital on [8 Jun 2009] after
falling ill, developed fever and dyspnea and died on [20 Jun 2009],
Dr Kumnuan Ungchusak, director of the Office of Epidemiology under
the Department of Disease Control, said along with Mr Witthaya.

An earlier report said that the woman died around 0300 a.m. local
time Saturday [20 Jun 2009].


Another victim, a 42-year-old male engineer from the central province
of Chonburi, also died from complications on [26 Jun 2009], Mr Witthaya said.

As of Saturday noon [27 Jun 2009], the ministry confirmed 1209
influenza A (H1N1) cases. Of them, over 1100 have recovered, and 22
of them, with 2 in critical condition, are being treated in hospitals.

National tally rises to 84 as 8 more H1N1 cases found

The Ministry of Health Friday [26 Jun 2009] confirmed 8 more cases of
influenza A, one in Hanoi and the others in Ho Chi Minh City [HCMC].

Two of the cases were HCMC locals who had come into physical contact
with some patients of the flu. The other 6 were returning home from
infected areas overseas.

As of Friday evening [26 Jun 2009], Viet Nam had detected 84 cases of
H1N1 infections, including 73 in the south, 7 in the north and 4 in
the central region. The 1st case was detected in HCMC on [31 May 2009].

Of the 84 patients, 32 have recovered and been discharged from
hospitals, and no case has proved fatal so far.

The disease, also known as swine flu, has so far been reported only
in people coming from abroad or locals who have had physical contact
with the former.

Health officials have requested passengers arriving at Noi Bai and
Tan Son Nhat International Airports on flights with the patients
detected so far should isolate themselves and contact local health
officials as soon as they develop flu symptoms.

Authorities watchful after virus detected in 20 villages

At least 20 villages in Purbalingga regency, Central Java, have been
hit with cases of bird flu, forcing local authorities to remain alert
to prevent it from infecting other areas, officials said Thursday [25
Jun 2009]. Purbalingga husbandry office head Hartono said the
contagious disease had 1st been detected in the regency in January 2009
.

More than 1000 infected chickens had been culled between January and
May [2009] by local authorities, he said. Hartono said his office was
collecting data and information on the spread of bird flu this month
[June 2009].

"We are continuing to collect data based on information from local
people whose chickens have died suddenly. The tests *on those birds*
have come back positive for bird flu," he told The Jakarta Post. The
chickens were culled, and their bodies incinerated, he said.

"We don't want to take any risks, and such a *culling method* is based
on existing official procedures," he said. Hartono said most of the
virus-infected poultry were domestic chickens that were not kept in
cages. "Because local villagers traditionally have chickens roaming
free, not kept in cages, bird flu has spread very quickly,"
he said.

To curb the virus from spreading further in Purbalingga, the local
authorities have set up a special team to combat bird flu.

"The team is already working 24 hours a day. Any time we receive a
report on a dead chicken we will go to the location immediately
,"
Hartono said. The dead chickens, he said, would be used as samples for
tests.

Purbalingga has also received 240 000 doses of vaccine from the World
Health Organization to help control the spread of bird flu, Hartono
said.

"Around 30 percent of the vaccines have already been used."

The WHO has also provided Purbalingga with anti-bird flu disinfectants.

"We have enough medical stocks to deal with bird flu cases. What we
need to do more is improve people's awareness so they will keep their
poultry cages clean," Hartono said.

More Indonesians have died from avian influenza than anywhere else in
the world, with 31 of 33 provinces known to have been infected by HPAI
H5N1, which has become endemic. The country's 240 million people are
spread over 6000 inhabited islands. Since 2006, FAO has helped to
train teams of local veterinarians and para-veterinarians in
'participatory disease surveillance and response (PDS/R)' techniques.
Veterinarians and paravets are engaging community members, tapping
into their local knowledge and involving them in control efforts.
Villagers are also trained to detect and report bird flu cases in
poultry and be responsible for their own safety and that of their
families.

In 2006, the veterinary programme was 1st implemented as a pilot
programme in 12 districts, rapidly expanding to cover 159 districts,
comprising the islands of Java and Bali, as well as 2 provinces of
Sumatra, by May 2007. When the programme was initiated, the extent of
HPAI infection was not known. Participatory disease surveillance
enhanced the sensitivity of the national surveillance system, and
within 3 months of establishing the PDS/R programme it became clear
that HPAI was circulating unimpeded in backyard poultry, with several
outbreaks detected per district per month in the pilot areas. Field
diagnosis of HPAI in Indonesia is based upon detecting active
outbreaks that meet the definition of a 'sudden death outbreak' and
which return positive results for the Antigen type A influenza rapid
test. Within the 1st 12 months of operation, the PDS/R programme
detected 800 HPAI disease events. The large number of outbreaks
detected by the PDS teams overwhelmed the response capacity of the
district animal health infrastructure, and led to recognition of the
need to re-evaluate the national control strategy.

MANILA, Philippines -- The Influenza A(H1N1) virus would generally go away with time and paracetamol and a good decongestant could manage the symptoms caused by the virus in persons without chronic illnesses, a health official said.

“The illness is self-limiting,” Health Undersecretary Mario Villaverde told the Philippine Daily Inquirer (parent company of INQUIRER.net) in a phone interview.

“If you have fever, you take paracetamol. If you have colds, you take a decongestant,” he added.

Villaverde’s statements come in the wake of a Department of Health (DoH) memorandum issued last week, that limits the government’s distribution of the anti-flu drug oseltamivir only to patients who are considered high-risk: infants or the elderly, pregnant women and those with chronic illnesses such as diabetes, asthma, hypertension, and HIV (Human Immunodeficiency Virus), among others.

“Generally, the cases in the Philippines and in other countries are mild,” Villaverde said when asked why oseltamivir would no longer be given to all A(H1N1) cases except those with existing medical conditions.

Close contacts of A(H1N1)-positive patients would no longer be given oseltamivir, the DoH guidelines said. Exceptions to this rule are health workers who came in contact with the patients without the necessary protective gear such as face masks.

Villaverde answered in the negative when asked if the surge in confirmed cases in the country—861 as of Friday—was a factor in limiting the government’s distribution of oseltamivir to avoid a shortage in the near future.

“Personally, I think we could have done this earlier,” Villaverde said.

He added United States has been limiting oseltamivir to high-risk cases. The US entered the mitigation phase much earlier because of the faster spread of the virus there, said health officials.

The DoH at the start of the A(H1N1) crisis said that the government had a stockpile of 1.5 million doses of oseltamivir.

Oseltamivir is now reportedly going for P150 a dose.

The DoH’s mitigation response at the beginning of a community outbreak gives particular attention to the vulnerable group of people who are likely to develop complications.

* Prepare well-equipped isolation wards in all state-run hospitals and health facilities for the treatment of A (H1N1) influenza cases;

* Raise the alert level for A (H1N1) influenza to the World Health Organization (WHO) level for increased alertness and preparedness; and

* Tap the pool of still to be employed nurses in helping organize information and education teams to elevate public awareness on proper hygiene and mitigation efforts against the disease.

The health department declared last week a low-level community outbreak in Metro Manila and later revised its guidelines with education officials regarding the suspension of classes in schools with A (H1N1) cases.

More cases

In Davao City, a Dabawenyo, who attended a seminar in Metro Manila, tested positive for influenza A (H1N1), bringing the total number of swine flu cases in Mindanao to four as of Saturday.

Davao Medical Center (DMC) chief Dr. Leopoldo Vega said the latest case is a 34-year-old male, a resident of Cabantian, Buhangin, Davao City.

He said the victim attended a seminar in Metro Manila where the speaker was a Malaysian who later tested positive of the swine flu virus upon returning to Malaysia.

Vega said the 34-year-old is now on home quarantine.

The boy from Butuan City will be released anytime soon after his condition gets better. The boy's playmate, a nine-year-old girl will also be released.

Also testing positive of the virus were a 37-year-old from Kidapawan City and a 20-year-old from Baguio District in Davao City. Both are on home quarantine.

The number of suspected cases that were observed but cleared by health authorities in the region has reached to almost 50 since the outbreak of the swine flu virus two months ago. Only four, however, were confirmed cases.

In Magalang town in Pampanga province, the Pampanga Agricultural College suspended its classes on all levels since Friday after 40 people manifested flu-like illnesses.

School officials immediately suspended classes, while the DOH conducted swab testing on 36 of the 40 possible victims.

"So far we have swab tested some 36 of them and the samples have been forwarded to Manila and we are hoping that these are not positive cases. If in case there would be any that would turn positive, the people would surely be the first to know," Magpantay told Sun.Star.

Don Bosco Academy in Mabalacat town has also suspended classes since last June 22 and is set to resume on July 6 after some 40 people showed flu-like illnesses.

Mabalacat Mayor "Boking" Morales said some 30 high school students did not report to their class last Monday after they manifested flu-like symptoms.

"Three students who did report to class were also found to have fever so swab samples were taken," he said.

Magpantay said 10 students from the said school have already undergone swab testing but the DOH is still awaiting the results of the tests.

In Manila, Ateneo de Manila University reported in its website on Friday that three students of the Ateneo Grade School were confirmed infected with the swine flu virus and are now on home quarantine.

Hog raisers hurting

President Arroyo also ordered Agriculture Secretary Arthur Yap to help the "tens of thousands" of silent but hurting hog raisers whose plight has been largely ignored.

She cited reports that backyard and small-scale hog raisers are suffering as farm gate prices dropped because of "baseless fears about unsafe pork."

Pork prices earlier dropped due to misconceptions that the influenza virus earlier identified as "swine flu" can be contracted by eating pork.

Arroyo said the Department of Agriculture (DA) should assure consumers there is no swine flu in local hog farms.

"Local pork is safe to eat, there is no single case of swine flu report from the Bureau of Animal Industry and the National Meat Inspection Service," she said.

"On top of properly informing the public, the DA should carry out all preparations necessary to prevent swine flu from infecting local hog farms," she added.

Updates from the rest of Asia showed that more than 1,200 people in Thailand have contracted the dreaded virus as of Saturday, including seven Navy cadets.

In Australia, 3,519 people had been diagnosed with the virus. A 26-year-old woman who also had a serious pre-existing medical condition died late Friday after spending several days in Royal Perth Hospital's intensive care unit, Western Australia chief health officer Tarun Weeramanthri said.

Myanmar reported its first case of the disease on Saturday in a 13-year-old girl who recently returned from Singapore.

New Zealand raised its tally of confirmed swine flu cases to 507.

In Hong Kong, the government confirmed 33 new cases, taking the city's total to 629. There have been no reports of deaths related to the virus in the territory.

Second Buffalo student dies of swine flu

A 9-year-old Buffalo girl died of swine flu Saturday — the second Buffalo School District student to die of the disease in a week.

Maya Harden, who had been in critical condition in Women & Children’s Hospital for almost two weeks, died Saturday, her mother said.

“They tried everything to get her lungs circulating,” Maya’s mother, Akea Hollingsworth, said later in the day.

The girl’s death comes one week after 15-year-old Matthew Davis, an eighth-grader at Buffalo’s Harvey Austin School 97 on Sycamore Street, died from swine flu complications.

Maya, a fourth-grader at the Charles R. Drew Science Magnet School 59, first went to the emergency room June 12, because of flulike symptoms, her mother said.

Doctors thought it was pneumonia, and she was sent home to recuperate.

At first, Maya seemed to be getting better, but then took a turn for the worse, her mother said. Hollingsworth took her back to the hospital June 14.

Maya was again sent home early the next morning, but became lethargic, Hollingsworth said. On June 16, Hollingsworth took her daughter to the hospital for the third time in less than a week.

Over the past 11 days, Maya had been in critical but stable

condition. She died Saturday morning.

In Matthew’s case, by the time Matthew entered the hospital, he was seriously ill with the flu, as well as co-infected with a type of bacteria known as methicillin-resistant staphylococcus aureus, or MRSA.

MRSA has been a problem in hospitals and nursing homes for decades, but has also spread to otherwise healthy people in the community, living normally on the skin and in the nose and throat. Research suggests that the overuse of antibiotics has contributed to the problem.

This bacteria can cause infections of the skin, nose, throat and ear. But in a small number of cases, especially in patients with weakened immune systems, it can lead to life-threatening pneumonia and blood infections.

It was unclear Saturday whether Maya suffered similar complications.

Hollingsworth talked about her daughter.

“She was a beautiful little girl,” the mother said. “She was boisterous, full of personality. She was healthy with no severe health issues. She had a sense of humor, very caring. She would give the shirt off her back to someone that needed it.”

Maya was a huge Beyonce fan. Just before Hollingsworth spoke with The Buffalo News by phone Saturday, she listened to Beyonce’s song “Halo,” which will be played at Maya’s wake.

“Everything happens for a reason,” Hollingsworth said. “God doesn’t put more on you than you can bear.”

James A. Williams, district superintendent, issued a statement.

“It is with a heavy heart and deep sadness that . . . we mourn the loss of another student. Please keep the family in your thoughts and prayers during this difficult time while they struggle with this terrible loss.”

Williams also pointed to the seriousness of the disease.

“. . . We ask parents again to remain vigilant in stopping the spread of H1N1. Please have your child wash and sanitize their hands as often as possible. The best way to remain disease free is to maintain proper hygiene and wash your hands on a regular basis.”

Philip Rumore, president of the Buffalo Teachers Federation, also expressed condolences.

He said the teachers union has made some recommendations to the district, including making schools a site for flu shots and having hand sanitizers on the school buses, as well as the classrooms.

The district also needs to make a greater effort to let parents know that if their child has a high fever they should go to the doctor and not to school, Rumore said.

“We have to all work together to ensure that no other parent or child has to go through this,” Rumore said.

And as Hollingsworth grieved for her daughter Saturday, she also thought about Matthew Davis.

“Her and Matthew’s death hopefully are not in vain,” she said. “They are now angels looking down from heaven.”

In Thailand, Navy Commander Admiral Kamthorn Pumhiran said the seven cadets who tested positive for the virus were at Sattahib Naval Base, about 80 miles (130 kilometers) south of Bangkok.
He said the other cadets with symptoms were isolated in separate quarters at the base. They are all in stable condition and being tested for the virus. Thailand reported 69 new cases Thursday, bringing the national total to 1,054.
Cambodian Health Minister Mam Bun Heng told reporters that three students who arrived from the U.S. state of Texas on June 19 had tested positive for the virus. They ranged in age from 16 to 20 and raised Cambodia's total cases to four.
On Wednesday, authorities announced the country's first swine flu case in a 16-year-old girl—part of the student group—who developed flu symptoms a day after arriving. She sought medical care Monday.
The World Health Organization said in a statement that the four Americans were recovering well without any complications.
In Papua New Guinea, the Post Courier newspaper reported Tuesday that Health Minister Sasa Zibe had been placed under quarantine with flu symptoms and was awaiting results of tests.

أعرب عدد من خبراء البيئة في مصر عن مخاوفهم من خطة وزارة الزراعة في التخلص بشكل كامل من الفئران التي تنتشر بشكل كبير في حقول الأراضي المتاخمة لنهر النيل، خوفاً من انتشار الطاعون بعد ظهوره في ليبيا، وحذروا من إحداث خلل بيئي ووضع المصريين في مواجهة مباشرة مع الثعابين.

In terms of the virologic testing that's being done, the new H1N1 virus is now making up more than 99% of all the typed isolates we're testing here in the U.S. with our collaborating laboratories, so virtually all the of the influenza that's circulating that's getting a diagnosis is this new strain right now here in our summer months. We're also tracking milder illness, outpatient visits for what we call influenza-like ill this and this is now returning below the national baseline but in -- at the national level, but two of our ten regions of the country are still above what we think of as a baseline for this time of year. Those two regions are New England and the New Jersey/New York area, regions 1 and 2, where they're still seeing more influenza-like illness than you would expect to see at this time of year. Those regional numbers can mask outbreaks of clusters at smaller geographic areas and we know some communities are still coping with outbreaks of this disease. Influenza-like illness increase during week 24 in 6 of the 10 regions compared to week 23. That's unusual. Usually this time of year, things would be going down instead of going up.

We continue to say there most of the impact of this new virus is affecting younger people compared with what we see with the seasonal flu when, of course, seniors, people over 65, are so greatly affected. We're seeing high rates of illness among people under 50. The highest rates are in those under 25. When we look at hospitalized cases, nearly 80% of people who have been hospitalized in the U.S. and reported to us have been under 50. The median age of hospitalized cases is 19 years old. When we look at the most severe outcomes, the people who have died, the age is a bit older. The median age of those who have died is 37, still quite young for anyone to be dying of an infectious disease, but a bit older than the hospitalizations and the average cases.

We do expect that this new strain is going to continue to spread in the Southern Hemisphere and intensify over the weeks ahead and we are continuing to watch this closely. We'll be watching for changes in the virus. We have not seen any changes in the virus that are important as of this point. We'll also be watching for changes in disease patterns. We've mentioned that people over 65 don't seem to be getting this illness or getting it in very great numbers here in the U.S. We'll be watching to see whether older people start to get this virus in the Southern Hemisphere as the flu season progresses. Those kinds of concerns have not shown up yet but we're continuing to watch. We'll also be continuing to watch the virus to see if it develops resistance to the oseltamivir type of drugs. We haven't seen that yet but it's the kind of thing we'll continue to look for.

Helen Branswell: Thank you very much. I was hoping I could ask couple of questions if I could. The first one is, did the CDC have an estimate as this point of how many people who die have underlying conditions? We had earlier heard maybe as many as half of the people who were dying were previously healthy, but increasingly, it seems like all of the death notices, almost all of the death notices have reference to underlying conditions. And the second question I wanted to ask is are you doing any sero surveys in parts of the U.S. to try to get a better handle on how many people have been infected in this wave?

Anne Schuchat: Yes, thank you. The vast majority of the fatalities that we hear of or that are officially reported to us do occur in people with underlying conditions. It's not 100%. It's more on the on order of three-fourths of them at this point. We're continuing to track that and get additional information. We're up to 127 deaths have been reported to us, but we don't have that underlying condition information yet on all of them

Nablus – Ma’an – The overall number of swine flu cases diagnosed in the northern West Bank city of Nablus rose to 11 after the virus was communicated by others, according to the director of the Nablus department in the Health Ministry, Dr Khalid Al-Qadiri on Sunday.

Al-Qadiri asserted that the Palestinian Ministry of Health has quarantined affected patients in western Nablus, and that a delegation comprising himself, the governor of Nablus, Jamal Muheisin, and other officials visited on Sunday morning the area where H1N1 had begun to spread. He insisted that the situation is under control.

Sources told Ma’an that some visitors who came from the US were carrying the virus, and they communicated it to other members of their families in western Nablus. Palestinian Minister of Health Fathi Abu Mughli is scheduled to hold a news conference on Sunday to address the phenomenon.
