HONOLULU -- The Hawaii Department of Health announced on Wednesday that it will no longer provide the public updates on the number of new cases of swine flu each week.

DOH officials said they would instead direct people to the Centers for Disease Control and Prevention that gets the information from each state.

The state did announce that 218 new cases were found in the past week. That brings the total number of cases to 516 since the first case was confirmed on May 5.

BAGHDAD, June 24 (Xinhua) -- Iraq has announced first six cases of A/H1N1 virus in the country's northern Kurdish region, the regional minister of health said Wednesday.

"After medical tests for the six people, including a team of female athletes returning from the United States and an Iraqi Arabcitizen, at a hospital in the city of Sulaimaniyah, they are confirmed with the A/H1N1 flu," Dr. Zriyad Othman, health minister of Kurdish regional government, said in a statement.

Othman said that all the affected people have been treated and are in good condition.

Those affected people are part of a basketball team that had returned from Chicago, and have been kept in hospital since June 21.

On their way back from Chicago the team passed through Amman where one member was held because tests found she had the A/H1N1 virus.

The WHO declared A/H1N1 flu pandemic on June 11.

شهد اليوم الأربعاء احتجاز 15 حالة بمستشفيات الحميات في خمس محافظات للاشتباه بإصابتهم بفيروس أنفلونزا الطيور، فيما أسفرت حملات مكافحة المرض في عدد من المحافظات عن ضبط وإعدام (6597) طائر، وإزالة (513) عشة لتربية الطيور في المنازل والأسواق.

أعلن الدكتور عبد الرحمن شاهين المتحدث الرسمى لوزارة الصحة اكتشاف ثلاث حالات إصابة جديدة بفيروس AH1N1 المعروف بأنفلونزا الخنازير ليبلغ إجمالى عدد حالات الإصابة البشرية بالمرض فى مصر إلى 44 حالة حتى الآن.

احتجز مستشفى حميات المنيا عبد الصبور محمود سيد قناوى (40 سنة)، مدرس مقيم بأسيوط، للاشتباه فى إصابته بمرض أنفلونزا الطيور، عقب ارتفاع شديد فى درجة حرارته، بعد انتهاء مراقبته بلجنة الثانوية العامة بمركز مغاغة.

IT APPARENTLY has been decided by someone in Ulster County that the pandemic spread of a new flu virus, which has been spurring governments and health organizations to feverish action around the globe, is now of no special concern to the public.



As in, “Don’t worry — we’re handling it.”



We must have missed that memo.



Both the Ulster County Health Department and the Kingston school district last week took what charitably might be characterized as a father-knows-best response to the second and third confirmed cases of swine flu in Ulster County, one a student in the Kingston school system.



The Health Department confirmed the two new cases, while declining to give any information whatsoever about one of the cases.

As for the other case, involving a Kingston district student, well, it was too much for everyone involved to even identify the school involved or grade.



A spokesman for Ulster County Executive Michael Hein passed the buck on the issue of this paucity of public information. The state Health Department, said Vincent Martello, recommends saying only whether an infected person is a child or adult, lest the patient’s privacy be disturbed.



The state Health Department also recommends school districts closely monitor illness, in part through absenteeism.

As for the state Education Department, it washes its hands of the matter by saying decisions about whether schools should continue to operate or close should be made by local districts in consultation with local health officials.



In effect, a school could have one, five, 15 or 100 cases of swine flu and you, as a parent, wouldn’t know it until such time as the authorities that be decide amongst themselves that the illness has reached some sort of critical mass demanding action. Until that time, the whole thing is, by policy, sub rosa as a matter of inviolate individual privacy.



With all due respect to privacy, which we all value, that’s just screwy.



In effect, this cluster-fluence of policies and recommendations means this: The school district alone is acting as the guardian of your child’s health with regard to an illness not terribly well understood yet by the medical world. It is doing so by monitoring how many cases a particular school gets before deciding — presumably in private consultation with health agencies — whether to close a given school.



Which leads to this question: Is my child or your child required to become part of a statistically significant number of sick students before all other parents are notified? 

By all appearances, the answer is, yes, quite possibly.



That’s an approach designed, at best, to protect the herd, at the expense of individuals. All done in the name of privacy. 

Many parents undoubtedly would like the option of keeping their children home if a school is known to have cases, especially if their children have “underlying conditions” that make them mortally vulnerable to swine flu. And if you think that has to be something rare like congenital heart problems, think again. The death of a 9-year-old swine flu victim in Florida last week was attributed to the “underlying condition” of asthma.

 There’s a clear contradiction here. Public health authorities and governments are scrambling to plan for the contingency that the H1N1 flu virus will prove itself a scourge. A good part of that plan — perhaps even the better part of that preparation — will assume the active cooperation of the public. But it is doubtful, at best, that such cooperation will be easily enlisted by civic institutions that have made a practice of throttling information and freezing out the public.

A 6-year-old Brampton girl with swine flu died only a day after she began showing flu-like symptoms.

The child, who attended Roberta Bondar Public School, passed away a week ago and it was only discovered after she died that she had the H1N1 influenza virus.

The exact cause of death has not yet been determined, but the virus is known to have been a contributing factor, Dr. Arlene King, Ontario's chief medical officer of health, told a news conference yesterday.

The child got a fever and began vomiting on Sunday, June 14, and died the following day. An autopsy was performed and lab results revealed last Friday she did indeed have the virus. She is the fourth Ontarian with the virus to die.

Quebec health officials released a statement late yesterday afternoon to announce two new deaths in the province. They gave no details and did not return calls.

The deaths of the two Quebecers and the Brampton girl bring to 16 the number of Canadians to die after contracting swine flu.

Though the child, whose identity has not been made public, did not have any apparent pre-existing medical problems, the coroner's office is now trying to determine whether she had an undiagnosed condition. It's not known how the girl contracted the virus, but two members of her extended family had mild cases.

Officials are providing few details about the girl's death, including where she died. All King would say was that she did not die in hospital or at home.

Yesterday, Dr. David Mowat, medical officer of health for Peel, sent a letter to students, parents and staff at the elementary school, advising them that the girl was not symptomatic when she was in class.

"As such, the risk of transmission of H1N1 flu virus from this student to others in the school is very low," he wrote.

King said children at the school should continue attending class unless they feel ill. "At this time, we do not recommend that any schools be closed if there is a suspect or confirmed case," she said.

There have been more than 2,650 confirmed cases of swine flu in Ontario – and more than 5,700 across Canada – but most are considered mild with symptoms very similar to an annual seasonal flu.



Meantime, an international conference on disaster management being held in Toronto heard that private businesses are purchasing their own supplies of antiviral medications to prevent employees from being infected.

Amin Mawani, a professor with the Schulich School of Business at York University, said it's advisable for companies to take such measures to curb employee absenteeism and protect profits in the event that a second, more severe wave of the virus strikes in the fall.

"Pandemic planning can be viewed as insurance," he said.

But Alison Stuart, assistant deputy minister of public health, said the province is not advocating that such measures be taken.

"We have not recommended to either the health sector or more broadly to other sectors in Ontario that they stockpile antivirals as (preventative measures)."

Best Clinical Management For H5N1 Infection Debated

The best ways of managing patients with H5N1 infection (avian influenza) are debated by experts in this week's open access journal PLoS Medicine.

Higher than recommended doses of the antiviral drug oseltamivir (Tamiflu) should be used to fight H5N1 influenza, argues Nicholas White (Mahidol University, Bangkok, Thailand). In contrast to the current WHO guidelines recommending that oseltamivir be given at a dose of 75 mg twice daily for five days, Dr. White argues that higher doses should be given for H5N1 infection to avoid any possibility of under-dosing those patients with unusual pharmacokinetics and more resistant organisms. This will come at the expense of increased toxicity, he says, but is necessary given the mortality burden of H5N1 infection and the fact that H5N1 replicates more rapidly than seasonal influenza viruses, reaches much greater viral burdens than do other human influenza viruses, and resistance develops swiftly.

Robert Webster and Elena Govorkova from St. Jude Children's Research Hospital in Memphis, USA, writing in response to Nicholas White's article, disagree. They argue that we must instead consider a multidrug approach to managing patients with H5N1, an approach that is supported by animal data and "can guard against the emergence of resistant strains." Tim Uyeki from the Centers for Disease Control and Prevention in Atlanta, USA, emphasizes the need for more data to help inform clinical management of patients with H5N1 infections. In the absence of these data, he argues, we need a multipronged strategy: pharmacological strategies including combination antiviral treatment, anti-inflammatory agents, and immunotherapy, and non-pharmacological strategies such as the standardization of optimal ventilator and fluid management, especially for acute respiratory distress syndrome, and management of other complications.

In a 2007 article in PLoS Medicine (PLoS Med 4(5): e119), Holger J. Schünemann and colleagues described a new process used by the World Health Organization for rapidly developing clinical management guidelines in emergency situations. These situations include outbreaks of emerging infectious diseases. The authors discussed how they developed such a "rapid advice" guideline for the pharmacological management of avian influenza A (H5N1) virus infection. The guideline recommends giving the antiviral drug oseltamivir at a dose of 75 mg twice daily for five days.

Indonesia confirms first two A/H1N1 flu cases

JAKARTA, June 24 (Xinhua) -- Indonesian Health Minister Siti Fadilah Supari confirmed here on Wednesday that a 37-year-old Indonesian pilot and a 22-year-old British woman were tested positive for A/H1N1 flu virus in the country.

This is the first time for Indonesia to report A/H1N1 flu cases. The minister said that both patients were infected abroad.

The pilot visited Perth of Australia on June 16 and China's Hong Kong on June 18 and got medical treatment at Sulianti Saroso Hospital in Indonesia on June 19 because of having fever, Supari said.

The British woman visited Bali island on June 19, after visiting Melbourne of Australia, she said.

On June 20 she had fever, and cough, and she came to hospital by showing a health alert card obtained from Ngurah Rai airport in Bali and then she was treated at Sang Lah Hospital, the minister added.

"The two cases H1N1 are cases which come from overseas, it (the virus) has not spread in Indonesia," she told a press conference here.

To prevent the spread of the virus, the health ministry boosted precaution measures already taken nationwide, the minister said. Visitors from infected countries, such as Australia, would have special surveillance, the minister said.

The minister said that new steps would be taken such as intensifying communication among health authorities in the region and stepping up surveillance in Bali, where many visitors from Australia, which has been infected by the virus with 2,436 cases, are coming in.

"With our neighboring countries, communication would be intensified through telephone. Should a patient is found in a plane, (for example), it would be examined all the passenger in the three line vertical and horizontal," she said.

As the spread of the virus in Australia was high, the minister said that the authorities gave every visitor from the country a health alert card.

"Honestly, I worry about those coming from Australia. We take a special measure at surveillance post in Bali to prevent those coming from Australia for vacation to Bali bring the virus," she said.

Vietnam: Bird flu returns to northern province [poultry]

QUANG NINH — Bird flu has resurfaced in the country in northestern border Quang Ninh Province’s Yen Hung District and infected more than 500 poultry.

An additional 1,300 birds have been killed and disposed of to prevent disease spread according to regulations on epidemic disease prevention, said Hoang Van Nam, head of the Veterinary Department.

Deputy minister of Agriculture and Rural Development Diep Kinh Tan said the reason for the problem was the lack of disease prevention measures taken by some localities.

Tan directed the Veterinary Department to thoroughly handle the disease in Quang Ninh and to soon build a centre for quarantining poultry and cattle. He also asked localities nationwide to concentrate on vaccinating duck flocks, as they were highly susceptible to the spread of the flu.

As many as 154 million tonnes of poultry have been vaccinated so far.

No cases of foot and mouth disease or blue-ear pig disease have been found this week. —VNS

In a related context, Major-General Hassan Hemeida, Vice-Chairman of the Higher Committee, to address the avian flu, which was formed by the Council of Ministers for the Eradication of the disease, said that the Committee has clauses to compensate farmers and educators in the home, in the case affected by bird flu, he said, pointing out that these conditions will help in the eradication of the disease.

وأضاف لـ«المصرى اليوم» أن التعويضات ستكون للملتزمين بشروط الوقاية البيطرية الآمنة. He told Asharq «Egyptian today» that the compensation would be committed to the terms of the prevention of veterinary safe.

وطالب حميدة، أصحاب المزارع والمربين بتحصين الدجاج والإبلاغ عن إصابات الطيور بالفيروس وعدم تسريبها لبيعها فى الأسواق. He called benign, farmers and breeders to vaccinate chickens and the reporting of influenza virus infections and non-diversion of the sale in the markets.

وأكد أن وزارة الزراعة والمنظمات الدولية ستشارك فى تمويل صندوق التعويضات للمتضررين من أنفلونزا الطيور. He stressed that the Ministry of Agriculture and international organizations will participate in the financing of the Compensation Fund for those affected by avian influenza.

وقال إن الحكومة سوف تقوم بنقل مزارع الثروة الداجنة من داخل المناطق السكنية إلى المناطق الصحراوية قريبا، كما ستقيم مجمعات صناعية لتطويرها والنهوض بها وإنشاء مصانع لتحويل المخلفات الناجمة عن تلك الصناعة إلى سماد عضوى، مشيرا إلى أن المجمعات الصناعية تعمل على انخفاض أسعار بيع الدواجن للمستهلكين وتوفيرها بكثرة فى داخل الأحياء والمحافظات. وأشار إلى أن أسعار الأراضى الصحراوية أقل من أسعار الأراضى المقامة عليها المزارع حاليا سواء كانت أراضى داخل الكتلة السكنية أو زراعية، مؤكدا أن الحكومة تستطيع القضاء على أنفلونزا الطيور عند نقل المزارع إلى المناطق الصحراوية ـ على حد قوله. وأضاف أن معظم الإصابات البشرية لأنفلونزا الطيور سببها التربية المنزلية، مشيرا إلى أنها أحد معوقات القضاء على المرض، وأن وزارة الزراعة لا تستطيع تحصين دجاج التربية المنزلية بنسبة ١٠٠%. He added that most human cases of avian influenza caused by domestic education, he said it one of the impediments to the eradication of the disease, and the Ministry of Agriculture can not vaccinated chickens per household of 100%.

وأكد أن خطط اللجنة قابلة للتغير إذا وجدت أى أسباب طارئة للسيطرة على الفيروس والقضاء عليه. He stressed that the Commission's plans are subject to change if there are any reasons for the emergency control of the virus and eliminate it.

وأشار إلى أن اللجنة ترصد الحالات المصابة سواء من الطيور أو البشر أولا بأول وتتخذ الإجراءات المناسبة طبقا لطبيعة نوع الإصابة. He pointed out that the Committee monitors the cases of both birds or humans-you-go and take the appropriate action according to the nature of the type of injury.
