Two Flocks Reported with Sub-Clinical ND
MEXICO - Evidence has been found of sub-clinical Newcastle disease (ND) in two commercial poultry flocks in the centre of the country.

The veterinary authority sent two Immediate Notification reports dated 22 November to the World Organisation for Animal Health (OIE).

The first report describes a positive reaction to routine surveillance at a farm at Col. Montoro in the region of Aguascalientes on 26 October. All 850,816 birds were destroyed.

The second report covers another outbreak of sub-clinical disease at a commercial broiler farm at Road Lagos de Moreno-San Luis Potosí in the region of Jalisco, where all 30,000 birds in the flock have been destroyed.

Both reports say that identification of the pathogenic strain of the Newcastle disease virus was made by active epidemiological surveillance and there were no clinical signs of the disease. ISRAEL - The Israeli veterinary authorities have reported five new outbreaks of Newcastle disease in Hazafon, Hamerkaz and also in the Jehuda and Samaria region.

The World Organisation for Animal Health (OIE) received follow-up report no. 3 on Monday, 21 November. The affected population ranges from three to six week old broilers.

In the case of 4-week-old broilers, the disease was diagnosed in two adjacent farms. The birds in the first farm were culled (38,000) and the birds in the second farm were culled on yesterday, 23 November. Regarding 6-week-old broilers, the OIE reports that they will be culled shortly. In the case of 5-week-old broilers divided in two pens, only one pen was affected. The two pens will be culled by today, 24 November. In Jehuda and Samaria, 6-week-old broilers were also affected. The birds will be culled shortly. According to the OIE, it should be noted that this outbreak is located into Israeli Settlements in the West Bank.

All in all, a total of five outbreaks have been identified, out of which 556000 broilers have shown signs of susceptibility and 176000 cases were reported. While a total of 31100 deaths were recorded, 56900 birds were destroyed.

The source of the outbreak remains inconclusive and the the epidemiological investigation is ongoing.

Highly Pathogenic Avian Influenza Virus H5N1 Infection in a Long-Distance Migrant Shorebird under Migratory and Non-Migratory States

Abstract

Corticosterone regulates physiological changes preparing wild birds for migration. It also modulates the immune system and may lead to increased susceptibility to infection, with implications for the spread of pathogens, including highly pathogenic avian influenza virus (HPAIV) H5N1. The red knot (Calidris canutus islandica) displays migratory changes in captivity and was used as a model to assess the effect of high plasma concentration of corticosterone on HPAIV H5N1 infection. We inoculated knots during pre-migration (N = 6), fueling (N = 5), migration (N = 9) and post-migration periods (N = 6). Knots from all groups shed similar viral titers for up to 5 days post-inoculation (dpi), peaking at 1 to 3 dpi. Lesions of acute encephalitis, associated with virus replication in neurons, were seen in 1 to 2 knots per group, leading to neurological disease and death at 5 to 11 dpi. Therefore, the risk of HPAIV H5N1 infection in wild birds and of potential transmission between wild birds and poultry may be similar at different times of the year, irrespective of wild birds' migratory status. However, in knots inoculated during the migration period, viral shedding levels positively correlated with pre-inoculation plasma concentration of corticosterone. Of these, knots that did not become productively infected had lower plasma concentration of corticosterone. Conversely, elevated plasma concentration of corticosterone did not result in an increased probability to develop clinical disease. These results suggest that birds with elevated plasma concentration of corticosterone at the time of migration (ready to migrate) may be more susceptible to acquisition of infection and shed higher viral titers—before the onset of clinical disease—than birds with low concentration of corticosterone (not ready for take-off). Yet, they may not be more prone to the development of clinical disease. Therefore, assuming no effect of sub-clinical infection on the likelihood of migratory take-off, this may favor the spread of HPAIV H5N1 by migratory birds over long distances.

St. Thomas Elgin General Hospital staff declared an outbreak of two antibiotic resistant bacteria Friday afternoon on the hospital's second surgical floor but seem to have contained it.

Following four hospital-contracted cases of Vancomycin Resistant Enterococcus (VRE), two more of Methicillin-resistant Staphylococcus aureus (MRSA) and one MRSA infection over the past month and a subsequent consultation with Elgin St. Thomas Public Health, STEGH staff put contingency plans in place and declared the outbreak.

Most of the cases are in a dormant form in which the patient doesn't display symptoms but risks infecting others.

"They (the seven cases) were contracted within the organization and that was where our concern came in," STEGH VP Patient Services Brenda Lambert said.

"We have many patients within the organization who are colonized (with the two organisms) and we isolate them. These individuals, unfortunately, contracted them while they were within our organization...so that the rationale for us declaring the outbreak."

MRSA and VRE, however, are both common, antibiotic-resistant superbugs and it's not unusual for either to be present within a hospital. Patients who are caught with them are isolated to prevent further spread.

STEGH had another upsurge of MRSA a month ago at which time five patients acquired the germs during their stay.

A release from the hospital states the precautions taken to prevent further transmission included: extensive curtain and furniture cleaning and more hand hygiene audits.

Hospital visitors are also being asked to wash their hands before they enter the site as well as when they leave.

Lambert adds there have been no new cases of either since the measures were taken but says they're measures hospital staff will likely continue to enforce as if there was a perpetual outbreak.

"We are going to be continuing to be very stringent with our protocols and really upping the ante on a continual basis to ensure that we can maintain this because our ideal is that absolutely no patients get it while they're in their building," she said.

 

But Lambert says they're precautions they'll likely have to continue forever...

Nov 21, 2011 (CIDRAP News) – Egypt reported five H5N1 avian influenza outbreaks in October, and the latest disease surveillance from Indonesia suggests that the highest incidence is in Bali, where three human fatal infections were recently reported, according to an overview from the United Nations Food and Agriculture Organization (FAO).

The Egyptian H5N1 outbreaks struck four governorates: Beheira, Fayoum, Gharbia, and Giza. The event in Beheira occurred at a farm where poultry was vaccinated against H5N1. The other four outbreaks affected unvaccinated backyard or rooftop poultry, according to the FAO.

In Indonesia, disease surveillance authorities said Bali had the highest incidence of villages affected by highly pathogenic avian flu in September, at 5.6 per 1,000, followed by Central Sulawesi (4.3 per 1,000), West Sulawesi (3.1), West Kalimantan (3.1), and South Sulawesi (2.8) provinces. The figures are from the Participatory Disease Surveillance and Response program, which covers 29 of the country's 33 provinces.

In October, the H5N1 virus killed three Bali residents, a 29-year-old woman and two of her children, a 5-year-old girl and a 10-year-old boy. Poultry in the family's household had died before the woman and her children got sick.

Outbreaks were also reported in two villages in East Nusa Tenggara province, the FAO reported.

More recently, the virus struck poultry in three subdistricts in South Sulawesi province, killing about 14,000 birds, according to a local media report cited on Nov 19 by Bird Flu Information Corner, a Web message board operated by Kobe University in Japan and Airlangga University in Indonesia.

The FAO overview of the latest H5N1 outbreaks also includes detections in Vietnam and Iran, which had been previously reported to the World Organization for Animal Health.

So far this year, 14 countries have reported H5N1 outbreaks, according to a chart in the FAO report.

H5N1 is endemic in a handful of countries, including China, Vietnam, Indonesia, Bangladesh, India, and Egypt.

An advisory group on the new World Health Organization framework agreement on pandemic influenza preparedness is preparing to meet for the first time from 21-23 November.

The advisory group is one of three “pillars” of the pandemic influenza preparedness (PIP) framework launched at the May 2011 World Health Assembly (IPW, WHO, 24 May 2011). As of last week, the 18 members of the advisory group were still being finalised, according to sources.

At next week’s meeting the advisory group is expected to set the priorities and the agenda for the group.

The other pillars are a benefit-sharing system and an influenza virus traceability mechanism, according the WHO webpage on the PIP framework.

Of the advisory group, the WHO website states:

“The Advisory Group is one of the three pillars of the PIP Framework’s “Governance and Review” structure. The Advisory Group is to monitor implementation of the PIP Framework and provide evidence-based reporting, assessment and recommendations regarding functioning of the PIP Framework. The Advisory Group will provide guidance and input to the Director-General, presenting an annual report on its evaluation of implementation of the PIP Framework.

The PIP Framework identifies 7 specific areas that should be covered in the annual report of the Advisory Group:

Necessary technical capacities of WHO GISRS.
Operational functioning of WHO GISRS.
WHO GISRS influenza pandemic preparedness priorities, guidelines and best practices (e.g. vaccine stockpiles, capacity building).
Increasing and enhancing surveillance for H5N1 and other influenza viruses with human pandemic potential.
The Influenza Virus Tracking Mechanism.
The sharing of influenza viruses and access to vaccines and other benefits.

LA JOLLA, CA – Scientists from The Scripps Research Institute and the US Army’s Medical Research Institute of Infectious Diseases have isolated and analyzed an antibody that neutralizes Sudan virus, a major species of ebolavirus and one of the most dangerous human pathogens.
We suspect that we’ve found a key spot for neutralizing ebolaviruses,” said Scripps Research Associate Professor Erica Ollmann Saphire, who led the study with US Army virologist John M. Dye.
The new findings, which were reported November 20, 2011, in an advance online edition of Nature Structural & Molecular Biology, show the antibody attaches to Sudan virus in a way that links two segments of its coat protein, reducing their freedom of movement and severely hindering the virus’s ability to infect cells. The protein-linking strategy appears to be the same as that used by a previously discovered neutralizing antibody against the best-known ebolavirus species, Ebola-Zaire. The new study suggests that this may be the best way for vaccines and antibody-based therapies to stop ebolaviruses.
Deadly Outbreaks
Ebolaviruses first drew the attention of the medical world with simultaneous deadly outbreaks in 1976 in the nations of Sudan and Zaire (currently known as the Democratic Republic of the Congo). These two outbreaks were caused by the two major viruses: Ebola-Sudan and Ebola-Zaire, and early field studies showed that sera from patients that survived one virus could not help patients infected with the other. . Both viruses persist in animal hosts–probably bats–and when they spread to humans, typically cause severe hemorrhagic fevers, killing up to 90 percent of people they sicken. Although not as contagious as influenza or measles, ebolaviruses can be transmitted in bodily fluids including exhaled airborne droplets, and scientists who study these viruses are generally required to use special “Biosafety Level 4” facilities. The US government regards the ebolaviruses as a potential bioterror threat.
Ebolavirus researchers hope to develop a vaccine that could be used to protect health workers and others in the vicinity of ebolavirus outbreaks, as well as an antibody-based immunotherapy that could help infected people survive. However, these tasks are complicated by the fact that there are now five recognized species of ebolavirus: Ebola-Zaire, also known simply as Ebola virus; Taï Forest virus; Reston virus; Bundibugyo virus; and Sudan virus.
“These species differ enough from each other that neutralizing antibodies to one don’t protect against the rest,” said Ollmann Saphire. “Sudan virus is a particular concern because it has caused about half of the ebolavirus outbreaks so far, including the largest outbreak yet recorded.”
Uncovering the Body’s Natural Protection
US government researchers recently demonstrated that an experimental vaccine containing proteins from Ebola and Sudan viruses provides monkeys with some protection against those viruses. But precisely how the vaccine works is unclear, and it has never been tested in humans. Moreover, until now no laboratory has isolated a neutralizing antibody against Sudan virus.
To find such an antibody, Dye and his colleagues at Fort Detrick, Maryland, injected lab mice with a harmless virus engineered to make copies of the Sudan virus coat protein. The coat protein provoked the mice’s immune B cells to make various antibodies against it, and the scientists were able to reproduce the mice’s repertoire of antibodies by harvesting their B-cells and culturing them in the lab. Testing each type of antibody for its ability to block the infection of cells with Sudan virus, the researchers found one good candidate, antibody 16F6, which not only neutralized Sudan virus in the lab dish but also significantly delayed the deaths of infected mice. They then sent 16F6 to Ollmann Saphire’s lab at Scripps Research in California.
"We were very excited about developing this antibody as a potential treatment for Ebola virus,” said Dye. “Collaborating with the Ollmann Saphire lab to determine the binding site was the perfect complement to our previous work"
Ollmann Saphire’s lab specializes in the use of X-ray crystallography and related techniques to visualize the atomic-scale details of viruses bound by antibodies. These details reveal where on a virus an antibody binds, and if the antibody is one that neutralizes a virus’s ability to infect cells, its binding site usually offers important clues to the virus’s workings and vulnerabilities.
In the new study, Ollmann Saphire’s team found that 16F6 attaches to the Sudan virus in a way that links two segments of the viral coat protein. The virus is known to use one of these segments, GP1, to grab hold of a host cell. When this happens, the cell automatically brings the virus inside, encapsulated within a bubble-like chamber known as an endosome. Normally the cell would destroy the contents of such an endosome, but Sudan virus–like some other viruses–employs its other viral coat-protein segment, GP2, to fuse to the wall of the endosome so that it and the rest of the virus escape into the doomed cell’s interior. Antibody 16F6 seems to prevent this fusion process from happening by keeping GP2 bound to GP1.

The virus is like a wolf in sheep’s clothing because its outer part is covered with human sugar molecules, that the antibodies do not see as foreign,” said Ollmann Saphire. “The binding site of the 16F6 antibody is one of the few places where viral protein is exposed, and it’s exposed because it’s a place where GP1 and GP2 need to be free to move.” To fuse to the endosomal wall, GP2 must separate from GP1 and uncoil itself. When it is held fast to GP1 by the antibody 16F6, GP2 can’t uncoil and perform its function–and so the Sudan virus, instead of escaping into the relatively unprotected interior of the cell, stays within the endosome and is eventually destroyed.
A Strategy Against Ebolaviruses
Ollmann Saphire and her colleagues suspect that 16F6’s protein-linking strategy is the best one that antibodies have against ebolaviruses. The antibody’s binding site on the Sudan virus coat protein is virtually the same as the binding site of an Ebola-Zaire-neutralizing antibody known as KZ52, which Ollmann Saphire and Scripps Research colleague Professor Dennis Burton found and analyzed three years ago. KZ52 is derived from antibodies made by an African patient who survived an Ebola-Zaire outbreak in 1995, and aside from 16F6 it is the only ebolavirus-neutralizing antibody whose binding site has been determined with X-ray crystallography.
“We think it’s not just a coincidence that these two different antibodies, evoked in two different host species by two different ebolaviruses, use the same strategy of linking GP1 and GP2,” Ollmann Saphire said.
She and her colleagues now are trying to obtain structural data on several other ebolavirus-neutralizing antibodies, and she suspects that at least one of these also works by linking GP1 to GP2. “There may be other neutralizing sites on ebolaviruses, but so far the only one we’ve found is this one,” she said.
The recognition that ebolavirus-neutralizing antibodies share this protein-linking strategy should guide the further development of vaccines and immunotherapies. “It helps us to understand more precisely what an ebolavirus vaccine or immunotherapy ought to do,” Ollmann Saphire said.
The lead authors of the paper, “A shared structural solution for neutralizing ebolaviruses,” are João M. Dias, a research associate in the Ollmann Saphire lab who is now a senior scientist at Heptares Therapeutics in the UK; and Ana I. Kuehne, a researcher in the Dye laboratory at Ft. Detrick. The other authors are Majidat A. Muhammad and Eugene Kang of the Dye lab at Ft. Detrick; Dafna M. Abelson, Shridhar Bale and Marnie L. Fusco of Scripps Research; Anthony C. Wong and Kartik Chandran of the Albert Einstein College of Medicine; Peter Halfmann of the University of Wisconsin at Madison; and Yoshihiro Kawaoka of the University of Tokyo. Sounding a fresh bird flu alert, the Tripura government has imposed a ban on import of poultry birds, ducks and other poultry products from West Bengal and Assam, an official said Saturday.

“The ban on import of poultry and bird flu alert was sounded earlier this week following the outbreak of the avian influenza in neighbouring Assam and West Bengal,” the animal resource development department official told reporters.
Bird flu was officially detected and confirmed recently in certain parts of Nadia district in West Bengal and Dhubri district in western Assam,” he added.

The official said said the Tripura government has set up check gates at the border and entry points in the state’s northern part adjoining Assam, and asked security forces to maintain a strict vigil along the India-Bangladesh borders to prevent the import of poultry and poultry products.
However, the state would allow import of poultry products and birds from Andhra Pradesh (from where the maximum amount of poultry birds and poultry products were being supplied to northeastern states) and other states excluding Assam and West Bengal.
Meanwhile, the Tripura government has urged the central government to bear the entire cost of compensation to the northeastern states.
“As India’s northeastern states bordering Bangladesh have become vulnerable to bird flu with the contiguous disease outbreak intermittently across the border and as well as the smuggling of poultry products into India, the centre should bear the full costs of compensation,” Agriculture and Animal Resource Development Minister Aghore Debbarma said.
“With the sporadic outbreak of bird flu, thousands of poor poultry cultivators have been affected in Tripura and other northeastern states.
The contiguous disease has also been affecting the fragile economy of the northeastern region,” the minister said here at a meeting.
Four states – Tripura, Meghalaya, Mizoram and Assam, which shares 1,880-km border with Bangladesh – were occasionally affected by avian influenza forcing the authorities to cull lakhs of poultry birds and ducks besides destroying huge quantities of poultry products.

Nov 17, 2011 (CIDRAP News) – A national biosecurity board that monitors "dual use" research is apparently worried about an as-yet-published study in which a mutant form of H5N1 avian influenza virus was found to be easily transmissible in ferrets, which are considered good models for flu in humans.

A National Public Radio (NPR) report today said the National Science Advisory Board for Biosecurity (NSABB) is scrutinizing the research by Dr. Ron Fouchier of Erasmus University Medical Center in the Netherlands. The board provides guidance on biological research that has a legitimate purpose but could be misused to endanger public health.

Dr. Paul S. Keim, acting chair of the NSABB, said today that the board is conducting a review of H5N1 virus transmission in mammals, but because of the board's confidentiality rules, he could not give any details. Keim is director of pathogen genomics in the Translational Genomics Research Institute at Northern Arizona University.

Referring to the Office of Biotechnology Activities in the National Institutes of Health's Office of Science Policy, Keim told CIDRAP News, "We have now been directed by OBA staff that we can acknowledge that a review process on the H5N1 transmissibility in mammals is under way. We are not allowed to provide additional details."

The H5N1 virus causes human illness relatively rarely, but it is often deadly when it does, with a case-fatality rate of about 60% among cases confirmed by the World Health Organization. Though the virus has circulated in poultry in many countries since 2003, it has not gained the ability to spread easily in humans. Scientists worry that if it did gain that ability, it could spark a fearsome pandemic.

With the goal of identifying genetic changes that could lead to greater person-to-person transmissibility, a number of researchers have introduced mutations in the virus and studied how the mutant strains behaved in animals.

Fouchier gave a general description of his experiments at a European meeting in September, according to a news story published in Scientific American after the meeting. He and his team introduced various mutations into the virus and watched their effects on its ability to attach to human respiratory tract cells. They found that with as few as five single mutations, the virus could bind to nasal and tracheal cells, according to the story.

But when tested in ferrets, this mutant virus still didn't spread very easily through close contact. Fouchier and his team then undertook to let the virus evolve naturally—a project that he described as "really, really stupid," according to the story. They inoculated one ferret with the mutant virus, and after it got sick, they exposed a second ferret to infectious material from the first one.

After they repeated this process 10 times, "H5N1 became as easily transmissible as seasonal flu," the story said. Fouchier said he concluded from this that H5N1 viruses "can become airborne" and do not need to reassort with other mammalian flu viruses to do so.

The report quoted another expert, Albert Osterhaus, also of Erasmus University, as saying the individual mutations that Fouchier introduced have already occurred naturally in animals, but not together.

Further details about Fouchier's study have been hard to come by. In response to a CIDRAP News query last week, Fouchier said he couldn't comment on the study until it is published. Other flu experts, including those at the Centers for Disease Control and Prevention (CDC), also declined to discuss the study or didn't reply to queries last week.

The NPR report said the Fouchier study is the subject of a "fierce debate" among disease experts and is being reviewed by the NSABB. The experiment was criticized by some experts quoted in the NPR report.

"It's just a bad idea for scientists to turn a lethal virus into a lethal and highly contagious virus, and it's a second bad idea to publish how they did it so others can copy it," said Thomas V. Inglesby, MD, director of the Center for Biosecurity at the University of Pittsburgh Medical Center, who is not a member of the NSABB.

While biology has a culture of openness and relies on the full sharing of findings, occasional exceptions to this policy are warranted, and Fouchier's study calls for an exception, he told NPR.

It was not immediately clear just how the NSABB might influence whether and in what form Fouchier's findings would be published. The board makes recommendations on policies governing publication, public communication, and dissemination of dual use research methods and results, according to information on the OBA Web site. It is up to the federal government to convene relevant agencies to determine how to respond to the recommendations.

One previous case in which the NSABB influenced a research publication pertained to reports on the reconstruction of the 1918 pandemic flu virus using material from preserved tissue samples, according to Keim.

"In this case NSABB recommended that the papers be modified to better represent the biosafety aspects of the research and also to explain the positive benefits of the work to public health," he told CIDRAP News. "There were no restrictions recommended by NSABB on the actual data or limitations to the results distribution."

Some other recent studies have tested the ability of mutant or reassortant strains of H5N1 to replicate and spread in animals, but the findings were less dramatic than Fouchier's.

For example, the December issue of the Journal of Virology includes a study in which researchers at St. Jude Children's Research Hospital in Memphis inserted the hemagglutinin gene from a 1997 Hong Kong strain of H5N1 virus into a pandemic 2009 H1N1 virus. When this hybrid was grown serially in human lung epithelial cells, it became more pathogenic in mice, "suggesting that these viruses may easily adapt to humans and become more virulent," the report says. The authors concluded that natural reassortment between circulating 2009 H1N1 and H5N1 viruses could lead to viruses with increased pathogenicity in mammals.

Dr. Lynn Enquist, the Journal of Virology's editor-in-chief and a member of the NSABB, told NPR that the journal's staff carefully considered whether to publish the paper. He observed that the type of reassortant the scientists created could occur in nature.

Another recent study, by researchers from the CDC and Scripps Research Institute, suggested that it would take a series of complex changes for H5N1 viruses to achieve airborne transmissibility in ferrets.

They determined that a particular strain of H5N1, when modified with certain mutations from previous pandemic viruses, could spread in ferrets by direct contact but not by airborne respiratory droplets, according to their report in Virology. Only when they inserted a neuraminidase gene from a human flu virus did this virus achieve a modest ability to spread via respiratory droplets.

"The complex genetic changes required by a clade 2.2 H5N1 virus to reach a low level of transmissibility in ferrets would indicate that considerable functional evolution is still required for acquisition of transmissibility in humans," the report concludes.
