Comments on Severity of Latest Cases of #H7N9: Merely Days From Onset to ICU

When observing and updating my list tonight, I have noticed a few cases which seem to be very extreme.

For example, below the onset set was 3 days before being admitted, and 6 days before being admitted to the ICU.

A 78 year old woman from Ningbo City,Zhejiang Province, who became ill on 17 January. She was admitted to hospital on 20 January and transferred to another hospital on 23 January. She is currently in a critical condition.
Below are two cases of exposure to a poultry market.  So we know they got their source from there.  We do not know when they were there last.   Either way, the time span from onset to severity is only a short few days.

 A 23 year old woman from Hangzhou City, Zhejiang Province, who became ill on 20 January. She was admitted to hospital on 22 January and is currently in a serious condition. The patient has a history of exposure to live poultry market.

·          A 82 year old woman from Hangzhou City, Zhejiang Province, who became ill on 16 January. She was admitted to hospital on 17 January and is currently in a critical condition. The patient has a history of exposure to a live poultry market.

Like the first case above (78F),  this 73 yo case also has no explanation of exposure.  And again, only a few days from onset to admission: 2 days.  And then only 4 days to ICU.  A total of 6 days from onset to ICU. 

   A 73 year old man from Shaoxing City, Zhejiang Province, who became ill on 18 January. He was admitted to hospital on 21 January and is currently in a critical condition.

A fairly young person (33 years old) with only 2 days from onset to admisson, and currently in ICU.  You can't get to the hospital much sooner than that.

A 33 year old farmer from Shaoxing City, Zhejiang Province, who became ill on 17 January. He was admitted to hospital on 19 January and is currently in a critical condition. The patient has a history of exposure to poultry.

And lastly, I believe the WHO has made a type in their update listing.  Specifically, the case below should be 76 years old, as the Ministry of Health out of Zhejiang Province has stated.

 A 78 year old woman from Ningbo City,Zhejiang Province, who became ill on 17 January. She was admitted to hospital on 20 January and transferred to another hospital on 23 January. She is currently in a critical condition.

    1, patients Yang Moumou, female, 76 years old, retired, Yuyao people. January 23 confirmed human infection of H7N9 avian influenza. Is now in critical condition, in Hangzhou, a hospital for treatment.

WHO Human infection with avian influenza A(H7N9) virus – update 1/25/14

25 January 2014 - On 25 January 2014, the National Health and Family Planning Commission of China notified WHO of 10 additional laboratory-confirmed cases of human infection with avian influenza A(H7N9) virus.



Details of the cases are as follows:

·          A 58 year old man from Changping District, Beijing, who became ill on 12 January. He was admitted to hospital on 16 January and transferred to another hospital on 24 January. He is currently in a critical condition. The patient has a history of exposure to poultry.

·          A 78 year old woman from Ningbo City,Zhejiang Province, who became ill on 17 January. She was admitted to hospital on 20 January and transferred to another hospital on 23 January. She is currently in a critical condition.

·          A 44 year old woman from Ningbo City, Zhejiang Province, who became ill on 17 January. She was admitted to hospital on 20 January and is currently in a serious condition. The patient has a history of exposure to poultry.

·          A 23 year old woman from Hangzhou City, Zhejiang Province, who became ill on 20 January. She was admitted to hospital on 22 January and is currently in a serious condition. The patient has a history of exposure to live poultry market.

·          A 82 year old woman from Hangzhou City, Zhejiang Province, who became ill on 16 January. She was admitted to hospital on 17 January and is currently in a critical condition. The patient has a history of exposure to a live poultry market.

·          A 73 year old man from Shaoxing City, Zhejiang Province, who became ill on 18 January. He was admitted to hospital on 21 January and is currently in a critical condition.

·          A 62 year old woman farmer from Shaoxing City, Zhejiang Province,  who became ill on 15 January. She was admitted to hospital on 20 January and transferred to another hospital on 21 January. She is currently in a critical condition. The patient has a history of exposure to poultry.

·          A 33 year old farmer from Shaoxing City, Zhejiang Province, who became ill on 17 January. He was admitted to hospital on 19 January and is currently in a critical condition. The patient has a history of exposure to poultry.

·          A 58 year old woman from Huizhou City, Guangdong Province, who became ill on 10 January. She was admitted to hospital on 16 January and transferred to another hospital on 23 January. She is currently in a critical condition. The patient has a history of exposure to poultry.

·          A 52 year old poultry salesman from Meizhou City, Guangdong Province, who became ill on 14 January. He was admitted to hospital on 20 January and is currently in a critical condition.
 

The source of infection is still under investigation. So far, there is no evidence of sustained human-to-human transmission.

China reports another #H10N8 case

NANCHANG, Jan. 26 (Xinhua) -- Health authorities in east China's Jiangxi Province Saturday confirmed a second human case of H10N8, a new strain of bird flu known to affect humans.
The patient, a 55-year-old woman in Nanchang, capital of Jiangxi, is in a critical condition, the provincial health and family planning department said in a statement.
The woman, surnamed Zhang, developed symptoms of sore throat, dizziness and asthenia on Jan. 8 and was admitted to hospital on Jan. 15, it said.
An investigation showed that she once had exposure to an agricultural market, said the statement.
All those who had come into contacts with her have not shown any abnormal symptoms.
Health experts called on the public to stay away from the live poultry and to have good hygiene habits.
On Dec. 17, local health authorities in Jiangxi confirmed the first human infection of H10N8, days after a 73-year-old woman died from the virus. Health experts then said H10N8 existed among birds, but human infection had never been reported before.
Meanwhile, many regions, including Zhejiang, Shanghai, Guangdong, Jiangsu and Beijing, have reported dozens of cases of H7N9, another strain of the bird flu.

East China's Zhejiang Province reported three new H7N9 bird flu cases on Saturday, bringing the total number of human infections there to 47.
Zhejiang, which has seen new H7N9 cases for 17 consecutive days, is home to the largest number of human infections among all provincial-level regions so far this year.
The three patients are a 49-year-old male worker from the city of Huzhou and two female farmers, aged 48 and 60 respectively, from the city of Ningbo.
All of the three, who were confirmed to have been sicken by the H7N9 virus Friday, are in severe conditions.
Also on Saturday, Jiangsu reported one more H7N9 case, bring total infections in the eastern province to four.
The patient, a 57-year-old man from Suzhou, is in a serious condition.

Vietnamese Man Dies of #H5N1 Bird Flu

A 52-year-old man in Ho Chi Minh City has become the first person in Vietnam to die from bird flu in nine months.

Vietnamese health officials said Tuesday the man was hospitalized January 11 with a high fever and died a week later. He tested positive for the H5N1 virus.

Earlier, the man reportedly killed a duck and came into contact with chickens at a market where several birds later died of unknown causes.

It is Vietnam's first bird flu death since April, when a four-year-old child died.

#H7N9 China Hangzhou closes live poultry markets to combat bird flu

Hangzhou, the capital of eastern China's Zhejiang province, has suspended the trading of live poultry in order to halt the spread of the H7N9 bird flu virus.
The city banned the trading in its main urban areas on Friday, according to local health and industrial and commercial departments.

Staff from the city's disease prevention and control department will sterilize the closed markets, and live poultry such as chickens, ducks and other birds will be collected, inspected and treated, according to local health authorities.

The city also will closely monitor domestic fowl at poultry farms, as well as the habitats of migratory birds, authorities said.

"Local urban management departments also will increase the supervision of residents who raise poultry," said Wang Boli, an official with Hangzhou's industrial and commercial department.

"In addition, we will expand daily patrols in case peddlers try to sell live poultry illegally," he told China Central Television.
On Friday, the province reported seven new human H7N9 cases, bringing the province's total to 44, the highest in the nation. 

The province has logged new H7N9 cases for 16 consecutive days.
Officials from the disease prevention and control department said contact with live poultry and exposure in live markets are the major ways the virus's spread.
In Zhejiang's next-door neighbor, Shanghai, similar measures are planned during the Spring Festival holidays and beyond. 

The city has reported three new cases of H7N9 bird flu since Monday, including two deaths: that of a 31-year-old doctor and a 77-year-old farmer.
The city has reported a total of eight cases so far this year.

According to the municipal government, live poultry markets will be closed from Jan 31 to April 30. 

China is set to celebrate one of its most important holidays, Spring Festival, starting next week. To usher in the Lunar New Year, Chinese residents traditionally dine on freshly butchered poultry during the holiday season, which usually lasts several weeks.

"Newly butchered chicken and ducks are the irreplaceable dishes at the family feast each year," said a woman surnamed Yang.

"To prohibit the live poultry trade makes me feel uneasy. But it's understandable — after all, health and safety are the main thing."

Medical experts say that so far, H7N9 bird flu cases have been scattered and there is no evidence to believe that the virus can be spread from person to person.
Experts also suggest that people be vigilant about hygiene and avoid contact with live poultry.

CDC: Testing Recommendations for Persons with possible infection with Avian Influenza A (H7N9) virus in the United States

Patients who meet both the clinical and exposure criteria described below should be tested for avian influenza A (H7N9) virus infection by reverse-transcription polymerase chain reaction (RT-PCR) assay using H7-specific primers and probes. Decisions on diagnostic testing for influenza using RT-PCR should be made using available clinical and epidemiologic information, and additional persons in whom clinicians suspect avian influenza A (H7N9) virus infection also should be tested. For more information on laboratory testing of persons under investigation for avian influenza A (H7N9) virus infection, please see Information for Health Professionals and Laboratorians. Guidance on testing, treatment, and infection control will be updated by CDC as more information becomes available.
Clinical Illness Criteria
  1. Patients with new-onset severe acute respiratory illness requiring hospitalization (i.e., illness of suspected infectious etiology that is severe enough to require inpatient medical care in the judgment of the treating clinician).
    AND
  2. Patients for whom no alternative infectious etiology is identified.
Exposure Criteria
  1. Patients with recent travel (within 10 days of illness onset) to areas where human cases of avian influenza A (H7N9) virus infection have recently become infected1 or where avian influenza A (H7N9) viruses are known to be circulating in animals2.
    OR
  2. Patients who have had recent close contact (within 10 days of illness onset) with suspected3 or confirmed cases of human infection with avian influenza A (H7N9) virus. Close contact may be regarded as coming within about 6 feet (2 meters) or within the room or care area of a person with a suspected or confirmed case while the person was ill (beginning 1 day prior to illness onset and continuing until resolution of illness). Close contacts include healthcare personnel providing care for a person with a suspected or confirmed case, family members of a person with a suspected or confirmed case, persons who lived with or stayed overnight with a person with a suspected or confirmed case, and others who have had similar close physical contact, especially without the use of respiratory protection.

  3. Persons with an unprotected exposure to avian influenza A (H7N9) virus in a laboratory setting.

Two charts by way of an update on cases of infection by the Middle East respiratory syndrome coronavirus (MERS-CoV)...which shows that there is not much to update from my last charts on Jan-17 hereand here.

The main feature of these two charts is that cases have plateaued. There has been little recent reporting of new MERS-CoV infections...from anywhere.. for some weeks.

#H7N9 Hong Kong CHP: DH urges travellers to take precautions against avian influenza during Lunar New Year

     With the Lunar New Year around the corner, a spokesman for the Department of Health (DH) today (January 24) urged members of the public who are planning to travel outside Hong Kong during the festive season to take precautions against avian influenza.
     The spokesman said, "In view of the three imported human cases of avian influenza A(H7N9) confirmed in Hong Kong and multiple cases reported in various provinces and municipalities on the mainland recently, the activity of avian influenza virus is expected to be higher in the winter season. Maintaining good personal and environmental hygiene at all times is thus of paramount importance to prevent infection."
     As severe infection can result in respiratory failure, multi-organ failure and even death, the spokesman appealed to all travellers to remain vigilant and take heed of the following preventive measures:
Before your trip:

* If you have flu-like symptoms (e.g. fever, cough, sore throat and shortness of breath), postpone your trip until you have recovered.
* Bring along adequate face masks and alcohol hand-rub.
During your trip:
-----------------
* Do not visit live poultry markets and farms. Do not have direct contact with poultry, birds and their droppings. If contact has been made, wash your hands thoroughly with soap and water.
* Cook poultry and eggs thoroughly before eating.
* Avoid crowded places and contact with fever patients.
* If you develop flu-like symptoms, put on a face mask and consult a doctor as soon as possible.

They should wash hands with liquid soap and water properly, especially when they are dirtied by respiratory secretions, e.g. after sneezing, and clean hands with alcohol-based hand rub when they are not visibly soiled.

**Correction** On #H7N9 Case List

I want to thank a fellow "tracker" from flutrackers.com.  Thru a comment here on this blog, they have helped me by correcting a case that I had put down as "discharged".   In error, I had the wrong case.
The two cases are related.  They are the Father and Daughter in Guangdong.  I mistakenly thought the Father had been discharged.  It was the Daughter.  I will correct my list.

Today, we had 7 new cases added in Zhejiang.  One was in Paojiang.  Below is an excerpt on that area.  What is more interesting, is that the patient is 33 years old, and in critical condition.
Paojiang Industrial zone is located in the Yangtze River Delta Economic Zone. It is only 200 kilometers from Shanghai and 20 kilometers from Xiaoshan International Airport. Basic infrastructure including water, electricity, gas, road, telecom, and sewage disposal all meets the demands of various kinds of investment projects.
In 2010,it was upgraded to a nation-level economic development zone with the approval of the State Council.

Another case reported today, is of a 23 year old in severe condition.

      Zhejiang Provincial Health and Family Planning Commission January 24 briefing, the province today added seven cases of human infection with H7N9 avian influenza. 
      1, patients Yang Moumou, female, 76 years old, retired, Yuyao people. January 23 confirmed human infection of H7N9 avian influenza. Is now in critical condition, in Hangzhou, a hospital for treatment. 
      2, patients with a certain song, female, 44 years old, individuals, now living in Ningbo Yinzhou District. January 23 confirmed human infection of H7N9 avian influenza. Now the condition is severe, in Ningbo, a hospital for treatment. 
      3, the patient Yu Moumou, female, 23 years old, staff, Xiaoshan District, Hangzhou people. January 23 confirmed human infection of H7N9 avian influenza. Now the condition is severe, treatment in a hospital in Hangzhou. 
      4, patients Zhou Moumou, female, 82 years old, retired, Hangzhou city people. January 23 confirmed human infection of H7N9 avian influenza. Is now in critical condition, in Hangzhou, a hospital for treatment. 
      5, patients Zhou Moumou, male, 73 years old, retired, Shaoxing City people. January 23 confirmed human infection of H7N9 avian influenza. Is now in critical condition, a hospital for treatment in Shaoxing. 
      6, the patient Jiang Moumou, female, 62 years old, farmer, Shaoxing Bridge District people. January 23 confirmed human infection of H7N9 avian influenza. Is now in critical condition, a hospital for treatment in Shaoxing. 
      7, the patient Ren Moumou, male, 33 years old, farmer, Shaoxing Paojiang people. January 23 confirmed human infection of H7N9 avian influenza. Is now in critical condition, a hospital for treatment in Shaoxing.
