WHO Update: Ebola haemorrhagic fever in the Democratic Republic of the Congo

26 December 2008 -- The Ministry of Health (MoH) of the Democratic Republic of the Congo declared on 25 December an outbreak of Ebola haemorrhagic fever in Mweka District, Kasai Occidental province based on laboratory results from the Centre International de Recherches Médicales de Franceville (CIRMF) in Gabon. CIRMF confirmed the presence of Ebola virus in 2 samples from 2 of the patients in the outbreak by antigen detection. In addition, laboratory tests conducted by Institut National de Recherches Biologiques (INRB) in Kinshasa, also confirmed the presence of Shigella.

As of 26 December, WHO is aware of 34 suspected cases including 9 deaths (CFR 26%) associated with the ongoing event. Additional samples have been collected and are en route to INRB.

The WHO Country Office, Regional Office and Headquarters are supporting the MoH in Kinshasa and in the field at the location of the outbreak. In addition, the WHO Country Office is supporting the operational costs of the investigation and response teams and has deployed four vehicles to the field. Additional staff, outbreak response equipment and supplies, including Personal Protective Equipment (PPE), are also being sent to the area. An enhanced team of national and international experts is being mobilized to implement control strategies for Ebola haemorrhagic fever and to support outbreak field response in the province.

WHO is working together with laboratory partners to provide comprehensive laboratory services, and to support the MoH in the control and investigation of the outbreak.

WHO is unaware of any reports signaling the international spread of disease and advises against the application of any travel or trade restrictions upon the Democratic Republic of the Congo .

WHO Update: Cholera in Zimbabwe

26 December 2008 -- As of 25 December 2008, a total of 26 497 cases, including 1 518 deaths, have been reported by the Ministry of Health in Zimbabwe. Cases are now being reported from all 10 of the country's provinces. Harare, particularly Budiriro suburb in the south west, accounts for the majority of cases, followed by Beitbridge in Matabeleland South and Mudzi in Mashonaland East. The current outbreak is the largest ever recorded in Zimbabwe and is not yet under control. In fact, the epidemiological week ending 20 December saw over 5 000 new cases - an increase in the number of weekly cases relative to previous weeks - and an increase in deaths outside treatment/health centres.

The overall Case Fatality Rate (CFR) has risen to 5.7% - far above the 1% which is normal in large outbreaks - and in some rural areas it has reached as high as 50%. Mortality outside of healthcare facilities remains very high. This is a clear indication that better case management and access to healthcare is needed - in particular an increased use of oral rehydration therapy with Oral Rehydration Salts in communities very early after onset of the disease.

The outbreak has taken on a subregional dimension with cases being reported from neighboring countries. In South Africa as of 26 December, 1 279 cumulative cases and 12 deaths (CFR of 0.9%) had been recorded, with the bulk of the cases (1 194) in the Limpopo area. Cases have also been reported in Botswana (Palm Tree).

The current situation is closely linked to the lack of safe drinking water, poor sanitation, declining health infrastructure, and reduced numbers of healthcare staff reporting to work. Other current risk factors include the commencement of the rainy season and the movement of people within the country, and possibly across borders, during the Christmas season.

WHO, together with the Ministry of Health and partners from the health and Water and Sanitation clusters, has established a cholera outbreak response coordination unit in order to strengthen the reporting and early detection of cases, improve the response mechanism and access to healthcare and ensure proper case management. WHO has also deployed experts in public health, water and sanitation, logistics and social mobilization. In light of the extent and pace of expansion of the outbreak, reinforcing all control activities across the country is critical.

Given the current dynamic of the outbreak and the context of the collapsed health system, a cholera vaccination is not recommended. Moreover, the use of the internationally available WHO prequalified oral cholera vaccine is not recommended once an outbreak has started due to its 2-dose regimen and the time required to reach protective efficacy, high cost and the heavy logistics associated to its use. The use of the parenteral cholera vaccine has never been recommended by WHO due to its low protective efficacy and the occurrence of severe adverse events.

In controlling the spread of cholera WHO does not recommend any special restrictions to travel or trade to or from affected areas. However, neighboring countries are encouraged to reinforce their active surveillance and preparedness systems. Mass chemoprophylaxis is strongly discouraged, as it has no effect on the spread of cholera, can have adverse effects by increasing antimicrobial resistance and provides a false sense of security.

‘‘We have imposed the prohibitory orders to prevent entry of smuggled chickens and eggs into Orissa through Balasore route. The Section 144 will be in force till the end of February,’’ Collector Alekh Chandra Padhiary said Friday adding that the immunisation of the live birds in the district was on.

Within a fortnight over 5,000 chicks, transported illegally, have been seized in the district and destroyed by the police and veterinary officials.

‘‘The chicks were traced from separate vehicles after some people sneaked into the district with the stock from the neighbouring state. The chicks however have been culled,’’ said Chief District Veterinary Officer (CDVO) RK Dutta.

Balasore has 1,74,497 birds, including 21,297 chicks. Of them 15,100 birds have been vaccinated, he added. Prohibitory order has been imposed in border blocks of Bhogarai and Jaleswar in the district following reports of the dreaded bird flu spreading its wings in more areas of WB. The administration has also tightened security at the fishing harbours to prevent smuggling of affected chickens and poultry products to Orissa through sea routes.

NEW DELHI, Dec. 26: The Centre is monitoring the situation in West Bengal and Assam where culling operations are on following outbreak of Avian Influenza. It has sent expert teams and medicines.
According to sources, the flu has spread to new districts in Assam. The fresh areas to be hit by the virus are Tihu- Barama of Baksa district, Chachaligaon Burachowk village in Nagaon district, Rangamati, Aurpakmoniari Tiniali in Kamrup (Metro) district and Doloigaon Uzanpara in Bongaigaon district, the sources said.
Apart from active surveillance and regular monitoring of the affected areas on daily basis following the review of the situation by the health minister, Dr Anbumani Ramadoss, regular culling of birds is being carried out to contain the flu from spreading.
As many as 32,495 birds have been culled in and 210 animal health workers put under chemoprophylaxis in Malda district of West Bengal following the outbreak of Avian Influenza.
Active surveillance is continuing in 0-3 km and 328 health workers have been deployed for active surveillance since the Centre notified the outbreak of the disease in English Bazaar block on 15 December. The state government has adequate stock of ventilators, PPE and Oseltamivir, according to an official release here.

‘The government has directed the Bangladesh Rifles to step up anti-smuggling drive in the border areas and also asked the deputy commissioners of 64 districts to reinforce the activities of the respective anti-smuggling committees’, he said.
Meanwhile, the department of livestock services has culled some 945 poultry birds including 128 backyard poultry on December 23 as fresh bird flu virus has been detected in backyard poultry at sadar upazila in Kurigram and a commercial poultry farm in Gaibandha.
The government has also decided to step up surveillance at all the poultry farms across the country to properly monitor infection of deadly viral disease (H5N1) in poultry birds.
The ministry concerned has formed a 16-member monitoring committee headed by fisheries and livestock secretary incorporating the health and family welfare ministry, city corporation, livestock services department and different private sector organisations in it.
The committee will sit in a regular meeting every week and discuss the bird flu situation in the country, the official said.
Besides, a two-member committee will be formed at every union parishad to be headed by its chairman as convenor and member as the member secretary, said the ministry official.
The committee will inform the surveillance team of information on any kind of diseases contaminating the poultry farm in their respective areas.
The government also has a plan to provide training to 2,000 poultry traders at some 300 kitchen markets in the Dhaka city to create awareness among them of handling of chickens.
The convenor of the Bangladesh Poultry Co-ordination Committee, a platform of five poultry-related organisations, Moshiur Rahman, told New Age that although bird flu situation in the country is not so grave as that of India, the government should immediately introduce an action plan to fight bird flu.

Blood samples from patients in the Western Kasai Province in central Democratic Republic of Congo that were sent to laboratories in Gabon have tested positive for Ebola hemorrhagic fever. Thirty -three people suspected of suffering from Ebola, including nine people who have died, have been reported since November 27. Additional blood and stool samples have been taken for testing.

A number of new patients suspected of being infected with the Ebola virus are being followed up. MSF is closely following a patient with fever at a health center in the village of Kaluamba .

An MSF team of nine Ebola specialists from Kinshasa and Brussels is currently working in Western Kasai. Logisticians are building an isolation ward in the village of Kampungu and putting in place all protection measures. The medical team is also providing care to all people suspected of having the disease. There is no cure for Ebola, so supportive and palliative care is provided.

In order to better understand the epidemic and to prevent further propagation of the virus, the MSF team is also identifying people who have been in contact with others infected with the Ebola virus. In case of new deaths, it will be crucial to ensure that bodies are buried properly, in order to prevent contamination.

An additional MSF doctor, a specialist in in hemorrhagic fevers, is departing tomorrow from Brussels and a health promoter will also reinforce the team from Kinshasa . MSF is considering dispatching additional specialists if needed.

The compound is a component in many plastics, adhesives, glues, laminated products such as plywood, cement, cleansers, fire-retardant paint, and more. But melamine does not always remain where it is placed. For example, melamine in plasticware may be leached from the product by acid and thus can migrate into food, though — at least as measured — not in amounts considered toxic. Fortunately, melamine-containing plasticware becomes discolored or fractures, so it is used less often than it once was.

A greater concern is that melamine is frequently added to crop fertilizer, from which it is absorbed into the soil and then, most likely, into crops themselves, though any uptake has been largely unmeasured. Thus, there is melamine in many products throughout the world, and we do not know what problems it may cause in the future.

A further problem is that melamine food contamination is more pervasive than was originally thought. Since melamine is in animal feed in China, it has now been detected in eggs; it has also been found in wheat gluten and other foods. After the discovery of the melamine contamination of pet food, a detection method involving liquid chromatography–mass spectrometry became widely available and reliably identifies both cyanuric acid and melamine. A number of suspect foods from China tested by the FDA were found to contain melamine (see table), and more are being reported around the world each week. Furthermore, the FDA has found trace levels of melamine in several U.S. infant formulas and, as of the end of November, states that 1 part per million is permitted.

The U.S. Food and Drug Administration again is cautioning pet owners about feeding their dogs chicken jerky treats manufactured in China, as the agency continues to receive reports of kidney-related disorders in animals that ate these products.

The FDA has received 151 complaints about dogs falling sick after eating chicken jerky products since September 2007, when the agency issued its warning. Twenty-six of those pets died.

Centre to set up four new labs to test bird flu samples

Coimbatore (PTI): Centre has decided to set up four new laboratories in the country to detect avian influenza virus, a top official in Union Animal Husbandry said on Friday.

"Centre has appointed a United Nations agency for this purpose. The tender process for setting up the labs would be completed by February 2009 and work expected to commence by June 2009," joint secretary of Animal Husbandry and Fisheries Arvind Goshal said at an interactive session on Bird Flu here.

Such samples cannot be tested in ordinary labs. At present the country has only two laboratories, one at Bhopal and the other at Bangalore, to test bird flu samples.

All the laboratories, including one in Namakkal in Tamil Nadu, were likely to become operational by the middle of 2010, Goshal said.

This year saw outbreak of bird flu in 15 districts of West Bengal and two districts in Assam and Tripura in June. The disease re-occurred again in West Bengal and Assam in November-December.

Bird flu spreads to six new areas: 4,16,758 poultry culled so far

GUWAHATI, Dec 25 – The Central Animal Husbandry, Dairying and Fisheries department has confirmed the spread of the virus of the dreaded bird flu to six new areas of the State, highly placed sources in the State Veterinary and Animal Husbandry department here said. Sources told this correspondent here today that the new areas included Chalchaligaon Burhachawk village under Barhampur Development Block of Nagaon District, Bardulipara village under Tihu-Barama Development Block of Baksa District, Monahkuchi village under Hajo Development Block and Rangamati and Ourpakmoniaritiniali villages under Chayani-Barduwar Development Block of Kamrup district.

Moreover, the Central Government has also confirmed the outbreak of the disease among the poultry of Doloigaon Uzanpara village under Dhaligaon Development Block of Bongaigaon District.

While culling of poultry has started in the three km radius area of Chalchaligaon Burhachawk today, similar operations in the three km radius areas of the other newly notified bird flu-affected areas will start from tomorrow, said the sources.

With the new areas, bird flu disease has so far affected the poultry populations of 18 locations of the State since November 27. Culling and dis-infection operations have so far been completed in nine locations of Rajabazar, Patgaon, Sarpara, Bejerajalah, Kathla Katajhar, Khanapara, Kharghuli and Dibrugarh town.

Cleaning and dis-infection operations in Boitamari Kheluwapara and Bajali Sarihasakla are expected to be completed by day after tomorrow, while due to resistance from the local people, culling operations are yet to be completed at Nilibari village of Chirang District, said the sources.

Till yesterday evening, 4,16,758 poultry were killed, while 1,60,317 eggs and 12,553 kgs of poultry feed were destroyed during the anti-bird flu drive in the State, said the sources.


State to take up post-operative surveillance to check bird flu

: The Animal Husbandry and Veterinary Department (AHVD) will conduct post-operative surveillance on fowls in areas from three to ten kilometers of Meghalaya's side of Khanapara on December 29 to collect samples to see if bird flu has spread to local poultry from Assam.

According to AHVD Director Dr Daurupathan Khonglah, the exercise is required in view of the recent culling operations conducted by Assam in areas on the inter-state border. "Post-operative surveillance will be done to collect samples from local poultry and check if there is any difference between them and the samples we had collected earlier," he said.

Dr Khonglah said there was no report of bird flu among local poultry and that people could consume them without any fear. He, however, said the ban on import of poultry would continue till further orders.

MALDA, Dec. 25: Fresh chicken deaths were reported from Kaliachak II block yesterday even as the animal resources development department started post culling surveillance in English Bazaar and Old Malda Bird Flu affected villages. Villagers buried more than a dozen dead backyard poultry after reporting the deaths to the vets.
The deputy director, ARD, Malda, Mr N K Shit admitted the death reports but said: “We checked the dead birds, there is nothing unusual.” According to him, post culling surveillance will continue for two months. ARD officials will keep vigilance in four phases fortnightly. Villagers will not be allowed to rear chicks in the affected zones for three months.
The deputy director added that the block level committees would prepare the list of beneficiaries for compensation of Rs 500 per family. “The list of beneficiaries will be forwarded to the government for allotment of funds after approval from the committees,” he added.

Washington, Dec 26 : A new study by researchers at the Australian Biosecurity Cooperative Research Centre for Emerging Infectious Disease (AB-CRC) has shown that global disease hot spots, where new and unknown plagues are most likely to erupt, spread in an arc to Australia's north.

The researchers said that Northern Australia is surrounded by a "ring of fire" where new human and animal plagues are likely to erupt without warning.

"We're in the front line for outbreaks of diseases like SARS, bird flu, Nipah virus, enterovirus 71 and chikungunya which infect both people and animals. Such diseases are most likely to emerge in our region," News.com.au quoted AB-CRC chief executive Dr Stephen Prowse, as saying.

"The study highlights the vital importance of developing and maintaining effective disease surveillance in our region as often the only defence against a new disease is to spot it early, before it spreads.

"However it also offers scope to anticipate where new plagues could arise," he added.

Dr Peter Daszak and colleagues of the Consortium for Conservation Medicine in New York, a partner of the AB-CRC, conducted the study of global disease hotspots.

"We've developed an earthquake zone map for emerging diseases. It shows which countries are most likely to be on the front line of an emerging pandemic," he said.

"We now have a way to predict the places from where the next HIV, SARS or avian influenza is likely to emerge," he added.

He said a risk map showed Australia was surrounded to the north by countries that were disease hot spots.

"This means Australia needs to be forward thinking in its approach to biosecurity risk.

"New and emerging diseases rarely have any form of treatment and death rates can be quite high as in the case of Nipah virus which kills about half of those who contract it.

"The best course is to stamp out the bushfire before it takes hold and spreads." he added.

All provinces and cities had been asked to implement epidemic control where avian flu had been found in poultry and strive for the early detection of suspected H5N1 infection to ensure immediate quarantine and control, he said.

The strict control of visitors arriving through border gateways from places where avian flu had occurred was also sought.

Health ministry figures show that five people have been infected with the H5N1 virus this year and all died.

Although the last death was in April, scientists warn that the deadly virus could return.

Viet Nam has diagnosed 106 H5N1-infected patients in 36 provinces and cities since the virus was first reported in December 2003. Of these, 52 died. — VNS
