Investigation and preventive measures will be done for 30 days started from Monday (30/3) to neighborhood 500 meter around victim’s house.

Bird flu H5N1 virus has killed 3 people in Riau since 2007. Two fatalities reported in 2007 at Kecamatan Rumbai and Marpoyan Damai; and one fatality in 2009 at Kecamatan Lima Puluh.

CAIRO, April 3 (Reuters) -

An Egyptian toddler has contracted the highly pathogenic bird flu virus, the latest in an upswing of cases in the most populous Arab country, state news agency MENA said on Friday.


The case brings to 62 the number of confirmed cases of the H5N1 avian flu virus in Egypt, which has been hit harder by bird flu than any other country outside of Asia and has reported seven human infections since March 1.

The 21-month-old boy, Hassan Gamil Hassan Mohamed, is from the province of Beheira in northern Egypt and was in a "good" condition after being treated with the antiviral drug Tamiflu, MENA reported.The new infection came several days after a two-year-old boy from the same province contracted the virus.MENA did not say how Mohamed contracted the virus, but most Egyptians who have fallen ill with bird flu are believed to have contracted the virus from infected household birds.

Egypt is one of the few countries affected by bird flu that does not compensate farmers when poultry is destroyed, though many experts say this is the best way to ensure the rapid detection of new outbreaks.

Since 2003 the H5N1 avian influenza virus has infected at least 410 people in 15 countries and killed 254 of them. It has killed or forced the culling of more than 300 million birds in 61 countries in Asia, the Middle East, Africa and Europe.

Some 23 Egyptians have died after contracting the virus, most after coming into contact with infected domestic birds in a country where roughly 5 million households depend on domestically raised poultry as a significant source of food and income.

While H5N1 rarely infects people, experts say they fear it could mutate into a form that people could easily pass to one another, sparking a pandemic that could kill millions.

Two-year-old Israa Saad Abdel-Shafi is the 60th case of the H5N1 strain of avian flu virus recorded in Egypt since the first outbreak of the disease in 2006 and the ninth victim since the start of the year. Twenty-three out of the 60 have died, the majority young girls or women from rural areas who raise fowl domestically.

In early March the World Health Organisation (WHO) called for an investigation into why Egypt is seeing increasing numbers of victims. The WHO has identified the country as being at the forefront of the spread of the disease though Abdel-Rahman Shahin, official spokesman to the Ministry of Health, points out that despite the high number of victims the percentage of fatalities from bird flu in Egypt is still less than in other countries that have seen outbreaks of the disease.

Egypt has reported nine cases since the beginning of the year, with zero fatalities. There were eight cases with four fatalities in 2008. Vietnam has reported two fatalities, and China seven victims with four fatalities. Shahin argues the figures indicate the disease is being controlled in Egypt.

"This year we do not have any fatalities whereas last year it was 50 per cent. The fatality rate in China this year is more than 50 per cent, and in Vietnam it is 100 per cent. The situation in Egypt is improving, not deteriorating."

Bird flu presents a massive challenge to the government given the uncertainty about the course of any possible pandemic. The virus has become endemic and it could take years to rid the country of the most virulent strain. The government, says Shahin, has prepared a detailed national plan to be implemented in case of a pandemic according to which half of Egypt's 150,000 hospitals beds will be reserved for avian flu patients.

Hamid Samaha, head of the General Authority for Veterinary Services, says the government is focussing on comprehensive public health efforts -- monitoring outbreaks in order to identify any change in the virus, increasing anti-viral stockpiles and building a more robust capacity for vaccine production.

Settlement patterns in Egypt, where 95 per cent of the population is concentrated on five per cent of the land, has serious implications for any pandemic outbreak. The virus has been found at nearly 300 sites and is now "rooted in the Egyptian environment" says Shahin, who now believes that following the first incidents of avian flu Egypt should have culled all poultry and taken draconian measures to stamp out the continued domestic rearing of birds.

"The whole country must resort to consuming frozen chicken instead of purchasing fresh birds. We cannot afford to implement such a scheme at the moment, but the least we must do is ensure that in cities people abide by the regulations and limit direct and indirect communication with poultry."

Shahin suggests the mortality rate resulting from bird flu infections is decreasing due to growing awareness in the wake of information campaigns conducted by the Ministry of Health.

The WHO is still concerned that human infections have escalated over a relatively short period of time, leading some experts to accuse the government of inadequate planning. Health officials, they say, are endeavouring to enforce preventive measures but are incapable of ensuring they are strictly implemented.

"Government planning is random. It is using the wrong vaccine to combat the virus," insists Talaat Khatib, professor of veterinary medicine at Assiut University. "American scientists have already confirmed the H5N1 virus has evolved into two genetically distinct strains, potentially increasing the risk to humans." Khatib believes public awareness campaigns to date have been too weak and Egypt should have begun to plan its preventative measures when the virus was first reported in the country in February 2006.

In Egypt, where poverty and illiteracy rates are high and urban rooftop and backyard rearing of poultry has long been a way of life, a more systematic approach to monitoring the disease needs to be put in place, says Samaha. "People do not respect instructions from the authorities. They consider poultry capital for which it is worth risking their health, and the health of their neighbours."

Egypt is one of very few countries affected by bird flu that does not offer compensation to farmers when poultry is destroyed. Since 2003 412 human cases of the virus have been reported in 15 countries, resulting in 256 deaths. More than 300 million birds have been culled worldwide.

The novel HA cleavage sites in the recently released H5N1 clade 7 sequences from northern Vietnam are cause for concern. The isolates from Vietnam are most closely related to clade 7 sequences from Shanxi and Hunan in China. All have the same basic cleavage site REGGRRKR. However, the two isolates from Vietnam have added an R to produce a cleavage site that is REGGRRRKR. This additional basic amino acid may make the H5N1 more virulent and expanded tissue tropism, because the cleavage is facilitated by tissue specific proteases and the extended cleavage site may offer additional specificities. Moreover, the most recently released sequence, A/chicken/Vietnam/NCVD-03/2008 has duplicated the firts two positions to produce an even larger cleavage site, REREGGRRRKR, which may be linked to even greater virulence and tissue tropism. This isolate also has an upstream 3 BP deletion, although neither recent isolate from Vietnam has the 3 BP deletion identified in the clade 7 isolates from Hunan and Shanxi.

This increase in genetic instability is likely linked to recombination driven by vaccine escape. The latest WHO update on vaccine targets notes that the two isolates from Vietnam were antigenically distinct and both were chosen as clade 7 vaccine targets, even though both came from 2008 Vietnam isolates. Selection of clade 7 vaccine targets is limited by the withholding of clade 7 sequences. China has yet to release sequences from the January cases, even though the sequence of A/Beijing/1/2009 was discussed in media reports in January. The WHO update was glaringly silent on this case, which was the first in the series of nine confirmed cases in China in January, which abruptly stopped in February.

China has acknowledged that the poultry outbreak in December, 2008 was clade 7, which created vaccine resistance problems, leading to new vaccinations in adjacent areas. However, the human cases were widespread throughout China, raising concerns that vaccine resistant H5N1 extended beyond provinces adjacent to Jiangsu. This is supported by the recent clade 7 sequences from Vietnam, which have a large number of non-synonymous changes that would compromise prior vaccines. Moreover, it is not clear that clade 7 is included in vaccines in Vietnam, which would facilitate H5N1 spread and instability.

To facilitate intensified collaboration between Animal and Health Departments both at Central and State levels in the event of the outbreak of the disease, the Ministry of Health and Family Welfare has stock-piled one million doses of the drug Oseltamivir (also known as Tamil Flu) as well as requisite quantities of personal protection equipment (PPE), he said.
The Serum Institute of India has been entrusted to manufacture pre-pandemic and pandemic vaccines to face any eventuality in case there is any extensive outbreak of avian influenza.
In the aftermath of the outbreak of avian influenza in Maharastra in February 2006, the WHO has designated the National Institute of Virology Pune as the Regional Reference Labo- ratory.
Moreover, the Ministry of Health and Family Welfare has been conducting integrated disease surveillance in public health institutions and laboratories, Dr Shiv Lal said. On the other hand, the Government of India has already trained Rapid Res- ponse teams at Central and State levels.

The workshop discussed in detail the possible steps that may be taken up by media when the highly pathogenic H5NI virus infects thousands of people and the situation reaches a pandemic stage when there are large human casualty, schools are closed, movement of people restricted and even cremation of dead bodies is prohibited.
The media persons conducted group discussion and made presentations to the Ministry and UNICEF to implement their observations.
The meeting agreed on formulating a strategy to create awareness among the people extensively on the issue of avian influenza.
It further discussed on the media acting as catalysing agents towards changing the attitude of backyard poultry farmers, producers, traders and sellers regarding the disease.
Taking part in the workshop Chief of Communication, UNICEF India Country Office Angela Walker said that FAO and WHO as UN’s leading technical agencies for animal and human health, have the lead roles in communicating technical information on the progression of virus. However, because children account for nearly half of all reported human cases and impact of avian influenza goes well beyond the immediate risk to their health, UNICEF has an obligation to advocate for the specific needs of children. Further adding the roles of international agencies such as UN as a part of broader strategy for prevention and containment of avian influenza, it aims to communicate relevant information at various stages of AI and pandemic preparedness.

Wind spreads disease faster than thought

CORVALLIS, Ore. (Map, News) - Plant and human diseases that travel with the wind can spread more quickly than previously believed, according to a study at Oregon State University and elsewhere. The findings cause concern not only for some human diseases but also a new fungus that threatens global wheat production.

The research concludes that invading diseases do not always progress in an orderly, constant rate and that some airborne pathogens can actually accelerate as they move.

"It's now becoming clear that some types of diseases can spread more rapidly and widely than we anticipated," said Chris Mundt, a professor of plant pathology at OSU. "This makes it especially important, in some cases, to stop a spreading disease quickly if you hope to stop it at all."

The studies explain, in part, how West Nile Virus spread so rapidly across the United States when experts had been expecting a slower movement.They help analyze the progression of some historic disease problems, such as the potato blight that led to the Irish potato famine of the mid-1840s. And they suggest that a new fungal wheat pathogen that emerged a few years ago in Uganda may be a bigger threat to world production than first thought.

Mundt, an international expert on pathogens of several important food crops, has studied wheat stem rust for years.

"If we didn't have crops that could resist wheat stem rust, we pretty much wouldn't have a wheat industry," Mundt said. "From this pathogen we've learned a lot about plant disease resistance in general.

"And this new study confirms that it is crucial to get prepared for the rapid spread of a new variety of wheat stem rust that appeared in Uganda in 1999."

The new wheat stem rust, Mundt said, could attack 75 percent of the world's known wheat varieties, and in a bad year might cause up to 50 percent crop losses in some areas.

"We don't want to suggest that the sky is falling, but major losses could occur if the right set of conditions converges," Mundt said. "This is something that we shouldn't take a chance on. It's already spread to Iran, and the new research shows that its global spread may be about to pick up speed."

He said there is little time to waste. "This wheat disease problem could be global within a few years," Mundt said. "We would be foolish to ignore it."

Most plant and animal diseases spread by contact or proximity tend to move in a fairly predictable way, researchers say. But some carried by wind-carried spores or migrating birds can spread rapidly.

From 2004-06 the avian bird flu spread across parts of Africa, Europe and Asia through migrating birds. From New York City in 1999, the West Nile Virus spread across most of North America within three years.

"It was surprising to see how closely the spread of very different plant, animal and human diseases followed the same mathematical relationship," Mundt said. "This is giving us a better ability to predict how various types of diseases may move, and hopefully prepare for them."

The study was published recently in The American Naturalist, a professional journal.

State health officials want the public's views on deciding who should get vaccinated — and who should not — against the next flu pandemic that reaches the Islands.

So far, a wide-ranging advisory group of about a dozen community members representing religious groups, the chamber of commerce and even an expert in mediation have been working to set up a system that will help determine who will be inoculated first and who will have to face the pandemic unarmed, at least until more vaccine can be manufactured and shipped to Hawai'i.

The final determination will rest with Gov. Linda Lingle.


"The decision is with the governor," said Lingle spokesman Russell Pang.

There is no way to stockpile vaccine for an unknown virus that has yet to arrive. And no way to calculate how much vaccine will be needed, how many people will be infected and how many could die, according to Bill Gallo, senior management official with the Centers for Disease Control and Prevention in Honolulu.

So it could take months before federal officials isolate the virus, create a vaccine for it and send it to Hawai'i.

By then, untold lives could be lost.


"It is a very sobering scenario to be thinking about," said Jim Tollefson, president and CEO of the Chamber of Commerce of Hawai'i who sits on the advisory committee. "But it's best to be prepared and educated."

The 20th century saw three pandemics and several more "pandemic threats" that ranged from the Spanish Flu that killed 50 million people worldwide and at least 1,300 people in Hawai'i between 1918 and 1920, to the modern-day threat that the avian flu could spread from Asia to the Islands.

Several hundred people were infected with avian flu in Asia, six died and more than 1 million chickens were slaughtered in an attempt to control the spread of the disease.

"If (the next pandemic) is not here now, it will be here someday," said Dr. Vivek Nerurkar, interim chairman of the department of Tropical Medicine and Infectious Diseases at the University of Hawai'i's John A. Burns School of Medicine. "When a pandemic comes to Hawai'i, it's going to affect a considerable amount of people, especially the elderly and children."

Nerurkar is not involved with the planning and preparations to decide who will be vaccinated first but supports the effort by state health officials.


"It's very important to involve the community," he said. "They are the people who are being affected. It is a huge task. It is not a simple task. We need to have a consensus. ... It should not be a unilateral decision."

To that end, the Health Department plans a public education campaign that will begin next month to explain the project and get community opinions.

"We're responsible for making science-based decisions all the time," said Dr. Sarah Y. Park, state epidemiologist and chief of the Disease Outbreak Control Division. "Without neglecting the scientific basis for our state's pandemic planning, this project gives us a unique opportunity to focus on the values-based aspects of some of our decision-making. We encourage everyone to participate in the public activities and let their voice be heard."


Park is the principal investigator for Hawai'i's pandemic priorities grant.

Hawai'i's position as a gateway to and from Asia makes any potential pandemic of particular concern since air travel could quickly spread the disease. To guard against such problems, a disease surveillance and quarantine system was begun at Honolulu International Airport in 2005. It was the nation's first such system, and it screens sick passengers as they arrive for signs of serious illness.

Hawai'i and five other states have been chosen to participate in a federal program to carry out further pandemic planning.


The advisory group has found itself working on a subject with more variables than facts, such as when the Islands would be touched by a pandemic, or global disease outbreak.

"Is Hawai'i on the front end or the back end of an epidemic?" Gallo asked. "It's an important issue. If it was later in the epidemic, there could be enough vaccine to vaccinate everyone."


Even without a pandemic, deciding which flu strain to vaccinate against every year relies on estimates.

"The vaccine has to be reconfigured each year to accommodate the newly evolving strains," Gallo said. "It's very difficult to do. It requires a meeting of the minds about what kind of flu to expect for the next year. ... A pandemic would set the wheels in motion for getting the exact strain isolated and reproducing a new vaccine in large qualities. You would first need a portion of the actual germ or bug to produce the actual vaccine."

Members of the advisory group, such as Elizabeth Kent, director of the Center for Alternative Dispute Resolution, are focusing less on the science and more on the values that Hawai'i wants to protect during a pandemic.

"We want to be careful and considerate of everybody's rights," she said. "What we're talking about is the fabric of society and how you keep the fabric as intact as possible. You may have some special hobby that contributes so much to the fabric of our society or have a job that does. Trying to take all of that into account is very important. That part has been for me quite challenging, thinking about how to make those decisions."

In the event of a severe pandemic, federal guidelines from the U.S. Department of Health and Human Services and U.S. Department of Homeland Security recommend that the first people inoculated should be "deployed and mission critical personnel,"
public health workers, inpatient healthcare providers, outpatient and home health providers, first responders such as paramedics, firefighters and police, manufacturers of pandemic vaccine and antivirals, pregnant women and children under age 3.

But Hawai'i health officials want to solicit a broad variety of opinions to put together a list of people — or, more likely, job categories and demographic guidelines — of who should be inoculated first.

"It's a very interesting question: Who's going to get the vaccine in the event of shortages?" said Jason Seaborn, who is working on the project for the state Department of Health. "We're trying to elicit information from the public to see what values are specific to Hawai'i residents. As you work through each group, it raises questions such as, 'Should you vaccinate morgue workers? What are you going to do when the bodies start piling up?' "


In an e-mail, Seaborn wrote that the Health Department wants to "get feedback from Hawai'i's residents on what they value (i.e. — canoe carvers? Single moms? Elderly? Young? Teachers? Garbage collectors?)"

Gallo also sits on the advisory group and believes "it says a lot for the state of Hawai'i how serious they take this kind of planning and want to hear community input. They want to make sure their unique needs are heard in Washington."

But ultimately, the person who decides the priority list most likely will be Hawai'i's governor, Gallo said.


"Are we playing God?" asked Kent, a member of the advisory group. "No. Somebody else is going to make the hard decision. But we're trying to make a very hard decision that much easier in a thoughtful way."

# Three live one-hour broadcasts on KHNL in April. A panel of speakers will discuss who should get vaccinations during a pandemic and solicit viewer opinions on air and via e-mail. The shows are tentatively scheduled from 8 to 9 p.m. April 15, 22 and 29.
