No bird flu mutation as tot fights on

No bird flu mutation as tot fights on

The bird flu virus that left a two-year- old boy fighting for life in hospital has not mutated but health experts warn Hongkongers not to let their guard down.
Experts said the quick detection of the case means Hong Kong has the right system to catch in midtrack the deadly H5N1 avian flu - seen as a potential pandemic strain even before H1N1 swine flu emerged in 2009.
The boy, who caught the virus in Guangdong, remains in serious condition in the pediatric intensive care unit at Princess Margaret Hospital.
Doctors gave him the antiviral drug Tamiflu in a bid to save his life.
He was admitted for five days to a general pediatric ward at Caritas Medical Centre and treated for obstructive hydrocephalus - or water in the brain - before the H5N1 diagnosis.

He was then transferred to Princess Margaret Hospital and put in isolation while his Hong Kong father and mainland mother were quarantined.
Hong Kong health officials have asked the Guangdong authorities to monitor the boy's grandmother who lives with the family.
Children in the Caritas ward were among 80 people put on medical surveillance for 10 to 14 days. Others include doctors and staff at a private clinic in Mong Kok the boy visited and the ambulance workers who transferred him.
The toddler caught bird flu when a duck that his mother bought in a Guangzhou wet market was slaughtered in front of them.
He also has an "atypical" presentation of hydrocephalus, which puzzled doctors, said Malik Peiris, professor of virology at the University of Hong Kong School of Public Health.
Gene sequencing analysis announced last night by the Centre for Health Protection found the H5N1 that infected the boy belongs to the dominant strain - clade 2.3.2.1, the same one as isolates from wild birds detected this year and last, and in an imported human infection case in late 2010.
University of Hong Kong professor of virology and gene sequencing expert Guan Yi said the strain has been detected in Hong Kong, the mainland, Mongolia and Eastern Europe.
The center added that the genes are of avian origin and there is no evidence that the strain is resistant to Tamiflu.
Peiris said: "I don't think we can remain relaxed, we should remain vigilant about H5N1.
"We know this virus remains endemic and established in poultry in many countries all the way to Bangladesh, India, Indonesia, Vietnam Cambodia, Guangdong and other provinces, infecting poultry and humans."
Although the virus can be transmitted to humans, this one case "does not dramatically change the situation one way or the other," Peiris said.
Bird flu measures in Hong Kong are still effective and people do not have to worry about eating poultry within the territory, he added.
"But people should be careful of going to live poultry markets when they travel overseas especially to endemic areas."
Nasopharyngeal aspirates taken from the boy's parents and five contacts who reported respiratory symptoms tested negative for influenza A (H5). Health authorities have received 20 inquiries from the public on hotline 2125-1111.

Southern News boys condition released yesterday, the Hong Kong Department of Health Centre for Health Protection confirmed avian influenza: the boy was still in the Princess Margaret Hospital for isolation and treatment, the situation remains serious. Guangdong Province Center for Disease Control said 26 contacts within Guangdong no fever and other flu-like symptoms, and incidence from the boy has been 11 days, so that the urban area of Guangzhou low possibility of human avian influenza cases. Hong Kong Ji boy still serious 2-year-old boy returning from Guangzhou recently been diagnosed to be infected with influenza H 5N 1 (avian influenza). Yesterday, Hong Kong Department of Health Centre for Health Protection on the boys' condition released. The boy is still Princess Margaret Hospital for isolation and treatment, the situation remains serious.
In order to prevent the spread of the epidemic, the Centre for Health Protection of Hong Kong Department of Health continue to contact who had contact with the boy, including two Caritas Hospital medical staff, they presented with respiratory symptoms but nasopharyngeal aspirate specimen influenza H 5 was negative.
The center epidemiological survey found that the boy had 20-22 May visit to the mainland, Anhui Province, during which there is no direct contact with poultry. He worked in mid-May to the live poultry market in Guangzhou near their homes. Hong Kong Centre for Health Protection has issued a letter to the districts of Hong Kong doctors to remind them to pay special attention to the patients served markets or contact with live poultry in Guangzhou.
Yesterday, the Southern reporter called Anhui Province CDC. Anhui Center for Disease Control Communicable Disease Control Branch official said, Because the boy is healthy, in Anhui Province and the absence of disease, so they do not have this epidemiological survey.
Guangzhou community human to human transmission risk is extremely low Yesterday, the chief expert of the Guangdong CDC, infectious disease prevention and control of the director Jianfeng said, boy fever has passed two weeks, in close contact with young boys as of yesterday 26 people who have not developed symptoms of fever symptoms Experts believe that the community of bird flu transmission risk is very low in the urban area of Guangzhou.
Jianfeng introduced, 26 may be exposed to avian influenza virus of stakeholders, including: clinic for boys over the Guangzhou Children doctors and nurses, doctors and nurses, as well as South Park markets in the Second Hospital of Zhongshan poultry stall owners, the boy's grandmother and other . General avian influenza infection in humans, the incubation period ranging from one day to seventy-eight days, mostly in the infection of two to four days onset. As the boys a fever on May 23, is now 11 days, nearly two weeks, 26 people are fever and other flu-like symptoms, therefore, provinces, municipalities, disease control experts after research, basic very low risk of transmission of bird flu transmission in human cases of avian influenza that is, urban people in the urban area of Guangzhou.
"Guangdong has been in the life bird flu monitoring this year, the current six months, also found no positive samples of human avian influenza, so the boy was infected with the avian influenza cases should be the case." Jianfeng said.
Guangzhou City CDC Renwang Ming said, dozens of close contacts in Guangzhou is not infected. Health professionals on the child's grandmother, contact your child's doctor, Meat Market staff have carried out human avian influenza throat swab paper testing and blood testing, the results were negative, CDC staff will continue to conduct dynamic monitoring. "
Macau flu level temporarily enhance Hong Kong's immigration boy infected with bird flu cases, and cause Macau alert authorities. Macao Health Bureau said yesterday that it found no birds or bird flu, temporary does not enhance the detection of immigration will not enhance influenza-level. Macau for nearly a week has been a collective of 10 influenza or EV 71 enterovirus infection, including prisons, nurseries, nursing homes, primary schools and so on.

Where the disease is still in doubt Past cases of human avian influenza in late autumn, winter and early spring temperatures lower, so why the boy will be the incidence in the summer heat? Guangzhou CDC Renwang Ming said, the all the incidence of human avian influenza situation in China's performance for the dissemination of its seasonal high incidence of properties is not obvious, is not surprising that similar cases in the summer heat season. "Before the child to be in Guangzhou, have been to Anhui, what and where disease is still in doubt."
Suspected the infection source is a duck Because boys had a history of live poultry exposure, ducks, and disease control experts suspect that the source of infection is a duck. Source of infection had more birds, chickens, ducks and other poultry infected with avian influenza virus different?
He Jianfeng, chief expert of the provincial CDC, said this case the source of infection may in Mainland China, Guangzhou is a high index of suspicion. "Ducks are not the same as with the other birds infected with avian influenza virus not develop the disease, it is the avian influenza virus reservoir host, is living with HIV. Part of the ducks infected with avian influenza virus does not die, while chickens infected with avian influenza virus will soon be die. "
Government initiatives
Southern Reporter yesterday learned from the Guangzhou Municipal Bureau of Agriculture At present, Guangzhou has organized veterinary experts to the prevalence of avian influenza in Hong Kong poultry markets boy visited to conduct epidemiological investigations, and epidemiological investigations within the city, and in the city's deployment to further strengthen the major animal disease prevention and control work. As of 18:00 yesterday, has not received the poultry unusual deaths and the outbreak of highly pathogenic avian influenza. June 2, the Guangzhou Municipal Bureau of Agriculture staff first boy been to Guangzhou Haizhu District, South Park Avenue farmers market survey from the monitoring of the situation, the market is still relatively stable. "He also said farms and wholesale markets, liquidity is relatively large, a larger volume, although the relevant departments in monitoring, but slip through the risk is relatively high. As for the boy whether the incidence of contact with poultry in Guangzhou, still under investigation.
Yesterday, the relevant departments of the Municipal Bureau of Agriculture has begun the farmers market traceability survey, Guangzhou City Animal Quarantine within has launched emergency investigation plan, all staff must be available 24 hours a waiting list. Yesterday morning, in three columns of the relevant departments of the Municipal Bureau of Agriculture, visited the the farmers market in the Haizhu District Baiyun District Panyu District, two large poultry wholesale market sampling, although the suspected incidence of boys had contact with ducks. However, due to the chicken to be highly susceptible and a larger volume, and blood sampling has begun to poultry such as chickens and ducks, especially the farmers market, Haizhu District, the sale of all poultry will be sampling. "From the current monitoring efforts, large-scale outbreak of avian flu is basically impossible." Experts say the Guangzhou City, in accordance with the relevant experience of the past sampling survey, and then carry out traceable, if the final diagnosis, will enter Fight link, but the outbreak of the highly unlikely.

Hong Kong authorities have confirmed that a two-year-old boy has been hospitalized with the bird flu. Although the bird flu alert level has been raised to “serious”, health authorities are urging people not to panic.
The toddler is from Guangzhou in southern China and had been running a fever for weeks before his family took him to a private clinic in Hong Kong on May 26. After testing positive for the H5N1 Influenza A virus, he was placed in intensive care in the Princess Margaret Hospital. His parents have been quarantined there as well, although they tested negative for the virus.
Now the search is beginning for the source of the infection.
[York Chow, Secretary for Food and Health, Hong Kong]:
"A preliminary investigation showed that the boy went to a poultry market once with his mother. But within a radius of 13 kilometers from their home in Guangzhou, there is no such market. So we have to investigate further."
Protocol dictates that a 13-kilometer radius from the source of the infection will be declared an import control zone. Imports of poultry products will be suspended for 21 days.
Because of the high risk of infection, Hong Kong authorities are checking all those who may have had contact with the toddler. He spent five days at the Caritas Medical Centre before he was diagnosed with the bird flu and transferred to the Princess Margaret Hospital. Two healthcare workers there began to show symptoms, but later tested negative.
The bird flu is a potentially deadly virus with a 60 percent mortality rate, according to the World Health Organization.  http://english.ntdtv.com/ntdtv_en/news_china/2012-06-04/Toddler-Diagnosed-With-Bird-Flu-In-Hong-Kong.html

Further updates on H5N1 case

The Centre for Health Protection (CHP) of the Department of Health today (June 4) provided further updates on the Influenza A (H5N1) case involving a 2-year-old boy.

Further tests conducted by the Public Health Laboratory Services Branch of the CHP on the Influenza A virus isolate from the nasopharyngeal aspirate taken from the boy showed that the H5 gene of the isolate belonged to clade 2.3.2.1, which is the same clade as the isolates from wild birds detected in 2011 and 2012 and in the imported human infection case in late 2010.

So far all the genes characterised belong to avian origin and there is no evidence of resistance to the antiviral agent oseltamivir (Tamiflu).

The boy is currently receiving intensive care in the Paediatric Intensive Care Unit of Princess Margaret Hospital.

Investigation into the cause of his obstructive hydrocephalus is still under way. His current condition remains serious.

The CHP so far has kept some 80 contacts under medical surveillance.

Nasopharyngeal aspirates taken from the parents and the five contacts who reported respiratory symptoms tested negative for Influenza A (H5).

As at 5pm today, a total of 20 public enquiries have been received on the DH hotline (2125 1111).

The investigation is continuing.Ends/Monday, June 4, 2012

Bangladesh Poultry vaccination plan triggers shock

Mon, Jun 4th, 2012 8:58 pm BdST
Nurul Islam Hasib
bdnews24.com senior correspondent

Dhaka, June 4 (bdnews24.com) – The government's move to vaccinate poultry against frequently changing avian influenza virus has 'surprised' experts because globally it is recommended not to use it for public health's sake.

Experts say the available vaccines in the world have been able to reduce poultry fatalities for some time, but the vaccinated birds' capabilities to transmit the disease 'silently' after being infected caused 'massive' human mortalities.

Indonesia, Egypt and Vietnam had been vaccinating poultries but stopped after an influx of human fatalities last year followed when the virus also changed its strain.

Fisheries and Livestock Minister Abdul Latif Biswas on Sunday told journalists that considering poultry farmers' 'long-standing demand' the government planned to 'experimentally' introduce the vaccine.

He said a committee had been formed to finalise the decision to that end.

The Food and Agriculture Organisation (FAO) and the World Health Organisation (WHO) experts say they are 'surprised' by the move.

Mat Yamage, Chief Technical Adviser to the FAO Avian Influenza Control Programme, told bdnews24.com that earlier they 'warned' the government against using vaccines.

He said the situation where the vaccine once used was 'serious' and that 'there is no record of successfully controlling H5N1 with vaccines'.

"We would strongly say it should be discussed with technical experts."

Globally, it is recommended that poultry farms maintain bio-security to prevent the virus and government have measures for early reporting, rapid action, culling and surveillance to control avian influenza outbreaks.

Yamage said once vaccinated, birds would not show the flu symptoms early.

"It means the detection would be very difficult. But we need to detect it as early as possible to contain its spread."

According to him, many people died even after Indonesia, Egypt and Vietnam –top three avian influenza-burdened countries – started vaccinating poultry.

"It's because of silent infection," he said, "finally they had to stop vaccination."

The FAO expert said they had a meeting recently with Indonesia, Egypt and Vietnam and discussed the problems. "They are concerned about how to get rid of their problems."

He said bio-security was the most important measures to control avian influenza, but Bangladesh was not practising it.

Last year Bangladesh witnessed 171 bird flu outbreaks in poultry. This year so far it recorded 21 outbreaks but Yamage said it did not mean the situation improved.

"We found sick birds in the market. Trucks were found at check points carrying sick birds.

"It means outbreaks are happening but remain under-reported."

Prof Mahmudur Rahman, Director at the Institute of Epidemiology, Disease Control and Research (IEDCR), told bdnews24.com that during last month's conference in Ho Chi Minh City, Vietnam's experts told them 'not to start vaccination.'

He said as four flu viruses –H1N1, H5N1, H3N2 and H9N2— was circulating in Bangladesh, silent infections might be 'devastating'.

"Once (the four viruses) reasserted themselves, it will be a big threat."

WHO influenza expert Dr ASM Alamgir said they never recommended a country to vaccinate its poultry against H5N1.

"It's not the way to combat H5N1," he said, 'the virus is changing.'

"Even in Bangladesh the present clade 2.3.2.1 is new. It was detected last year. Since 2007 clade 2.2 was circulating in Bangladesh."

He said there is no vaccine in the world for the clade that currently circulates in Bangladesh.

Coordinator of One Health Bangladesh Nitish Chandra Debnath said the vaccine would not be 'effective' in Bangladesh.

"The virus has already changed its strain," he said, and "it will continue to change."

"If it (the vaccine) is effective, then why would we oppose it?"

He said the vaccine was not working but creating problems instead and added that it should be technically evaluated.

The government has confirmed six human infections after the first case of H5N1 influenza was detected on May 22, 2008. http://www.bdnews24.com/details.php?id=225833&cid=13

Boy triggers bird flu alert

2012-06-03 09:54

SOUTH China's Guangdong province is on high alert after a Hong Kong hospital confirmed it has admitted a two-year-old boy infected with the H5 strain of avian influenza.

The boy was taken to hospital on May 28 after arriving from Guangdong's capital city of Guangzhou. He tested positive for the H5 strain.

He was confirmed to have been in contact with a live duck between May 17 and 19 at a farmer's market in Guangzhou. However, tests conducted at the market, those nearby and a hospital in Guangzhou that admitted the boy, found no signs of H5, said He Jianfeng, director of the Epidemic Studies Institute of the Guangdong Disease Control Center.
He said the province has launched an avian influenza response plan.

The clinic yesterday as usual, the nurses on a reporter's question to answer is not clear.Dr Chow said, whether it is in Shenzhen earlier cases of human infection or the AFCD dead bird testing about 20 samples, DNA arrangement there is no major gene changes, I believe that this number of cases is also transmitted from poultry. The Government hope that the study find boys routes of infection, with the Guangdong health authorities to a higher understanding of when and where the possibility of infection to determine the source before deciding on the scope of virus control.Dr Chow: gene variation should be avian-human transmissionDr Chow said the boy sick immigrants but served antipyretics, so border crossings may not be detected in the body temperature test. He said that the daily temperature checks will detect 10-20 high body temperature of passengers returning from overseas, Hong Kong people, border officers will check whether they are particularly medical records, including whether contact with poultry, or from special infectious diseases places. The majority of patients if no special symptoms, will be treated as normal influenza, requiring them to seek medical treatment immediately and seek care. The microbiologist cases of avian flu occurred in May and June, relatively rare, but Dr Chow said in recent years, a maximum of avian flu cases is hot in Indonesia and Egypt, Vietnam recently also the case, I believe that no direct relationship with the season. He said cases of human infection cases not directly related to temporary and poultry, it is temporary there is no need to the mainland for Hong Kong chicken farms to take measures.

26 close contacts of the boy had no flu-like symptoms

For those in close contact with the disease control department will continue the monitoring of health status.
Do not rule out a separate case "Although the boys before the onset of live poultry exposure history, it can not determine in the markets of South Park Avenue to buy duck infected, the source of the boy's virus also further investigation." Jianfeng said that, in addition, contact with the boy with suspicious symptoms, do not rule out that he was only a single case has not yet been found.

Boy Confirmed with Bird Flu in Hong Kong

2012-06-02 23:14:03

Hong Kong health authority said on Saturday that a two-year-old boy has been confirmed with H5N1 Influenza A and urged the public not to panic.
The Center for Health Protection said the boy's condition has changed from stable to serious after testing positive for the deadly avian influenza, while his parents' tests for influenza were negative.
"The boy's parents are all along asymptomatic, which means that the chance of a human-to-human transmission is slim," said a spokesman of the center in a statement.
The boy was put to intensive care treatment while his parents were in quarantine in the same hospital.
A native of China's southern city of Guangzhou, the boy went to Hong Kong with his family on May 26 to seek medical treatment after running a fever for weeks.
After reporting the influenza case, the city government raised its bird flu alert level to "serious" and tightened control on live chickens imports, and strengthened supervision of local farms and markets.
The authority has intensified disease monitoring and infection control in public hospitals. Hospital staff will keep close watch over visiting patients and enforce triage assessment and appropriate segregation at waiting areas in line with clinical guidelines.
Besides, all 30 local chicken farms will be inspected within two days to ensure that their chickens are healthy and biosecurity and health measures are followed strictly.
The Center for Food Safety has contacted with its mainland counterparts in search of the infection source. Once the source is located, an area within 13 km radius will be declared an import control zone. Imports of live poultry and poultry products will be suspended for 21 days in the zone.
"At the moment, it is an isolated case. We feel that there is no need for panic among Hong Kong citizens," said York Chow, the Secretary for Food and Health of the city government, while addressing media earlier in the day
