Early detection of the arrival of either is considered key to response plans to protect native species and human life.

The decision to eliminate the program was a result of the state's budget crunch and a decrease in concern about West Nile virus because of lower illness and fatality rates.

The program was run primarily by the Department of Land and Natural Resources and the Department of Health.

The state still conducts periodic tests for avian flu in live birds, Okubo said. In addition, there is limited funding available to test dead birds, but only if they're native species or are found in a cluster of 10 or more dead birds.

"We do have the capacity to gear up for it if we see a rise or legitimate threat," Okubo said. "It's just that we're not doing it regularly and we're not doing collections of dead birds."

Mutated H5N1 Virus Raises Doubts About the Effectiveness of Flu Vaccines

A genetically mutated avian flu virus is believed to be behind a string of deadly outbreaks in bird populations over the past two years, a scientist said on Tuesday, warning that the new virus had the potential to be more lethal than its ancestor should it infect humans.

Virologist I Gusti Ngurah Mahardhika said the government’s vaccination program with a large number of poultry farms in the past few years may backfire because the mutation was partly suspected to have been caused by an outdated vaccine.

“I have strong evidence that the old vaccine seed [already developed by the country’s vaccine makers] cannot stop the new virus,” said Mahardhika, who is also head of the Biomedical and Molecular Biology Laboratory at the Udayana University’s Veterinary Medicine School.

He said isolated virus samples taken from already vaccinated farms in Java, Sumatra, Bali and Sulawesi from 2008 to 2009 were compared with samples from their ancestor H5N1 virus taken from Legok, Banten, in 2003.

“From the study, we found genetic differences of up to 8.7 percent from the ancestor virus,” he said.

The Legok virus is still being used by most vaccine producers here as the vaccine seed and the recent mutations may mean the seed virus is outdated.

The genetic differences in the virus samples are mostly represented by changes in the acid composition, or RNA [ribonucleic acid], which “have never been found before [in the 2003 virus samples].”

Genetic mutations, according to Mahardhika, could be triggered by either natural causes or vaccine resistance.

In the case of samples he studied, Mahardhika said the changes pointed more to the possibility of vaccine resistance rather than natural causes.

“Flu viruses are highly mutagenic and their RNA structure can quite easily undergo changes, or a deletion. The vaccine also has a masking effect whereby it only provokes immunity in the birds’ respiratory systems although the virus can still be found in the bird’s saliva. So now we can see a seemingly healthy chicken but its droppings and saliva are full of this virus,” he said.

The most recent viral outbreak in poultry populations was reported on Monday from Pekanbaru where thousands of chickens died in several villages. In one of the villages in Indragiri Hulu, two toddlers — aged three years and six months old — are suspected of having been infected.

A Web site jointly developed by Surabaya’s Airlangga University and Kobe University in Japan, www.birdflucorner.wordpress.com, reported that the two neighboring toddlers had had a history of contact with dead chickens.

Indonesia has been the hardest hit by the spread of avian flu, both in humans and poultry. The first outbreaks in bird populations were reported in 2003 with the virus spreading to humans two years later. At least 125 people have died of this bird flu.

So far, human infections can only result from direct contact with dead fowls and there is no scientific evidence that the virus is able to transmit from human to human although suspicions were raised after a few cluster cases were found in which a number of members from the same family had became infected. Simple hygienic practices such as hand washing help prevent infections.

Microbiology clinic professor with the University of Indonesia, Amin Soebandrio, concurred with Mahardhika’s findings, saying he had suspected that such changes would come.

“Research has also been carried out by my students on a similar issue using only 2004 virus samples. And genetic changes are apparent.”

Amin said further study must be done to check whether the changes directly affect the resistance of the virus to the vaccine being used by the government.

“We need to see whether the changes increase the capacity of the virus to infect the human respiratory system,” he said. “If that’s the case, it’s more than likely the virus will also be resistant to the antiviral we are now using to treat patients with avian influenza.”

Tjandra Yoga Aditama, the director general of disease monitoring at the Ministry of Health said the public should not worry too much about these findings because it did not mean a new virus had been created.

“Viruses always have the potential to mutate — that’s common in nature — but these studies don’t meant we’ve found a new strain of virus,” he said.

Memed Zoelkarnain, spokesman for the National Commission for Avian Influenza and Pandemic Prevention, said the findings must be studied further in a bid not to scare people and called on the media to be sensible when reporting on this issue given the findings are preliminary.

“We don’t want to frighten the public,” he said.

Memed, also a veterinary doctor, said Mahardhika should double check with “all vaccine makers” because there was more than one vaccine maker in the country but only a few of them used the 2003 vaccine seed.

British Journal of Health Psychology: How People Behave During A Pandemic

ScienceDaily (Feb. 1, 2010) — A study that looked at how people behave during pandemics has identified key demographic and psychological factors that may predict protective behaviours. The study is published online January 30 2010, in the British Journal of Health Psychology.

Dr Alison Bish and Professor Susan Michie at the Health Psychology Unit, University College London, investigated the results of a number of studies into how people behave during pandemics, such as the recent swine flu outbreak, to better understand protective behaviour and to improve interventions and communication in the future. The review included the results of 26 published studies on associations between demographic factors, attitudes and behavioural measures during outbreaks including SARS (Severe Acute Respiratory Syndrome) between November 2002 and July 2003, Bird Flu (Avian influenza) in 1997, and Swine Flu (in 2009).

Dr Bish said: "These illnesses have far reaching effects because of how easily they are transmitted. When an outbreak occurs however, people can choose to take steps to protect themselves. Protective behaviours can be preventative, avoidant or disease management, such as hand washing, avoiding public places, or taking antiviral medication. We wanted to discover the groups of people that are most likely to take such steps, and the attitudes that are associated with these behaviours."

Many of the studies found significant gender differences in protective behaviours. Studies in Hong Kong and Singapore, the UK and the USA found that women were more likely than men to carry out protective behaviours such as washing their hands, wearing a mask or following quarantine restrictions. Older people were also found to be more likely to carry out such protective behaviours. Dr Bish explains: "These patterns could be explained in terms of perceived risk, with women and older people feeling that they may be more susceptible to disease than men, or younger people do." In fact, greater perceived susceptibility to disease was found to be a strong predictor of protective behaviours in studies carried out in the UK, Hong Kong, Australia and the Netherlands, with those people who felt they were more at risk carrying out more protective behaviours such as good hygiene, vaccination, and disinfecting the home.

In studies investigating SARS and swine flu, greater perceived susceptibility was associated with avoidant behaviour such as avoiding public places. Having a high level of trust in authorities was found to be associated with compliance with preventative, avoidant and management of illness behaviours. Dr Bish explains: "As trust is a key emotion relevant to risk behaviour, people who trust in authorities are more likely to follow their advice. The issue of trust becomes weightier in uncertain situations, making this an important factor in whether people follow government advice during pandemics." Dr Bish concluded: "We hope that this insight into the demographic and psychological variables associated with protective behaviour during a pandemic can be used in future pandemic situations to encourage behaviour that will reduce the spread and impact of disease."

Current research suggests that pandemic H1N1 influenza of swine origin has distinct means of transmission from the seasonal flu, yet does not result in the pathogenic severity of avian flu viruses. The related report by Chan et al, "Tropism and Innate Host Responses of the 2009 Pandemic H1N1 Influenza Virus in ex Vivo and in Vitro Cultures of Human Conjunctiva and Respiratory Tract," appears published online ahead of print in the April 2010 issue of The American Journal of Pathology.

Pandemic H1N1 influenza of swine origin is a novel influenza strain that causes a generally mild respiratory illness, but results in severe disease or death in vulnerable individuals. The World Health Organization reports that "as of 17 January 2010, worldwide more than 209 countries and overseas territories or communities have reported laboratory confirmed cases of pandemic influenza H1N1 2009, including at least 14142 deaths." High risk groups include the very young and old, people with compromised immune systems, and pregnant women.

Unlike seasonal flu, which only infects cells located in the nose and the throat, pandemic H1N1 can replicate efficiently in cells deeper in the lung, similar to the more pathogenic H5N1 'bird flu'. Researchers led by Drs. Michael C.W. Chan and Joseph S.Malik Peiris at Queen Mary Hospital, Hong Kong SAR, China compared the cell infection pattern and immune responses of pandemic H1N1 to seasonal flu as well as to highly pathogenic avian influenza strains. They found that in contrast to seasonal flu, pandemic H1N1 and highly pathogenic avian flu could infect the conjunctiva, a membrane that lines the eyelids and covers the white part of the eye, suggesting an additional route of transmission as well as differences in receptor binding profile. However, pandemic H1N1 did not differ from seasonal flu either in replication in nose, throat, and lung cells or in induction of an inflammatory immune response, which is dysregulated in high pathogenic avian flu infections. Taken together, these results are consistent with epidemiological data that suggest that while pandemic H1N1 has subtle differences in transmissibility and pathogenesis from seasonal flu, it does not induce as severe disease as bird flu viruses.

Chan et al conclude that "the pandemic [H1N1 virus] (but not the seasonal virus) infects conjunctival epithelium, suggest[ing] that the eye may be an important route for acquiring infection with [pandemic H1N1] as compared with seasonal influenza viruses. Furthermore, this observation implies important differences in receptor preference and tissue tropism between the pandemic H1N1 and seasonal influenza viruses, which may have relevance in pathogenesis. - [However,] the 2009 pandemic H1N1 influenza virus is comparable with seasonal influenza in inducing host innate responses and does not have the intrinsic properties of cytokine dysregulation possessed by [the highly pathogenic avian influenza] virus or the 1918 pandemic H1N1 influenza virus." "While generally mild in the majority of cases, the pandemic H1N1 virus is not just another seasonal flu virus and has subtle peculiarities of its own".
