Commentary on Past 4 Months of Events Leading Up to H7N9 Outbreak

I'll take you back to December, and bring you up-to-date on articles out of Vietnam and China, with a few enlightening stories on the side...very interesting events leading up to this outbreak of H7N9.  I have been tracking news in Vietnam for years now, and I don't recall their poultry being laden with antibiotis to the point that they were dangerous to consume.  
These are all excerpts, along with the link to the post here at the Pandemic Information News.

Back to Vietnam, here is a story of a guy, bought pigs slaughtered them, then he died, and they got rid of the pigs..but the townsmen were not happy, they protested...

February 20th - Here's a story on the Migration this winter/Spring:

On March 4, the CDC announced a human vaccine for H5N1 Avian Influenza...
Starting on Friday, the CDC began to provide a vaccine against the H5N1 avian influenza and the service will continue until the end of August, the agency said.
The CDC recommended that people who work in the inspection and quarantine sectors, medical institutes and those who frequently visit countries with reported outbreaks of H5N1 bird flu should get vaccinated against the virus.

And on March 6th, we come full circle back to Vietnam and the Poultry with Antibiotics in them...China has been injecting their poultry for years against H5N1, why is there so much residue in them now that they are unfit for human consumption?  Could there be a new strain evolving?

Bird flu spreads in Vietnam ominous #H5N1 #BIRDFLU

Vietnam found that the first influenza A H3N2 influenza patients 2-year-old girl

U.S. funding anti-bird flu in Vietnam #H5N1 #H7N9

[The Case Fatality Rate of 50% quoted below is for the H5N1 Human Cases in 2012.  According to my list , I have 5 confirmed cases with 3 death.]

U.S. funding for prevention of avian influenza in Vietnam.  Department of Veterinary Medicine, Department of Animal Husbandry, Veterinary Diagnostic Center for Central and regional centers, and laboratories involved are the beneficiaries of funding 1 million 700 thousand U.S. dollars by the Development Authority International, USAID awarded many more.
Announcement of the U.S. Embassy reported this today, which is a new project with funding by USAID and the Food and Agriculture Organization, FAO, done.

Vietnam: New Strain 2.3.2.1 Has Not Yet Been Found in Humans; Unexpectedly Appears in July, Very Uncommon

HANOI, Sept. 14 (Xinhua) -- Bird flu has spread to two more provinces in northern Vietnam as of Sept. 14, making the total number of provinces infected with the disease to seven, local media reported on Friday.
In Tuyen Quang province, more than 17,600 infected fowls had been culled, a reporter of Vietnam's state-run News Agency told Xinhua on Friday.
He said bird flu first appeared in one commune in Son Duong district on Aug. 10, and as of Sept. 7 it spread to other nine communes in this district and Tuyen Quang provincial town.
To prevent the spreading of H5N1 virus, the province's Department of Animal Health has implemented strict measures in response to the epidemics, including establishing a Steering Committee for avian influenza epidemic prevention and control at all levels from district to commune, setting up mobile checkpoints to control illegal poultry transportation, killing infected poultry and stopping all poultry slaughtering, purchasing and transporting activities in the infected areas.
So far, the local authority has supplied 10,000 liters of disinfectant and 70 tons of lime powder to clean those areas having infected poultry. In addition, the national reserve support fund has provided the province with 250,000 doses of bird flu vaccines.
Nguyen Van Quang, a local resident in Yen Son district, which is close to the provincial town of Tuyen Quang, told Xinhua on Friday, that his family raises a flock of about 50 fowls. Thanks to his careful breeding, the flock is safe and growing well.
Meanwhile, in Hoa Binh province, four hamlets in two communes in Luong Son district have 1,140 infected fowls and 848 others died, reported local media on Wednesday. The local authority instructed the culling of all infected poultry and implemented measures to prevent the expanse of the disease.
According to the Department of Animal Health under the Vietnam' s Ministry of Agriculture and Rural Development, as of Sept. 12, bird flu has spread to seven provinces in Vietnam's northern and central regions including Ha Tinh, Ninh Binh, Nam Dinh, Bac Giang, Quang Ngai, Tuyen Quang and Hoa Binh.
In early September, the ministry released news that a new bird flu strain was developing unexpectedly in Vietnam and caused a big concern to the government and public.
According to the National Animal Health Diagnosis Center, the new strain of bird flu virus, which is suspected to have higher risk of causing human death than previously known ones, appeared in July and strongly spread in August in Vietnam's central and northern provinces.
The new strain was thought to belong to H5N1 - Clade 2.3.2.1, which is different from the two strains of A and B that appeared in Vietnam in 2011, due to its higher pathogenic risk.
Apart from the newly-found strain of the virus, the bird flu has developed unexpectedly in the country so far this year. Normally, avian influenza appears shortly before or after the traditional lunar Tet holidays, which often falls in late January and early February. However, it has appeared since July this year and fast spread with unexpected signs.
According to local experts, the epidemic would become more complicated once it reaches its peak in the coming months.
As of mid-September, four cases of A/H5N1 infection had been recorded with two deaths in Vietnam, but the new strain of bird flu virus has not yet been found in humans, reported the Ministry of Agriculture and Rural Development.

Vietnam Add a patient dying of bird flu

This is the third disease virus infection of bird flu in Vietnam since early this year. Two former patients are killed including a young duck in Soc Trang 18 years old and a 26-year-old woman living in Kien Giang province.

Vietnam struggles to fight HFMD, H5N1 flu

The Health Ministry’s Preventive Health Department has asked all local authorities to set up steering boards for epidemic prevention and control to combat Hand, Foot and Mouth disease and H5N1 avian flu that have spread rapidly and killed 11 children so far.

At yesterday’s meeting on epidemic control, Nguyen Van Binh, head of the department, said local authorities should not wait to set up an interdisciplinary board until 24 hours after the declaration of epidemic as currently regulated, since such a delay will affect the efficiency of the fight against the diseases, he said.

The Health Ministry yesterday sent messages to chairpersons of the People’s Committees of provinces and cities asking them to prepare enough manpower, supplies and finances to cope with the epidemics.

Members of the local steering boards must be assigned to specific tasks to prevent and control the epidemic effectively.

The boards should advise all families to ensure hygienic conditions for children’s eating, drinking and playing. It is necessary to keep children’s toys clean and practice regular hand washing among children and their caretakers, the Health Minister said in the messages.

Rapid spread

The department reported that 6,328 HFMD cases had occurred in 60 of the country’s 63 provinces and cities during the first 6 weeks of the year, killing 9 children, and that the ratio of death to total HFMD cases in the period was 0.14 percent, equal to that in 2011, the year with the worst-ever HFMD epidemic.

This year’s epidemic may be more severe than that of last year, since it has broken out sooner - in January-February instead of April or May as in 2011, the department warned.

Meanwhile, after 20 months with no cases of humans contracting the H5N1 flu virus, the disease has re-occurred and spread to 12 provinces and cities so far, affecting 2 people in southern Kien Giang and Soc Trang provinces. Both have died.

Bac Ninh province is the latest that has been affected by the disease, following 11 other localities, including Thanh Hoa, Quang Tri, Ha Nam, Ha Tinh, Quang Nam, Hai Duong, Hai Phong, Thai Nguyen, Bac Giang, Soc Trang and Kien Giang, the Veterinary Department reported.

Vien Quang Mai, vice rector of Nha Trang city-based Pasteur Institute, states that the HFMD cases have increased in all central provinces, especially in Khanh Hoa and Binh Dinh, and killed a people in Da Nang city.

Compared to the same period last year, the number of affected localities this year has increased by three, said Dang Van Ky, head of the department’s Epidemiology Division.

Ky forecast that the number of HFMD patients this year may be the same as last year (110,000 cases), but the death toll may be lower, since there is now a new treatment regimen for the disease.

Flu vaccine not highly effective

Ky warned that the H5N1 avian flu has evolved to have sub-clade 2.3.2, which itself developed into two types A and B, in which the former is three times more dangerous than the latter. Meanwhile, the H5N1-Re 5 vaccine, which has been used to vaccinate poultry in the country for the past several years, has been found too weak to cope with the sub-clade.

In addition, a survey of 30 provinces and cities showed that the average proportion of poultry bearing the H5N1 virus was 4.13 percent, with Ha Tinh taking the lead with 18 percent. These rates were far higher than those of last year, when the average rate was just 1.6-2 percent and the highest was not more than 6 percent.

The Ministry of Agriculture and Rural Development, which has asked the Finance Ministry to facilitate the import of 50 million doses of H5N1-Re 5, has therefore forecast that the country might suffer a large bird flu epidemic this year.

Ky also warned that in the past four years, the rate of mortality among H5N1 flu cases was 100 percent, while in the previous period, the rate was just 50 percent. This situation proves the virus has evolved to be more dangerous.

Experts say greater vigilance and prompt action are needed to prevent the virus from spreading to epidemic proportions

She has never had the fowls vaccinated against bird flu, except once several years ago when local authorities offered free vaccinations for chickens and ducks following an epidemic of the H5N1 virus.
“It’s only a couple of chickens. Many others who breed small number of chickens at their house also don’t have them vaccinated,” said the retired midwife in Long An Province’s Chau Thanh District.
Just before the Tet (January 23) festival, she gave her relative in Ho Chi Minh City two chicken who brought them back alive although it is illegal to transport live fowl into the city. The chickens were later slaughtered for a meal as an offering to deceased ancestors – a traditional ritual conducted on the third day of the lunar year.
The rearing, consuming and gifting of chicken is common practice. This has prompted experts to warn of low awareness of the dangers posed by bird flu that claimed two lives last month - an 18-year-old man in Kien Giang and 26-year-old woman in Soc Trang province.

“Many farmers have ignored the crucial preventive measure of vaccination and often panicked once bird flu spreads,” said Nguyen Xuan Binh, director of the Animal Health Agency No. 6 (The agency monitors animal health issues in HCMC and the provinces of Ninh Thuan, Binh Thuan, Dong Nai, Binh Phuoc, Binh Duong, Ba Ria-Vung Tau, Long An, Tien Giang, Ben Tre and Tay Ninh).
Duc, owner of a large chicken farm in Long An, said only those breeding thousands of chickens at a time like him have their fowls vaccinated because the possible damage is significant.

A vet in Long An, who wanted to remain unnamed, pointed out that it was dangerous for farmers to ignore vaccination or refuse to cull diseased fowls.
“Many farmers go to local veterinary medicine shops to buy bird flu vaccine. “Poultry producers and the general public should always take simple precautions to reduce exposure to the virus from infected poultry,” the World Health Organization (WHO) said in a statement.
“These include extra vigilance in cases of sudden disease and death in poultry, rapid reporting of disease to the authorities and good hygiene practices while handling, slaughtering and preparing poultry for consumption,” the statement said.

“The current vaccine is able to protect poultry against the majority of the viruses in the new clade 2.3.2.1 and also the existing clade 1, if properly vaccinated,” he said, adding that the new vaccine for poultry may take between 6 months to 2 years to develop with all safety and protection parameters completed as per international protocol.
